Are you in Western Australia and in need of attendant care support to participate in community activities? 🌟 We have great news for you! You can apply for a Companion Card! 🎉 Here's how:

1️⃣ Download the Cardholder Application form from our website. You can find it in PDF format or an accessible Word document. https://www.wacompanioncard.org.au/apply-now/

2️⃣ Fill out the form:

Complete Section 1 with your personal details.
Take the form to a Health Professional or Service Provider, who will fill out Section 2.

3️⃣ Attach two identical high-quality, color passport-sized photographs to Page 10 of the form using a paperclip.

4️⃣ Submit your completed form with photographs by either:

Emailing it to [email protected], or
Mailing it to:

5️⃣ Please allow approximately 20 working days for processing, although this duration may increase during peak periods.

6️⃣ Your application will be assessed against four eligibility criteria, which are as follows:
Being a permanent resident of Australia, residing in Western Australia.
Demonstrating a significant and permanent disability, including issues related to ageing and psychiatric illness.
Showing that you would be unable to participate in most community activities without attendant care support due to the impact of your disability.
Demonstrating that your need for this level of attendant care will be life-long.

7️⃣ If your application is successful, a Companion Card will be sent to you by post. If, unfortunately, your application is not successful, you will receive a letter notifying you of the outcome.

Get ready to kick up your heels and dance the night away at the City of Cockburn's annual Family Dance! This is an all-abilities dance, so everyone can get involved and have a great time.

Everybody loves to have a bbq at the Rockingham foreshore. ❤️ Great to see more accessible pathways in Rockingham for inclusion for all. 🙌

We've completed the first section of the accessible pathway through Churchill Park as part of our access and inclusion upgrades at Rockingham Foreshore. ✅ This pathway now also provides direct access to accessible barbecues.

Works to extend the accessible path through Bell Park will start on 1 May. Please note that the Flinders Lane car park will be closed on weekdays from 6am to 4pm during these works. Alternative parking is available nearby.
