2 months after Deep plane surgery.

A big part of my practice involves operating on patients below 50, and sometimes as young as early 40s. Although this is considered a young age for facelift surgery, there are definitely some great advantages in doing so.

This lovely 44 year old lady had an extended deep plane face and neck lift 2 months ago, and she is doing really well. Still some swelling present around her neck but that's normal at this stage.

I previously carried out a subtle lip lift with a browlift, and after a few months later she decided that it was time for a full face refresh!

I am grateful for her generosity in allowing me to share her results πŸ™

Lip Lift result, 6 weeks after.

Balance and proportions are key elements for a successful lip lift. One of the biggest concerns for patients considering this procedure is the overlifted central lip, which is often associated with mouth closure problems.

The lovely young lady in the pictures would have been a perfect example of this type of unnatural look, should a traditional bullhorn approach had been used. Instead, I carried out an asymmetrical bullhorn, with more tissue taken on the side, as well as a combination of an internal and a vermillion corner lip lift.

She does wear makeup, however the scars are looking pretty good for 6 weeks and her nose is nearly back to normal πŸ˜€

6 months after Deep plane face&Neck lift.

Back to work after a break, and I am delighted to share the results of this beautiful 60 year old lady. She had a long non-surgical journey, which included threads, ultrasound, radio-frequency, fillers, and fat dissolving injections to her neck.

Despite the surgery being extended and complex, the long-term results are super natural, and it's nearly impossible to spot any signs of surgery on her face and neck. She just looks fresher, smoother, younger, and happier πŸ˜€

Bullhorn and corner Lip lift combination.

This was a challenging case where a complex Lip lift approach was adopted in order to improve the appearance of the whole mouth. I have carried out a modified bullhorn lift on the deep plane, along with orbicularis muscle suspension and an extended corner lift.

Does facelift make men look more feminine?

This is not more the case with the use of modern facelift techniques. The recent advances in deep plane surgery have brought male facial surgery into the spotlight. Male patients, even under 50, are now more than ever opting to tackle facial ageing with surgery.

This charming 53 year old gent is just 6 weeks after deep plane face and neck lift, and earlobe reduction. He previously underwent weight loss surgery and has had regular fillers for years. In cases like this, the tissues can be lax and abundant, hence the swelling can take months to subside. Swelling fluctuations are common during that period and can be quite frustrating. Time is working in our favour, but other things such as good diet and sleep, avoiding salt and alcohol and manual lymphatic drainage can speed up this process.

This gent is now on the right track and I'm grateful he has allowed me to share a part of his journey πŸ™

Restoring the neck.

Another example of how impactful the deep neck lift approach can be, even in cases of complex pathophysiology.

This lovely 60 year-old lady was referred to me by a good colleague plastic surgeon in view of the complexity of her neck.

She is now only 4 weeks after deep plane face&Neck lift and the results are already quite striking. For her, this surgery was more corrective than cosmetic, therefore she did not opt for any additional procedures.

In this age group and where significant neck laxity is present, a facelift is also necessary to obtain balanced results. What is also interesting here is how the neck and chest skin quality have improved - this is down to some technical advances with minimal separation of the skin from the deep tissues, as well as an accurate and strong muscle repair.

This lady is now so happy to have her neck exposed and be able to wear her favourite clothes and jewellery, and I am grateful for her generosity with her pictures πŸ™

Deep plane surgery to reverse filler fatigue

Another challenging case here of a beautiful 50 year old lady who previously had a number of non-surgical treatments; 12 years worth of regular fillers, threads, and ultrasound/ radio-frequency devices.

We decided to dissolve the filler in 2 sessions and carry out a deep plane face & Neck lift. No additional procedures. The last 2 before photos are taken before filler dissolving.

Not to my surprise, I found excessive scarring in pretty much everywhere in the face. The neck tissues were normal. The submandibular glands were large and ptotic, and her mandible (low jaw bone) was relatively short.

The deep neck work involved a combination of sculpturing and reduction techniques for the glands, deep fat and digastric muscles, and repair of the platysma muscles. The hyoid bone was also released to be positioned higher and posteriorly. This manoeuvre gives more depth and definition to the cervicomental (chin) angle. Note on the side views the improvement on the neck depth and chin projection without the use of a chin implant or fat grafting.

Filler fatigue is definitely a thing. Surgery in those cases can be challenging and require experience, attention to detail, and most importantly very motivated patients πŸ’ͺ

Not the usual before and afters πŸ˜ƒ. A little throwback from 2 weeks ago when I was invited as a speaker to a wonderful event organised by the .

This was an excellent opportunity for me to discuss and share ideas with some of the leaders in our field on a topic that I am passionate about; facial rejuvenation and . I 've met some lovely people, and I thoroughly enjoyed the interaction with the faculty and the guests.

These are really exciting times for aesthetic practitioners and surgeons, but more so for patients who are looking to look and feel the best version of themselves. Patient education is key in our industry, and I thoroughly recommend the next version of this festival- hopefully soon! πŸ™Œ

Complex secondary Face and neck lift.

This was a tough case..

My lovely 58 year old patient had previously undergone a face and neck lift with fat grafting 6 years ago. When I saw her, she presented with some contour irregularities and loss of definition on her neck, as well as facial descent. Her scars have also migrated and became very pale.
I carried out a deep plane facelift and deep neck reduction/ sculpturing, and here are her results at 4 months post-op.
During surgery I found an excessive amount of scarring with tethered nerves, fat grafting lumps and very difficult tissue planes on her neck. These are tough cases and there is no way to fully predict the level of difficulty. With meticulous dissection and sculpturing I was able to sculpture her tissues and achieve a good improvement with well healed scars and a smooth recovery.

Male deep plane face and neck lift.

This very brave 53 year old gent is only 4 weeks after his surgery and he is doing amazingly well. I carried out a deep plane facelift along with deep neck lift with reduction of the submandibular glands.

The scars are still slightly visible but will be almost impossible to spot in a few months, as they will blend in with the natural creases and hair follicles. This a major drawback for men seeking facelift surgery as they cannot be covered easily. Another drawback is the risk of an unnatural look with feminisation of the face. As you can see, true deep plane surgery can avoid all these risks, hence men are now becoming more keen to undergo surgery even at a relatively young age.

Lip lift result at 6 weeks after surgery.

Subtle but impactful changes for this lovely lady, who also had a mild asymmetry corrected. The scar has healed beautifully and most of the swelling has gone. Some tightness during movements is to be expected up to 4-6 months.

When assessing a lip lift patient there are a number of very important anatomical considerations to make; philtrum length, bone structure, orthodontics, nasal shape, mouth closure, asymmetries, soft tissue quality, lip volume, vermillion show are some of the most important to mention.

Sometimes, patients are looking/hoping for changes that unfortunately are not compatible with their anatomical characteristics. Significant risks can occur when a "cookie cutter" approach is employed without making a careful assessment. My role during consultation is to warn those patients who are looking for dramatic changes from potential issues that can occur when these anatomical limiting factors are ignored.

Subtle changes after liplift can make a very positive impact but for some patients are not enough to justify going through surgery. Lip lift complications can be very difficult to hide and correct.

Early results 3 weeks after multi-procedure facial surgery.

This beatiful 60 year-old lady travelled from Italy to see me and jave her surgery. She is an experienced aesthetic practitioner, and after many years of having non-surgical treatments, including multiple threads, she decided it was time to go under the πŸ”ͺ

I carried out an extended deep plane facelift, deep neck lift, lip lift, upper blepharoplasty, and fat grafting.
Her surgery was complex and I have removed a large number of thread material. After such extensive surgery and at only 3 weeks after,
her face is still swollen and "leathery" as expected. Despite that, however, she is doing amazingly well and healing beautifully after following our post-op instructions and after having several sessions with hyperbaric oxygen. Her face will continue to improve over the next few months and will be posting here her updates. She has now returned home and I look forward to her review next year πŸ™

What is a scarless lower blepharoplasty?

Lower eyelid surgery to treat bags can be performed without visible incisions. This procedure is also known as transconjuctival lower blepharoplasty and is carried out through an incision inside the eyelid. With this approach the fat bags can be accessed and modified. I very much prefer the fat repositioning technique, whereby the fat is not removed but mobilised into a pocket over the bone to correct the tear-trough deformity. The transconjuctival technique can also be combined with skin excsion, canthopexy. Fat grafting can also help but may need repeating in a second session after few months.

In this particular case, this 54 year old lovely lady had fat repositioning and nano fat in the peri-orbital area. This is the result at 4 months. No changes in the eye shape, no skin tightness, no animal eyes. Just naturally refreshed eyes, with no signs of surgery.

For the last 15 years, I have dedicated my career in Head & neck cancer reconstruction and facial surgery. In addition to my UK training, I undertook 2 high-level international fellowships in Taiwan and in Melbourne, Australia, before returning back to the UK to work as a consultant Head and Neck surgeon.

For the last 18 months, I have been working full time privately, and my practice is now exclusively dedicated to facial rejuvenation surgery. My background and experience in complex reconstruction of the face & neck have been fundamental in allowing me to adopt, learn and teach the most advanced Deep Plane techniques.

The work I do is complex and demanding so I am committed to keep learning; visiting international colleagues, attending conferences, and participate in courses, both as an instructor and as a delegate, are all indispensable parts of this journey.

Extended Deep Plane face and neck lift - 5 weeks post-op.

This lovely 60 year old lady had many non-surgical treatments over the years: thread lift, fillers, ultrasound, radio-frequency, and fat dissolving injections to her neck. She came to me after a recommendation by a colleague plastic surgeon in view of the anticipated complexity of a face lift due to the previous treatments.

The surgery was in deed complex, especially the neck , which required deep neck reduction and sculpturing in the presence of significant scar tissue from the previous interventions.

She has recovered really well and we are both delighted with how brightened her face looks at this stage! There is still some residual swelling that will go over the next few months, however she already feels very confident with her new look πŸ™Œ

I am delighted to share the 5 month results of my lovely patient, who flew over from Spain to have her surgery with me.

At the age of 45, she had noticed some facial laxity and neck heaviness that she wanted to tackle. I have carried out a deep plane facelift along a deep neck lift with reduction of the submandibular glands. She also had a small chin implant to give her a subtle increase in chin projection.

When operating in relatively younger the results are not usually too dramatic, but the changes made with surgery cannot be achieved with less invasive treatments. Surgery can also have a preventative effect on the ageing process.

I regularly see and operate on international patients and we have developed a safe setup to support patients who are travelling for surgery.

Deep plane Face & Neck, 5 months post-op.

After having regular injectables and ultrasound treatments for a number of years, this lovely 53 year-old lady felt that it was the right time for the big decision..πŸ”ͺ

She had all her fillers dissolved and I carried out an extended Deep plane facelift, a Deep neck lift, fat grafting, and a lower blepharoplasty. Despite having all her fillers dissolved, during her surgery I came across significant scarring on the deep layers. This is pretty much the norm for me these days 😁. Her neck lift involved reduction of the submandibular glands and anterior digastic muscles, as well as plication of the platysma muscles. The fat grafting made some subtle changes in specific facial areas where there was loss of volume.

She is a very private person and I am grateful that she allowed me to use these pictures πŸ™

Isolated Neck lift, 10 weeks post-op.

This an update of this 51 year old gent following a deep neck lift and facial fat grafting.

An isolated neck lift is carried out through minimal access incisions, which are well-hidden under the chin and behind the ear. This surgery doesn't involve a face lift, howeversome lower face laxity is also addressed. There are no scars in front of the ears and temple, and that's the challenge with this procedure, as a lot of work is being done with no visible scars.

Surgery takes approximately 2.5 hours, and I prefer a general anaesthetic, although twilight anaesthesia can also be used.

Male and female patients who have neck concerns and only mild or no face laxity are ideal candidates.

If you have recently seen some celebs looking great with no visible scarring and wondered what they had done, then maybe the answer is isolated neck lift πŸ˜‰

This is my lovely 59 year old patient at 8 months after her surgery. She had an extensive deep plane face & Neck lift with submandibular gland reduction, as well as a tranconjuctival lower blepharoplasty with fat repositioning.

When you see this beautiful lady in person, you will never think that she had surgery. You have to look at the before pictures to appreciate the changes. Even the patients themselves are surprised when they look at their before pictures! That's the magic of deep plane surgery🌟. Heavy necks and saggy facial tissues are corrected, but we still see the same person- we just bring out the best version of themselves.

Deep plane Face & Neck lift, 7 months post op.

I am grateful to be able to share this lovely lady's results after her extended deep plane surgery. I previously shared her journey with the early results at 48h, 1 week, and 4 weeks - check previous posts on my feed.

She looks brighter and youthful, and the improvements on her jowls, jawline, and neck are noticeable. You can also notice the improvements on her neck skin despite not having any non- surgical treatments; just surgery. The only additional procedure I did on her was an earlobe reduction.

It's great to see how natural, confident, and happier patients look a few months after deep plane surgery. The results do get better with time 😊

Deep plane Face and Neck Surgery.

This 67 year old lady underwent an extended Deep plane face and deep neck lift, along with a lip lift procedure.

I was absolutely delighted to see how well she progressing at only 12 days after surgery! There is still some swelling and bruising to settle, after which the face and neck will become softer, more supple and natural looking.

The Deep neck lift included reduction of the submandibular glands. Although this is challenging surgery, it is however the only way to sculpture the deep neck and obtain optimal neck results in cases like this.

When performing deep neck reduction I normally use suction drains, which are removed after 48h. Total surgical time for this surgery is between 4.30-6 hours and a general anaesthetic (TIVA) is most commonly used, although local anaesthetic with deep sedation is also an option. Regardless of the type of anaesthetic patients can go home the same day 🎯.

Follow up appointments are at 48h, 1 week, 2 weeks, 6 weeks, 6 months and 1 year. Patient have access to our team 24/7.

Surgeries under general anaesthetic are carried out at Harley street Hospital and also at the Cadogan clinic in Sloane Square, Chelsea.
