REVISE AND REVIEW: BONDED RETAINERS
Advantages
• Easy and well tolerated by the patient.
• Do not compromise on aesthetics.
• Minimal interference with speech.
• Less reliant upon compliance than removable retainers. However, a randomised clinical trial has shown that images of relapse shown to both patients and parents can increase compliance with the removable retainers and decrease the chance of relapse (Lin et al., 2015).
• Allow some physiological movement of the teeth.
• Minimal periodontal damages (Booth et al., 2008)
Effectiveness
Although relapse in the lower labial segment was found statistically insignificant with both removable and fixed retainers (Forde et al., 2018, Atack et al., 2007), another RCT found that bonded retainers are better at maintaining alignment in the first 6 months after debonding than thermoplastic retainers (O’Rourke et al., 2016). The same researchers followed up their sample over a period of 18 months (O’Rourke et al., 2016) and 5 years (Al- Al-Al-Moghrabi et al., 2018). During the 18 month follow up, the degree of relapse almost became identical. However, over a more extended period (5 years), they concluded that fixed retainers are more effective than removable retainers in maintaining mandibular anterior segment alignment.
Disadvantages
• Placement is time-consuming.
• Technique sensitive.
• Interference with the bite, especially in deep bite cases.
• Potential increases in caries rate as interdental cleaning becomes difficult under partially failed bonding material (Bearn, 1995).
• Interference with the settling of occlusion.
• Do not retain transverse expansion.
• Bonding failure: Some studies reported a high failure rate (23%-30%) (Artun et al., 1997, Renkema et al., 2011) or as high as 46.4% (Pandis et al., 2013) though 30% of patients with bond failures had an average of 0.81 mm increase in the incisor irregularity (Renkema et al., 2011, Forde et al., 2018). A systemic review reported a 12-50% failure rate, most commonly between the lateral incisor and canine (Iliadi et al., 2015).
• Fixed retainers might fail without patients knowing until relapse occurs, which can add to the clinician’s responsibility. Therefore, a removable backup retainer should also be supplied.
• A fixed retainer is inefficient in maintaining extraction space unless extended posteriorly. However, this usually increases the failure rate.
• Fixed retainers might deform and become active, resulting in some movement. A study found that using flexible spiral wire or twist-flex retainers bonded to all mandibular anterior teeth, may result in unwanted labiolingual movement or torque the lower anterior teeth (Katsaros et al., 2007).

Cervical Vertebral Maturation (CVM)
The cervical vertebral maturation method relies on the shape of cervical (C) vertebrae C2, C3 and C4 to predict mandibular growth (Baccetti et al., 2002, Mito et al., 2003). For the ease of use of CVM, several smartphone Apps have been developed, including EasyAge.
The simplified version of the CVM assessment consists of 6 stages (CVMS):
CVMS 1: The lower borders of C2, C3 and C4 are flat. The bodies of both C3 and C4 are trapezoid in shape. The peak in mandibular growth (PMnG) occurs on average 2 years after this stage (Baccetti et al., 2005).
CVMS 2: C2 lower border is now concave. C2 and C3 are still trapezoid in shape. The PMnG occurs on average 1 year after this stage 3.
CVMS 3: The lower border of C2 and C3 are concave. The bodies of C3 and C4 may be either trapezoid or horizontal rectangular in shape. The PMnG occurs during the year after this stage.
CVMS 4: C2, C3 and C4 lower borders are concave. C3 and C4 are horizontally rectangular in shape (rectangular with the long side being in the horizontal plane). PMnG usually occurs at the start of, or slightly before this stage.
CVMS 5: At least one of the bodies of C3 and C4 is square in shape. The PMnG has ended at least 1 year before this stage.
CVMS 6: At least one of the bodies of C3 and C4 is vertical rectangular in shape (rectangular with the long side being in the vertical plane). PMnG has ended at least 2 years before this age
RELIABILITY
• The inter-observer reliability for cervical vertebra maturation assessment was found to be 50 %, and intra-observer reliability was found to be 62% (Gabriel et al., 2009).
• Another study found that CVM methods have poor reliability and validity (Santiago et al., 2012). Therefore, the CVM method is not recommended for isolation (Zhao et al., 2012).

How to control incisors torque during retraction?
In a recent study using the finite element method, a team from Lebanon 🇱🇧 led by Dr Zalaquett aimed to clarify the effect of power arm length combined with additional torque on the controlled movement of anterior teeth.
🔬This study involved an adult patient who required medium anchorage after extraction of maxillary first premolars. The power arms were placed at three different levels - 3 mm, 6 mm, and 9 mm, between the lateral incisor and the canine. A retraction force of 150 g was applied from each height of the anterior hook to the first molar tube, with different levels (0°, 5°, and 10°)
of applied lingual root torque on the incisors.
💡 In conclusion, 3-mm hook with 10° of applied torque, a 6-mm hook with 5° of applied torque, or a 9-mm hook with no extra torque constituted the best combinations targeted at controlling the inclination of incisors during retraction. Extrusion

How to reduce attachments flashes (AFs) during clear aligner therapy (CAT)?
AFs affect aligners fitness. Fitness of aligners is crucial for CAT efficiency. Poor aligners fitness compromises CAT outcomes.
WHAT ARE THE SUGGESTED SOLUTIONS?
1. Avoid using attachments!
2. Bond attachments then scan and plan.
3. Etching template as suggested by some clinician.
4. I understood that Dr D Walt mentioning what is called Trial-insert (see the slides below).
MY PERSONAL OPINION
I did not try option 4 but I assume it will take extra time.
I have used option 3, it is a good option, requires etching template but the AFs were reduced significantly.
We are currently researching option 2 in an in-vitro study.
