Arthrogenic Muscle Inhibition (AMI)

In short: when a joint is in an injured state, swollen or inflamed, the body generates a natural protective response, a Reflex Inhibition of the joint surrounding muscles.

The classic example is the AMI of the Quadriceps following knee surgery, trauma or a chronic inflammatory state. When the knee is swollen, you will struggle to build muscle, or to even activate the Quadriceps. This results in the inability of the knee to achieve full active extension and the feeling of instability (the main job of the Quadriceps).
The main goal has to be, to combat this AMI as soon as possible to avoid loosing muscle mass (atrophy). This entails muscle activation exercises (retrain the central nervous system, that activation is actually good, as it often seems counterintuitive, as it can cause some dyscomfort and pain at the start) as well as managing the swelling in and around the joint, to switch off that reflex.

A weight has finally been lifted off of your shoulders - telling the world, you have moved past ACL recovery ages ago, and are dealing with a much bigger issue: an autoimmune disease called Reactive Arthritis.
You never used this as an excuse (not even the muscle tear, which ended the dream of returning to the Games last year). It would have been so much easier to reduce the pressure from the public & sponsors and silence all the haters. But that’s not how you roll; you wanted to tell your story form a perspective of success and not defeat - That is who you are & for this I respect you immensely!
It is easy to focus on all the things that did not go our way, but I have a different perspective. Look at all the things you have achieved: competing at the highest level, producing world class scores, lighting up venues with your performances - All this without an ACL. With so much swelling in the opposite knee, that meant hitting depth was never a question of strength, running or bounding never a question of stability. Without a few consistent weeks of training at full capacity. Battling an autoimmune disease which continually attacks your joints and makes them swell up / hurt bad to even walk down the stairs. The mechanics we can handle & are the easy part, but the random inflammatory flare ups is what creates havoc. Trying to build strength, despite the muscles being inhibited by swelling. An immune system so deprived, that any bug could turn into an infection that floors you for weeks. Above all, the stress all of that creates, making consistent sleep impossible...yet still delivering top tier performances.
Now just imagine what this girl can do with a “normal” functioning system?!
I have been there with you through every step of this insane ride and have witnessed you tackle every day with bravery.
I promise you that this will not change. I will be with you through all the peaks and troughs it will take, to get you to where you belong. My belief in you is unshaken and it has already been an absolute honour to witness this battle of a Champion!
The world has not yet seen Sara Sigmundsdottir at 100% of her abilities - World watch out when she is healthy!

Maria Längfors is a Swedish professional CrossFit Games Athlete, competing on Team CrossFit Prestanda Kriger.

We have just started working together a couple of weeks ago and despite a very tight timeline, we managed to troubleshoot a persistent issue, just in time for CF Games Team Quarter Finals.

I look forward to see all the great things we will achieve, beyond the point of recovery and dive into the world of maximising bio-mechanical potential.

Congratulations to 5x Crossfit Games Athlete , who punched her 6th ticket to the Games at the Oceania NoBull Crossfit Semifinals.

In a very strong women’s field in Brisbane, Aus, Jamie showed a stellar performance and put the finishing touches on her comeback with a very impressive 3rd day, resulting in a 2nd place finish .

I am very grateful for Jamies trust in me & for her to be part of momentum.

What a weekend for Gabi in Berlin.
With 6 out of 7 Events in the Top 5, she won the European Semifinals in a very convincing fashion.

I have been supporting with momentum since Sept 2022, whenever she needed my help. I am proud to play a small part in her journey. I look forward to see you shine this summer in Madison & show the world what you can do.

I got to see this smile, and it made me incredibly happy! This is what i take away from this weekend, because i promise you, you will get to see that inspiring smile a lot more in the future. Why am I so certain of that? Because:
1. Sara is the most remarkable athlete I ever had the pleasure to work with. Tough as nails, her grit, her fighting spirit & composure in the face of adversity & her kindness… and kindness is such a signature of the strong!
2. we have assembled an incredible team around her, and we will continue to create the environment, we believe it takes to unleash Saras full potential. A potential no-one outside the team had the pleasure to witness yet.

The amount of adversities Sara had to face to even make it to the start-line in Berlin is unimaginable, but we won’t make any excuses. Because there is nothing to excuse - she put her best possible effort forward in every single moment - despite facing strong headwinds throughout the competition. That’s all we can ask!
Not many things went our way last weekend, but that’s Sport.
People criticise & draw conclusions purely based on their opinions without knowing the facts. A signature of the weak.
I am incredibly proud of her & so should everyone else be, because you get to witness a legend of the sport, who has inspired hundreds of thousands of people & will continue to do so. There are great athletes, and there are icons & role models, who transcend beyond a specific sport- Sara is both. We win together, we cash in the heavy blows together.

No. 1 Priority

A little over 2 years ago, I have made a promise to Sara: I pledged, that would leave no stone unturned & do anything in my power to to get her back to the top.
Everyone in my care gets my very best I have to offer every day, and I am humbled to get to help a lot of amazing people and athletes.
However the level of detail, effort and commitment that has gone into this quest over past plus 2 years are unmatched & unique, and I will keep it coming for as long as Sara is in the Sport.
I am excited to travel to Berlin on Team Sigmundsdóttir to take on the next stage of our journey.

Elliot reached out to me a few months ago, when we had the difficult decision to make, for him to undergo a surgical procedure first.

The Rehab is after a little over 3mths well underway and going very well. As tough as it was initially, we took this as an opportunity for us to rebuild his body completely, address the underlying root-causes, to come back better & stronger than ever.

“I am back”

A short text from Sara after the individuals at WZA - words, that meant the world to me:

My belief in her has always been unshakable & I always knew she would be back - but now she knew it too. This realisation and seeing the smile on her face made me incredibly happy.

Whatever this season will bring, I am beyond proud of .

For to get through the competition with no pain in her body and a smile on her face matters most to me. Sara did 17 events in 4 days. She hasn’t done volume like this since 2017 and these events were no joke either. I’ve learned not to take this for granted and even though I was hopeful it would turn out this way, I was definitely prepared for it to go sideways.

Sara has been laying low since the Last Chance Qualifier. She’s taken 6 months, pretty much in the shadows. Less social media, less sponsor stuff. Just working ever so hard at all the little things to make sure that once she would return to the competition floor both her body and her mind would be ready for it. She decided to spend the past 2 months in Dubai where her physio Nik Jordan (.online ) is located. He is not only a physiotherapist but also an excellent communicator and someone who has her trust and has become a great friend of ours. The work the two of them do together has in many ways been the key to her building up belief in her body. That the knee injury is in the rearview mirror and that she is ready to rock again. This weekend proved that this is the case.

This was also the first time working in a competition environment with her coach . It was wonderful to see how well they clicked and how well tuned in they are. To clarify any misunderstanding. Sara is very much still with . By living there all of last season and doing so much work with Perrin it was just a very natural progression that she would step in as her main coach.

Whatever the new season holds I can fully state that Sara is happy and feeling great. It’s been a while since I could convincingly say that and it warms my heart to be able to. A happy athlete is a dangerous athlete❤️

Now it’s go-time, but also time to have fun & push the systems.

The season is still a few months away - so now we test on the competition floor, we learn & go back to the drawing board to then peak for the season 2023.

Structure follows Function

Source of pain vs Cause of Pain.

What often goes wrong with treatment of tendonitis/tendinitis: the sole focus on the Tendon, the pain. Tendinitis is almost always reactive, never primary (a few rheumatological inflammatory conditions excluded).
Don’t treat the tendinitis, because you can’t. Treat what is driving the problem and what’s causing it and your body will treat the rest.
Anti inflammatory means will contain it, but never solve it.

Pain is just a signal, not the problem itself.

Movement Efficiency & Quality

Efficiency enables the minimum amount of energy to be used for a given task, whilst maintaining physical integrity.

The goal of physical capability has to be to optimize your movement competency by making your skills as efficient as possible, while also progressively increasing your work capacity (strength and/or endurance).

The result: Better power output, at lesser effort whilst conserving your energy. Not a bad Deal. Now go and do your Accessory!

There are very few things that p☠️💣s me off, but this can drive me up the wall and puts fuel on my fire!

Positive reinforcement, encouragement, reframing problems and finding a strategy out of it - that’s what I do - that’s what creates momentum! People who know me and have worked with me know that I always try to find a way forward, over or around issues. Improvise Adapt Overcome.

Most of all thank you for the trust you put in me as a young Physiotherapist in 2009/2010 when you let me be part of your journey (I still feel partially responsible for the Roland Garros win 😁).
You made me feel at home form the first minute we met, and together with Pierre made me part of the decision making on the first day - which wasn’t intimidating at all…
You showed me the down to earth man you are, which was always what impressed me the most.

Control the controllables. Accept physiology.

Healing is the restoration process of a harmed tissue.
Recovery is regaining full function & control over an activity level or movement.

Healing is defined by the Physiology of the body. Most tissue damages take on average 6-8 weeks to heal, often a little longer to consolidate. Whether you are an athlete or a person of average activity won’t change a thing. It’s nature. Getting upset about this fact won’t speed this up. Yes you can give your body a better framework for healing by having your sleep, food, mindset etc on par - but it will still take 6-8 weeks to heal.

Recovery on the other hand can be sped up dramatically by the amount of effort you put in:
1. What can i work on whilst the structure is healing?
2. Is a structure stabile for movement, but not yet for loading; What functions can I expose it to & improve in the meantime? This will not shorten the time for healing of the injury but dramatically shorten the recovery.

Healing is taken care of by your body, if you look after it, Recovery is taken care of by yourself by exposing the body to the right stimulus.

Healing and Recovery are two very different things. One will happen without you doing anything, the other will not happen without you doing anything.

Hippocrates: “That which used develops, and that which is not used
wastes away. If there is any deficiency in exercise the body will become liable to disease, defective in growth and age quickly. “

Injuries are signs of a problem in a movement, an inefficiency, compensation or load capacity issue.
Pain does not mean damage - our bodies are much tougher than you think.
Being injured, though not fun, is an opportunity to correct such inefficiencies and come back stronger.
The first step: acceptance that we need to take action and analysis of the underlying core problem. Form a plan and execute. Isolate the problem, work around it whilst fixing the issue and come back stronger once we piece it back together.
It doesn’t have to be a struggle - it’s all about perspective.
