What is trigger finger/ thumb?

- Usually starts with pain at the base of the affected finger or thumb - on the palm side.
- The finger can start to click or "trigger" as you bend and straighten it.
- The finger may begin locking into a bent position as it gets worse. This can usually be manually released using the other hand.
- In severe cases the finger may remain in this bent position and unable to unlock.
- Locking is commonly reported first thing in the morning or after gripping activities.
- Initial treatment may include anti inflammatories, rest from repeated gripping activities and sometimes splinting.
- If the above doesn't give good improvement, corticosteroid injections for trigger thumb/ finger have a very good success rate.
- The ring finger is most commonly affected, however any finger or thumb can be a problem.
- People may also have a few fingers affected at the same time.

What is Greater Trochanteric Pain Syndrome (GTPS)/ Trochanteric Bursitis?

- Focus of pain on the outside of the affected hip
- Pain when lying on the affected side - often people will report broken sleep due to the pain.
- Aggravated by exercise, activity, standing, walking, stairs.
- Often a bio mechanical or muscle imbalance component to the condition.
- The gluteal tendons / Trochanteric bursa become irritated.

Treatment

- Rest from aggravating activities.
- Review of foot posture and lower limb alignment.
- Specific strengthening and stretching programme.
- Pain medication.
- Responds well to injection if symptoms persist.

Fantastic improvements seen in clinic today. Patient reports 85% improvement in pain levels following injection therapy in their knee - for osteoarthritis.

I've done more injections for patients today - looking forward to seeing the outcomes in a few weeks.

Have you been told you have shoulder impingement? A problem with your rotator cuff? …. Or do you have ongoing shoulder pain and don’t know the cause.

- The rotator cuff is a group of muscles that work in sync to move the shoulder.
- They have a bursa (like a glide sheet) that drapes over the top of them to reduce friction.
- The tendons or bursa can become inflammed and cause pain.
- This is the most common source of shoulder pain.
- Treatment initially involves resting from the aggravating activities where possible.
- Medication can help to manage the pain.
- Exercises to improve shoulder strength and control ultimately help to manage the condition longer term. These are really important.
- Corticosteroid injection can be useful to manage the pain if tablets have been unhelpful.

Let’s talk about osteoarthritis of the knee.

🦴Osteoarthritis is the most common form of arthritis.
🦴The knee joint is one of the most commonly affected joints.
🦴The cartilage becomes reduced in depth which is considered part a normal ageing process.
🦴Joint pain can increase - especially when standing/ walking.
🦴The joint can feel stiff especially in the morning or after rest.
🦴Sometimes the pain can wake you through the night.
🦴The joint may swell.
🦴All of the above can impact on your function - walking/ stairs/ kneeling.

How do we improve symptoms of osteoarthritis of the knee?

🦴Losing weight can reduce load put through the joint.
🦴Movement exercises keep the joint mobile and prevent stiffness.
🦴Strengthening exercises offer support to the joint and can ease pain.
🦴Discuss medication with your GP to help to alleviate pain.

Did you know that a frozen shoulder has nothing to do with temperature? 🥶❄️🧊

It gets its name because of how stiff the movement of the shoulder becomes.

Frozen shoulder facts.

🧊Usually commence for no apparent reason.
🧊Starts with increasing severe pain in the shoulder.
🧊Follows with a progressive stiffness to shoulder movement.
🧊Reaching around your back or behind your head becomes more difficult. As does raising the arm above shoulder height.
🧊Twice as common in females than males.
🧊Average age 40-60 years.
🧊More likely if you have diabetes.
🧊Usually lasts for 12-18 months.
🧊Initial treatment is pain medication and exercise.
🧊If function and sleep is still a significant problem corticosteroid injection can be useful to manage pain levels.

It’s been a great day in clinic today with some fabulous patients. Feeling motivated and positive for them all.

Do you know much about “cortisone injections”? I’m sure you’ve all heard of them but what do they do?

💉The drug injected is a corticosteroid which is used to reduce inflammation.
💉 Corticosteroids can be injected locally into joints, bursa and around tendons.
💉 Reducing the inflammation of a joint or soft tissue can help to reduce pain.
💉 It can take 1-2 weeks to feel the benefit from an injection.
💉 People who have less pain are more able to carry out exercise or return to their normal activities.
