Ultrasound is my second interest in Radiology, particularly vascular and musculoskeletal applications. I’d like to share an interesting case of male 48 years old underwent Color Duplex US for carotid and vertebral arteries. Request unfortunately lacks important clinical information. One sign only could be detected in the Color Doppler study and it was guiding for inspecting patient left upper limb which showed digital ischemia. In my opinion, Reversal of flow in vertebral artery in Color Doppler is one of most valuable indirect signs in Radiology. Patient then had underwent CT angiography confirmed left subclavian steal consequent to Ostial atherosclerotic aneurysm followed by high grade stenosis of proximal subclavian artery before origin of vertebral artery

Advanced sarcoidosis presenting with paracicatricial emphysema

Extremely interesting rare case of female 27 years old known sarcoidosis since 2017 (first scan), then underwent successive HRCTs in 2019, 2022 and 2023. What is rare here is the presentation of advanced sarcoidosis in two things, first it mimics UIP (predominance of lower lobar, peripheral and subpleural distribution of honeycombing, traction bronchiectasis/bronchiolectasis and irregular reticulation). The second thing is the upper lobar paracicatricial emphysema and bullae (hence the name in literature, bullous sarcoidosis). Can radiologist diagnose bullous sarcoidosis in unknown cases ?
