Locking plate surgery involves the use of metal plates with special screw holes that lock the screws in place. These plates are designed to provide stable fixation and distribute the load evenly across the fractured bone. The screws used in locking plate surgery are threaded into the plate, creating a fixed-angle construct that can withstand significant forces.

Tibial fractures are one of the most common fractures in the whole body, and the mainstay of treatment is intramedullary nailing with the standard patellar nailing technique: the infrapatellar approach.

However, this approach requires 90° knee flexion to facilitate insertion of the intramedullary nail, and such a position is likely to exacerbate displacement for fractures with a proximal fracture.

The suprapatellar approach tibial nailing technique is an alternative to the standard infrapatellar approach, and more and more orthopaedic surgeons are favouring the suprapatellar surgical approach due to its relatively broader indications.

If we look at the traditional tibia nail, it has some limit:

Position: Unfavourable for fluoroscopy; Aggravates soft tissue irritation from fracture ends; Not easy to maintain in a repositioned position.

Fractures of special parts of the tibia: e.g., proximal tibial 1/4 fracture, common deformity: anterior angulation deformity.

Chronic anterior knee pain, Mean prevalence 47.4% (10%-86%), Injury to infrapatellar fat pad, Injury to infrapatellar branch of saphenous nerve

Tornetta was the first to propose the technique of intramedullary nail fixation in the semi-extension position in 1995, where the knee is flexed 15°, the patellofemoral joint is separated medially and the patella is semi-dislocated.

Cole proposed the suprapatellar approach for intramedullary nailing in the semi-flexed position: an incision of approximately 2.5 cm in length was made proximal to the suprapatellar pole, split in the direction of the muscle fibres, and the intramedullary nails were inserted using a protective sleeve.

Indications for suprapatellar approach:
1) Fracture of the proximal 1/4 of the tibia
2) Segmental fracture of tibial stem
3) Fracture of distal tibial metaphysis
4) Fracture of the hip or knee joint which has been restricted in flexion (e.g. degeneration or fusion of the hip joint, osteoarthritis of the knee joint) or inability to flex the knee or hip (e.g. posterior dislocation of the hip joint, fracture of the femur on the same side).
5) Tibia fracture combined with skin injury at the infrapatellar tendon.

The suprapatellar approach to the knee for tibial intramedullary nailing has a number of technical advantages, including the avoidance of tissue separation in the proximal tibia and the avoidance of saphenous nerve branch injuries in the infrapatellar approach.

Compared with the infrapatellar approach, the fracture was better repositioned and the pinning point was better positioned, but the knee pain was no different from that of the infrapatellar approach.

The Titanium Elastic Nail System (TEN) is an internal fixation device made of titanium alloy with a sickle-shaped elbow, specialized in the treatment of long bone fractures in children. Dr. Ligier of the Nancy Hospital in France was the first to report clinical results of TEN fixation in children with femur fractures. Subsequently, the TEN has gained popularity in Europe and North America, and has become a common treatment method and technique in the management of long bone fractures in children.

TEN has sufficient fixation strength for children's long bone fracture, but also has a certain degree of micro-motion of the fracture site, which promotes the healing of the fracture. Compared with the conservative treatment, the fixation of the fracture is more stable, and compared with other incision and reduction fixation surgical methods, it has the advantages of less trauma, less scar, less tissue damage, no stripping of the periosteum, no damage to the epiphysis and so on, and most of the fracture can be achieved without cutting the fracture end to reset the effect. After the operation, the children will feel less pain and recover quickly, usually they can be discharged from the hospital in 3-5 days after the operation, and the removal of internal fixation in the later stage is also easier.

It should be noted that although elastic intramedullary pins have many advantages in the treatment of fractures in children, not all fractures of the long bones in children can be fixed with elastic intramedullary pins, and the decision should be made by a pediatric orthopedic surgeon, depending on the age of the child, the type of fracture, and the location of the fracture.
