Psoriasis is a very common long-lasting skin condition, showing symptoms of red scaly patches and in some cases white ones on the skin too.
The landscape on treatment of people living with chronic moderate-to-severe plaque psoriasis suitable for systemic therapy or phototherapy is changing quickly. On one hand, the arrival of biologic remedies stands among the most impressive advancements in handling psoriasis patients.
๐๐ข๐จ๐ฅ๐จ๐ ๐ข๐โ๐ฌ ๐๐จ๐ฅ๐ ๐ข๐ง ๐ญ๐ก๐ ๐๐๐ง๐๐ ๐๐ฆ๐๐ง๐ญ ๐จ๐ ๐๐ฌ๐จ๐ซ๐ข๐๐ฌ๐ข๐ฌ.
Historically, the management of psoriasis has included topical treatments, phototherapy, and systemic treatments, including methotrexate, cyclosporine, among other agents. Despite their successes, these therapies are accompanied by limitations like systemic toxicity and long-term reactions. However, biologic treatments for psoriasis proved to be game changers in how we approach management of this condition because they offer targeted intervention addressing the actual immunologic mechanisms at play in the disease.
Currently, biologics are considered the initial and preferred systemic therapy for patients with chronic mild to moderate to severe plaque psoriasis who have been considered suitable candidates for systemic therapy or phototherapy. These drugs are usually used in situations where topical treatment and light therapy do not work well enough or cannot be used due to certain disadvantages. Factors such as disease severity, treatment side effects, patientโs choice, and treatment outcomes influence the decision for initiating biologic therapy.
๐๐๐๐ก๐๐ง๐ข๐ฌ๐ฆ๐ฌ ๐จ๐ ๐๐๐ญ๐ข๐จ๐ง
Biologic medications used in the treatment of psoriasis target specific components of the immune system that play a crucial role in the development of the disease. The most common targets include tumour necrosis factor-alpha (TNF-ฮฑ), interleukin-17 (IL-17), and interleukin-23 (IL-23). These cytokines are known to be overactive in individuals with psoriasis, leading to excessive inflammation and abnormal skin cell growth.
๐ง๐ก๐-ฮฑ ๐๐ป๐ต๐ถ๐ฏ๐ถ๐๐ผ๐ฟ๐: Medications work by inhibiting TNF-ฮฑ, a pro-inflammatory cytokine. By blocking TNF-ฮฑ, these biologics reduce inflammation and slow down the progression of psoriatic skin lesions.
Main agents under this group includes: etanercept, infliximab, adalimumab and certolizumab pegol.
๐๐๐ฎ๐ป๐ฒ๐ฟ๐ฐ๐ฒ๐ฝ๐: Etanercept is approved for patients aged four years or older.
Infliximab: the onset of action of infliximab is faster than several other commercially available biologic agents (according to systematic review)
๐๐ฒ๐ฟ๐๐ผ๐น๐ถ๐๐๐บ๐ฎ๐ฏ ๐ฝ๐ฒ๐ด๐ผ๐น: A pegylated humanized antibody Fab fragment with specificity for TNF-alpha.
A potential advantage of certolizumab pegol is minimal transfer across the placenta.
๐๐-๐ญ๐ณ ๐๐ป๐ต๐ถ๐ฏ๐ถ๐๐ผ๐ฟ๐: Drugs target IL-17, another key player in psoriasis-related inflammation. These biologics help normalize immune responses and alleviate psoriatic symptoms.
Main agents under this group includes: Secukinumab, Ixekizumab, Brodalumab and Bimekizumab
๐๐ฟ๐ผ๐ฑ๐ฎ๐น๐๐บ๐ฎ๐ฏ: In the US, use of the drug will require participation in a REMS program due to concerns regarding risk for suicidal ideation. However, causal relationship between Brodalumab treatment and suicidal ideation and behaviour has not been confirmed.
๐๐-๐ฎ๐ฏ ๐๐ป๐ต๐ถ๐ฏ๐ถ๐๐ผ๐ฟ๐: target IL-23, which is involved in the differentiation and activation of T cells. By blocking IL-23, these medications interrupt the autoimmune response that drives psoriasis.
Main agents under this group includes: Ustekinumab, Guselkumab, Tildrakizumab and Risankizumab
๐๐ฌ๐ญ๐๐ค๐ข๐ง๐ฎ๐ฆ๐๐: Ustekinumab is a human monoclonal antibody that targets IL-12 and IL-23.
Ustekinumab is approved for treatment of adults and children 12 years and older with moderate to severe psoriasis.
๐ฅ๐ถ๐๐ฎ๐ป๐ธ๐ถ๐๐๐บ๐ฎ๐ฏ: Risankizumab is a humanized monoclonal antibody directed against the p19 subunit of IL-23 and IL-39

With biologicals changing the treatment game for psoriasis. They provide a precise approach that attacks the immune mechanisms fueling the illness, often resulting in marked improvement in disease symptoms and the patientโs overall quality of life. These drugs work very well but must be utilized judiciously, through cautious patient selection and monitoring for safety. With growing knowledge about the pathophysiology of psoriasis, we can expect innovative biologics will emerge, which can provide better control over this chronic dermatitis for sufferers.

Alice in Wonderland syndrome (AWS) is a rare condition that causes temporary episodes of distorted perception and disorientation. It disrupts your brainโs ability to process sensory input. The disruption affects how you perceive the size of things you see around you, the feel or look of your own body, or both. It can also distort your sense of reality. You may feel larger or smaller than you actually are. You may also find that the room youโre in โ or the surrounding furniture โ seems to shift and feel further away or closer than it really is. These episodes arenโt the result of a problem with your eyes or a hallucination. Theyโre caused by changes in how your brain perceives the environment youโre in and how your body looks. This syndrome can affect multiple senses, including vision, touch, and hearing. You may also lose a sense of time. Time may seem to pass faster or slower than you think.
AWS is also known as Toddโs syndrome. Thatโs because it was first identified in the 1950s by Dr. John Todd, a British psychiatrist. He noted that the symptoms and recorded anecdotes of this syndrome closely resembled episodes that the character Alice Liddell experienced in Lewis Carrollโs novel โAliceโs Adventures in Wonderland.โ

Alien hand syndrome, also known as Dr. Strangelove syndrome, is a phenomenon in which one hand is not under control of the mind. The person loses control of the hand, and it acts as if it has a mind of its own. The etiology includes neurosurgery, tumors, aneurysms, and rarely stroke. This case is presented to create awareness of this interesting clinical scenario, which can be terrifying to the patients and confusing to the physicians who are not aware of it.
It was first described in 1908. It is an interesting situation in which a person loses control of his or her hand, which starts to act independently. These motions are often completely unnecessary or inappropriate at the time and can be terrifying to experience. It describes involuntary complex goal-directed activity of one limb. Recent usage of the term โalien handโ is more liberal and requires having observable involuntary motor activity along with the feeling that the limb is foreign or that it has a will of its own. The syndrome has been reported after surgery on the corpus callosum and with brain tumors, aneurysms, degenerative diseases of the brain, and uncommonly stroke. Alien hand as a manifestation of cardioembolic stroke is extremely rare, with only a few cases reported in the literature.
Film buffs may remember that the title character of the classic 1964 dark comedy Dr. Strangelove, or: How I Learned to Stop Worrying and Love the Bomb had what looked like alien hand syndrome. Dr. Strangelove's hand would perform what appeared to be purposeful acts but were in fact unintentional. This quirk was used for comic effect in the movie, but in real life the condition can be distressingโand can present in varying ways. One hand may swat at the other involuntarily, or an arm may inexplicably shoot into the air.
In a report documented in the medical literature, a man's alien hand started undoing the buttons on his shirt as his other hand buttoned them. In other case reports, patients have said they awoke from sleep to find their hands choking them.
Lesions implicated in causing alien hand syndrome include those in the corpus callosum and posterior parietal cortex, supplementary motor area, and the anterior cingulate cortex. Functional MRI has been used to study brain activity in patients with alien hand syndrome. In normal individuals, initiation of motor activity shows activation of multiple extensive neural networks. However, in patients with alien hand syndrome, only isolated activation of the contralateral primary motor cortex is observed. It has been proposed that lesions in the parietal cortex result in isolated activation of the contralateral primary motor area due to its release from the intentional planning systems. Damage to the parietal cortex can also cause lack of awareness of movements due to loss of proprioceptive feedback or left hemineglect. The combination of these factors results in initiation of spontaneous movements without the patient's knowledge or will.
Alien hand syndrome has been reported to be associated with several abnormal involuntary movements when different regions of the brain, like the corpus callosum, parietal region, or frontal region, are involved. It can be classified into at least four categories: 1) diagnostic dyspraxia/intermanual conflict (when one hand performs actions contrary to the other hand); 2) alien hand sign (a subjective feeling that the hand is not one's own); 3) syndrome of anarchic hand (when the affected hand performs goal-directed activity not under the will of the person); and 4) supernumerary hand (a feeling of having an extra limb). Another type of alien hand is the levitating hand, where the affected limb tends to levitate without volitional action.
Alien hand might manifest as a self-groping behavior and self-oppositional behavior. Autocriticism has also been reported, with the person slapping the alien hand with the normal hand. The person loses control of the affected hand as if it is being controlled by an external force. The alien hand might grab onto things and the person might have to use the other limb to release the objects from it. At extremes, the alien hand has been reported to even suffocate the patient.

Alien hand syndrome is essentially the same thing as alien limb syndrome. The broader name reflects the fact that a similar condition can happen to your legs too. So far, people have only had one limb affected at a time.
There are three main categories of alien hand syndrome based on the part of your brain thatโs causing the condition. They are the:
Frontal lobe version: This is the only version reported that affects your right hand.
Callosal version: This involves the corpus callosum area of your brain.
Posterior version: This involves your parietal lobe.
There is no established treatment for alien hand. It has been reported to last for several days to several years. Alien hand as a manifestation of cardioembolic transient ischemic attack has been reported only once based on our review (6). The extremely short duration of alien hand in this case report (30 minutes) is the shortest reported duration of this phenomenon recorded.

There isnโt one universal cause for alien hand syndrome. But itโs always caused by some kind of damage to your brain in a way that disrupts your movement and control.
Usually, when someone chooses to start moving their arm, they activate multiple extensive neural networks within their brain. In people with AHS, the areas of activation are more isolated.
On a broader level, causes of brain damage that can lead to AHS include:
Brain tumors
Stokes
Aneurysms
Brain damage from an injury
As a side effect of brain surgery
ParryโRomberg Syndrome
CreutzfeldtโJakob
Atypical Parkinsonโs variants, like corticobasal syndrome
Various other neurodegenerative diseases
Keep in mind that this is a very rare side effect of most of these conditions. For example, itโs highly unlikely that youโll develop AHS if you have a stroke.
But for some specific conditions, the rate can be surprisingly high. Data indicates that it appears in 30% of people with the Parkinsonโs variant โ corticobasal syndrome. It also occurs in 4% of people with CreutzfeldtโJakob disease.
In some cases, the condition is idiopathic โ meaning that the exact cause remains unknown.

Have you ever seen someone around you who enjoys making other people suffer? If yes, that person may have a mental disorder which we will discuss today.
๐๐๐๐๐๐๐๐ Antisocial Personality Disorder (ASPD), sometimes also called sociopathy, is a mental health condition in which a person behaves in a harmful manner towards others without remorse. About 3% of men and 1% of women possess antisocial personality disorder. Certain infamous people, such as serial killers John Wayne Gacy, Jefferey Dahmer, and Ted Bundy did have antisocial personality disorder. A person with ASPD ignores the rights and feelings of others, often violates the law, becoming criminals. They may lie, behave violently or impulsively, and have problems with drug and alcohol use. They do not perform their responsibilities properly related to family, work or school.
๐๐๐๐๐๐๐๐: Symptoms of ASPD include repeatedly:
๐ Ignoring right and wrong.
๐Telling lies to take advantage of others.
๐ Being hostile, aggressive, violent or threatening to others.
๐Having a sense of superiority and ignoring others opinion.
๐ Having problems with the law, including criminal behavior.
๐ Feeling no guilt about harming others.
๐Doing dangerous things with no regard for the safety of self or others.
๐ Being irresponsible and failing to fulfill financial responsibilities.
๐ Have difficulty sustaining long-term relationships.
๐Always blame others for problems in their lives.
๐๐๐๐๐๐ personality causes are complex, but may be associated with environmental factors as well as genetics.
๐ Genes may make you vulnerable to develop antisocial personality disorder and life situations, especially neglect and abuse, may trigger its development. Exact gene responsible is yet to be identified by researchers.
๐ Childhood trauma may trigger ASPD. For example: Negligence from parents, torture, sexual abuse, malnutrition etc. If parents engage in criminal activity, their children are more likely to inherit behavioral problems.
๐ Social and home environments can also be responsible for the development of ASPD. DIAGNOSIS There's no physical tests to diagnose ASPD as it is a mental disease. Diagnosis should be conducted by a licensed psychologist and it can only be made if the person is aged 18 years or older and at least 3 of the following criteria behaviors apply:
๐ repeatedly breaking the law
๐repeatedly being deceitful
๐being irritable and aggressive
๐having a reckless disregard for their safety or the safety of others
๐being impulsive or incapable of planning ahead
๐being consistently irresponsible
๐ lack of remorse
๐๐๐๐๐๐๐๐๐๐๐๐๐: Criminals behavior is a key feature of ASPD and it's obvious that someone with the disorder will commit crimes and will be imprisoned at some point in their life. Spouse abuse or child abuse, problems with alcohol or drugs, trying to kill someone for a small reason, financial or social instability is very common with these ASPD patients. Moreover, they have an increased risk of dying prematurely as a result of reckless behavior or attempting su***de.
๐ง๐ฅ๐๐๐ง๐ ๐๐ก๐ง:ASPD is one of the most difficult types of personality disorders to treat. A person with ASPD are not likely to seek help on their own. If you suspect that a friend or family member may have the condition, you might gently suggest that person to visit a mental health provider.
Healthcare providers may recommend a combination of medications and psychotherapy:
๐๐๐๐ข๐๐๐ญ๐ข๐จ๐ง๐ฌ: Medications can help treat symptoms like aggression and mood changes. For example, Antidepressants drugs, which can regulate serotonin levels in brain. ๏ท Psychotherapy: Cognitive behavioral therapy (CBT) is a type of counseling that focuses on changing of thinking and behavior. Therapy can help the patient to see how their behavior affects others. They may be benefited from individual therapy, group therapy or family therapy.
๐๐๐๐๐๐๐๐๐๐ There's no sure way to prevent ASPD from developing in those at risk. Because antisocial behavior is thought to have its roots in childhood. Parents, teachers and pediatricians may be able to see early warning signs. It may help to try to identify the disorder and taking extra care of them from beginning.

We should be calm & patient with them, remind them of their positive traits and not to ignore them if they seek help. BPPV is the sudden sensation of spinning or feeling that the inside of the head is spinning. It is a common inner ear disorder that causes brief episodes of mild to intense dizziness. It usually occurs among people over the age of 65 and rarely among children.

The signs and symptoms of BPPV can come and go and usually last less than one minute. The onset of BPPV may be frightening. People may think they are having a stroke. The common symptoms are:

BPPV is thought to be caused when otoconia (tiny calcium crystals) come loose from the utricle (a sensory organ in the inner ear). The utricle is a part of the balancing system. It detects the accelerations, especially the horizontal acceleration. An injury or degeneration of the utricle is thought to be the cause of escape of the crystals. When these crystals escape into the fluid that are filled into the canals of the inner ear, the balance system is interfered.
BPPV can be caused by-
โ Head injury
โ Keeping the head in the same position for a long period of time.
โ Bike riding on rough trails
โ High intensity aerobics
Activities that bring about the signs and symptoms of BPPV can vary from person to person, but are almost always brought on by a change in head position. Some people also feel out of balance when standing or walking.
In some cases, BPPV may be a symptom of another inner ear condition, such as:
โ Labyrinthitis
โ Vestibular neuritis
โ Acoustic neuroma

BPPV is rarely serious and the diagnosis is simple enough to be conducted through physical examination performed by a doctor and answering questions regarding symptoms. A MRI or CT scan of the brain is usually not needed.

BPPV can be treated successfully with simple exercises. They involve sequentially turning the head in a way that helps remove the crystals and help them float out of the semicircular canal.
Medications are given to relieve the symptoms of motion sickness.
In many cases, BPPV goes away on its own eventually. However, it is suggested to visit a healthcare provider and learn how to manage recurring symptoms.
