The urine crystal picture below comes from NEJM journal and is questioned by Dr. Zhang Shimin recently. This fan-shaped crystals came from a 42-year-old man who was receiving trimethoprim–sulfamethoxazole. So the author diagnosed sulfonamide-crystal–induced acute kidney injury. But according to Dr. Zhang Shimin from Peking Union Medical College Hospital, these crystals are from sufadiazine, not from TMP/SMX. So it is not prudent to directly make such a diagnosis.

From ancient times until the Victorian era, urine was used as the primary diagnostic tool. Laboratory medicine began with the analysis of human urine, which was called uroscopy and today is termed urinalysis. Uroscopy was the mirror of medicine for thousands of years. From a liquid window through which physicians felt they could view the body's inner workings. Numerous, somewhat accurate, physiologic theories arose from uroscopy. Then the importance of urinary diagnosis became exaggerated, and increasingly complex, until physicians required only the presence of urine, not patients, to diagnose disease. Uroscopy then escaped medical control, becoming first a home health aid and then a tool of uneducated practitioners. Thomas Brian led a medical rebellion against all uses of uroscopy and published the Pisse Prophet, a book that devastated uroscopy.

The story of how artificial kidney was first invented. After trial and error, eventually success came.

Dubbed as the “Father of Artificial Organs”, Dr. Willem Kolff was known for his discoveries that have saved many lives - especially his invention of the first artificial kidney. So how did he achieve such a risky invention? Believe it or not, by using sausage casings and orange juice cans.

At the beginning of WWII in 1939, Kolff started working on artificial kidneys at a rural hospital in Kampen, Netherlands. Since supplies were limited during the war, he used whatever scraps he could find. He started off by wrapping sausage casings around a wooden drum to build his kidney machine. Blood was pumped into the casings and the drum was rotated to eliminate impurities. Later on, he developed the device even further by adding orange juice cans and a washing machine.

However, the invention was not met with immediate success as his earliest patients died of kidney failure a few days after treatment. But in 1945, after more research, a woman lived 7 more years thanks to Kolff’s invention. This device has seen many developments since its release and has evolved into what we know today as - dialysis.

Do you know these strange crystals are struvites?🙃
Struvite crystals (magnesium ammonium phosphate, triple phosphate) usually appear as colorless, 3-dimensional, prism-like crystals (“coffin lids”). Occasionally, they instead resemble (vaguely) an old-fashioned double-edged razor blade (lower frame). Urinary tract infection with urease-positive bacteria can promote struvite crystalluria (and urolithiasis) by raising urine pH and increasing free ammonia.
In vitro study has revealed that struvite crystals experienced a significant morphologic evolution from pin pointed structure to X-shaped and tabular morphologies in the presence of two bacteria that were originally isolated from the kidney stone patients. In conclusion, these experiments revealed that the bacteria play a major role in the specific morphogenesis of struvite and can able to control the nucleation

Urinary tract infection (UTI) is a term applied to a great variety of clinical conditions, from asymptomatic bacteriuria to severe infections like sepsis.
