Great week, Bollen Health, working with Gold Coast Primary Health Network - GCPHN to commence the Healthy Ageing Program across the region. Two workshops with 72 enthusiastic GPs, nurses, practice managers and allied health learning to assess their own Muscle Health and where they are on the trajectory of hashtag . Plus, 11 practice visits with GPs, nurses, and the admin team discuss the Healthy Ageing/Frailty Screening QI project. All were surprised and shocked to learn that 20% of their practice population would be living with the long-term condition known as hashtag . Much more prevalent than expected, and much of it can be reversed! Looking forward to continuing this work over the next 8 months and breaking down the barriers to frailty screening in General Practice. - Australian Primary Health Care Nurses Association

Don’t risk a fall. Use ladders safely.

Falls from ladders result in many emergency department presentations each year.

If you’re alone, not physically able to use a ladder safely, have health conditions or are taking medication:
◾ Ask for assistance from family, a friend, or neighbour.
◾ Consider hiring a maintenance person.
◾ Seek assistance with home maintenance through your local council, or disability and community support services.

Recently Bollen Health and Adelaide PHN held the third and final in the series on early intervention for Workshops. Eight very engaged General Practice teams (GP, Nurse and Practice Managers) reflected on their achievements after 8 weeks of learning to screen for people living with frailty in age group 40-70. The big surprise was the prevalence of frailty/prefrailty in 40-49 age group! Wonderful to have Exercise Physiologist Sidney Vanderzalm from AEP Health Group to share knowledge on how resistance exercises are so important to change the trajectory of ageing. An important take home message was how prevalent the use of smart watches, apps and fitness trackers are with 40-70yo age group. This can support the assessment, measurement and motivation of patients for activities. All clinicians can help their patients better understand the use of technology to improve their lifestyle habits. Gamifying and socializing activity helps people initiate and sustain the changes.
Engagement with the community on this topic in improves with the use of better language such as “Muscle Health Checks” rather than “ screening” and offering objective assessment of such as grip strength, timed sit to stands and walking speed tests.
Bollen Health and Adelaide PHN will be working with General Practices on this Quality Improvement topic again in 2025. Expressions of interest will be called for later this year.

Keeping people well and living at home is a vital role of primary care. Early recognition of risk of decline is now possible. Dr Chris Bollen is the MC at the Adelaide PHN and AMPHEaT May 11 Healthy Ageing- Early Intervention Event in Adelaide for GPs, practice nurses and practice managers. Come and hear Geriatrician Professor Sue Kurrle (“Old People’s Homes for 4year olds” Intergenerational Ageing programs on ABC TV) discuss what can be done to support improving our trajectory of healthy ageing. Plus, local General Practices will present the results of their Healthy Ageing early intervention projects to improve outcomes for their patients that are sustainable for the patients and the people doing the work. Two weeks ago,the second Healthy Ageing Workshop in the Bollen Health and Adelaide PHN series on early intervention for Frailty. Eight very engaged General Practice teams (GP, Nurse and Practice Managers) invited members of their Local Medical/Healthcare Neighbourhood to take part in the event and share their knowledge on what can be offered to change the trajectory of ageing. Great night with lead speaker the energetic Dr Tim Henwood Exercise Physiologist and researcher with Southern Cross Care (SA, NT & VIC) Inc reminding us not only of the value of resistance exercises, but also how it does need to feel “difficult” in order for progress to be made, but it should NOT feel like it hurts! Otherwise we won’t do it again! “The value of exercise among older adults is irrefutable” says Tim. Enjoying an activity with another person adds to the overall benefit with the social interaction being an additional key frailty fighter. Tim shared great examples of social prescribing options available across Adelaide.

Great night with lead speaker Judith Lowe sharing stories of success with the wonderful offerings available at the City of Onkaparinga and how they can make an impact for people over 50 looking for social interaction and adding purpose to their lives. The issue of confronting retirement is a big issue. (See slide) Great examples of social prescribing options which are available across Adelaide were discussed and stories of change shared. It’s these personal stories of making a difference that trigger the change in mindset about what is possible for our patients. It’s now two weeks since the workshop and Jane Bollen and I are in regular contact with the GPs and nurses from the practices. They are all now talking about how they have become social prescribers to support people living with/or at risk of low intrinsic capacity (aka frailty 😁).

Last week 9 General Practices commenced a 12 week Quality Improvement sprint with Bollen Health on the topic of identifying at least 20 people aged 40-70 who are prefrail or frail. Adelaide PHN has commissioned Bollen Health as part of the Commonwealth’s Healthy Ageing- Early Intervention program. The first workshop helped participants understand the concept of frailty, trialled a frailty screening tool, and had some playtime in which everyone was able to personally assess their trajectory of healthy ageing using recognised sarcopenia assessment tools such as grip strength, 30 second sit to stands, Timed Up and Go(TUG) and 4m walking speed tests. Bollen Health is looking forward to supporting these vanguard practices.

Last month Dr Chris Bollen and Jane Bollen RN from BMP Healthcare Consulting, on behalf of Country SA PHN , were invited to Clare Bowling Club Inc .
The topic was “Let’s Chat about Healthy Ageing”
There were over 50 participants in the room.
They enthusiastically embraced the topic of what keeps them "living younger longer." Many decided they had to be active participants in their ageing. Some had their assumptions about ageing challenged, whilst others understood it is important to keep doing the things that matter to them. They learnt about the options to assist with reducing their decline as well as how to work more effectively with their health professionals. The session finished with Neil the Paramedic getting everyone to do 5 “sit to stands”, and everyone learnt it is never too late to build muscle.

It’s a free event with afternoon tea provided. July 26 2-4pm at Valleys Function Centre. Supported by Country SA PHN to keep rural and regional communities fit and well. Come and join Jane Bollen and Dr Chris Bollen to ask questions and spend some time chatting about the myths of ageing and understanding what we call all do to improve our experience of getting older.
