Ingrowing toe nails… ouch!!😖😖😖😖

I have had quite a few enquires about ingrowing toenails recently so thought I would explain a little about it and the treatment that I offer for this.

Anyone who has had an ingrowing toenail will know the pain is like no other, there are thousands of nerve endings in your foot which can explain why the pain is so awful.

Ingrowing toenails ( onychocryptosis ) can be caused by a number of factors such as genetics, toe nail being too big for the toe plate, shoes that are tight and put pressure on the toe and poor nail cutting technique. It mainly affects the big toe nails but any toenail can be affected.

Treatment options..

PNA - for a permanent solution a partial nail avulsion ( PNA) can be carried out in a clinic, this is minor surgery so requires local anaesthetic. The ingrowing part of the nail is removed and then a chemicals is applied in the nail matrix so it doesn’t grow back.

Pro’s -
👍can be a permanent solution meaning no further treatment is required 🙌
Con’s-
👎Cost, you can expect to pay hundreds per nail for this treatment.
👎Does not always work.
👎If not done correctly the whole nail can be damaged.
👎NHS/ private clinic waiting list are loooong…..

PNR - this is the treatment option that I offer and it involves partial nail removal ( PNR) of the affected nail, so the part of the nail that is putting pressure on the nail wall is taken away to alleviate pain.

Pro’s
👍 much more affordable than PNA
👍Can be done in the comfort of your own home
👍 Little down time.
Con’s
👎 Can be painful.
👎Not a permanent solution. This is conservative treatment so needs to be carried out every 4-6 months as the nail grows out in most cases.

Criteria for a PNR-

🦶Nail cannot be infected. If infection is suspected then a course of antibiotics is required first to treat any infection.

🦶I get a lot of enquires from parents whose children are affected by this, and I will always be honest and say that it is a painful procedure and some children are unable to tolerate it. I will ask for a photograph and if I think that the treatment will be too painful then I will recommend having treatment under local anaesthetic.

Do you have thick nails that you just cannot get your nippers through? if this is you.. please don’t be embarrassed! It’s a very common condition that can occur through repetitive trauma ( footwear been the usual suspect) over time these thickened nails can cause discomfort as they put pressure on the nail plate, especially when wearing shoes and can increase the risk of developing fungal infections.

Summer feet are made in the winter! 👣

Please don’t ignore your feet just because it is not sandal season! Cracked callus on your feet ( fissures) can be both unsightly and painful! In addition to this they can lead to complications such as skin infections!
Management of this condition involves a range of techniques to reduce the callus and then daily application of a thick emollient 😊

Another example of thickened nails that are impossible to manage independently. Quite a challenging job this one!
This patient also has a condition that makes the nails grow like horns and is referred to as ‘Rams Horns Nails’.
Looking much better and feeling more comfortable after some heavy reduction with the drill!

Look at this for a before and after..
Believe it or not this is a verruca!
Although the general consensus is that these are contracted from swimming pools etc, they are actually a sign that your immune system is compromised. In this patients case due to medication.
Although it might never go due to the medication, regular appointments where it can be parred down will offer great relief.

My lovely patient wasn’t able to walk on her carpet without catching her little toenail,
It’s not understood why, but as we are age our toenails grow thicker rather than longer making toenail maintenance difficult to achieve. If you are struggling to cut your own toenails, chances are they’re too thick!
Thickened toenails need to be reduced before they can be trimmed 👣

here is an amazing transformation for a lovely lady today who was a ballet dancer in her prime! Right foot is before treatment, left is after.

This lovely chap has arthritis in his hips and was not able to reach his feet due to pain. His nails were too thick for his wife to cut. Nails reduced with the drill and trimmed down ✅
