We are supporting Carers Week!

Carers week is a campaign which is run annually. The main purpose is to raise awareness and to highlight the challenges faced by carers.

Anyone who looks after a family member or friend who has a mental or physical illness, a disability, an addition or requires additional support as they age, is a carer.

This year the aim of carers week is to 'put carers on the map', with the hope to highlight the increasing pressures on unpaid carers and to campaign for their recognition and support.

If you’re over 40, you should get your blood pressure tested at least every five years.

You can do this yourself if you have a monitor at home, or as part of an NHS health check, at some workplaces, at a pharmacy or your GP practice.

The pneumococcal vaccine protects against serious and potentially fatal pneumococcal infections including pneumonia, septicaemia and meningitis ✅.

Babies and children, those with long-term health conditions and adults aged 65 and over may be at a higher risk of serious illness from a pneumococcal infection ⚠️.

It's important that vaccines are given on time for the best protection, but if you or your child missed a vaccine, please contact your GP to catch up 📞.

Frailty Scoring

The addition of the Electronic Frailty Index score is to help identify individuals whom may benefit from further assessment and enable the practice to identify those whom may benefit the most. It is an administration code added to the healthcare record based only on existing information contained in the record. It is not an indicator of a clinical diagnosis of frailty nor can be used as such. The use of the index score is part of improving quality of care to individuals with frailty across Lancashire and South Cumbria.
