Patella-Femoral Pain (PFP)💥

Is often described as a poorly localized pain around the knee cap.

Often irritable to ascending stairs, squatting, lunging and running 🏃🏼‍♂️

One of the many contributing factors to PFP can be muscle tightness in the quadriceps muscle group.

Tension in the quadriceps muscle and patella tendon drives the patella into the surface of the distal femur creating an increased patella femoral joint reaction force 💥

Clinically we can get a pretty good idea of the quadriceps tension by performing the Thomas Test.

Ideally in the Thomas test you want to see the knee flexion angle rest close to 90 degrees. This would indicate a good resting muscle length in the quadriceps.

As you can see in my client he rests in a significant knee extension angle which would indicate a tight quadriceps muscle group which would result in an increase in patella femoral joint reaction force and therefore could be a contributing factor to his knee pain.

We addressed this by performing a dry needling treatment of the quadriceps and performed some corrective lengthening exercises for homework.

This client will have to be diligent to his home exercise program and gradually reintroduce loading to the knee as pain and inflammation settles.

11/17/2022

ACLR 8 weeks post-op

BG is now 8 weeks post-op ACLR surgery. She has full knee range of motion, normalized gait, minimal to nil swelling and has her eyes set on a return to jogging program soon 👀

Yesterday we performed an isometric quad strength test at 90 degrees. The purpose was to get a limb symmetry score compared to her non-operative leg. This gives us valuable feedback on how well our strength programming is working for one of the key rehab muscle groups, the quadriceps.

BG scored 72% compared to her non-operative leg. This is a reasonable score given the timeline and additional surgeon restrictions placed in her protocol (no BFR, no knee extensions).

What we did with this information was set new goals, and made slight modifications to her program to address the quadriceps group on her operative leg.

We will re-asses this test at week 12 and aim for >90% 💪

ACLR 4 weeks post-op

🚨Update 🚨

BG has met her knee active range of motion (AROM) goals 4 weeks post-op! 👏

We set the goals of 0-130 degrees of motion. We are currently working on getting a little bit of hyper extension from her knee to create symmetry with her non-operated knee. I have no doubt she will get there!

Big shoutout today to one of our clients Alex B. He is currently less than 4 months post-op ACLR and has already hit our goal of >90% quad strength symmetry. In fact he is about 102% quad symmetry 👏

How were we able to do this?

1️⃣ Alex comes in with a killer attitude each time and is highly motivated to rehab 100%

2️⃣ Early implementation of gym strength training including seated knee extensions, squats, lunges, hamstring curls etc…

3️⃣ Use of Blood Flow Restriction (BFR)Cuffs. BFR allows us to load Alex at 20-30% Repetition Max but get training results as if he were loading at 80% Repetition Max. If you are not using BFR post ACLR you are missing out on tremendous early gains that are difficult to regain later in rehab
