Felicitation of Armur Women Doctors yesterday night and delivering Robotic Surgery Lecture for IMA Doctors.I used to operate in Nizamabad, Karimnagar, Jagityal, Nirmal, Warangal, Mahboobnagar, Kurnool, Zaheerabad, Miryalguda and so many other places. Dr.Laxma Reddy sir(Nizamabad) DrMadhushekar Baddam sir (Armur), Dr.Rammohan Rao sir, Dr.Naveen Sir, Dr.Sagar Rao sir, Dr.Bhoom Reddy sir(Karimnagar) were the ones with whom it started. Used to carry a black bag with important instruments and come back home at 2 am midnight sometimes in bus ,sometimes in car. So much so their children and even their grandchildren worked with me. . God knows how God let me operate in all those places. for the last two decades. IT WAS MY GOAL REACH TO RURAL that drove me.

Mentoring Robotic Colo Re**al Surgery for very skilled Surgeons , Dr.Vinay Gaikwad - CK Birla Hospital , Gurugram and Dr.Arun Giri - Aakash Hospital Delhi., today at MS Ramaiah Academy of Applied Sciences Bangalore. Paid extra 2.500 for missing Indigo flight security gate closure by 30 seconds, most unhelpful Indigo and paid another 3,000 for early check in at a hotel as their check in is illogical 2 pm. Mentoring is a commitment to Surgeons who came all the way from Delhi even though it means spending from own pocket. Yes at the end we forget everything when we are thanked for guiding able Surgeons. It does make me wonder why I got up at 2.30 am today to catch a flight at 5.00 ,

In these modern times have seen many whom we taught robotic surgery by holding their hands but never acknowledge us in public but yesterday at Delhi , Prog Subhash Khanna President of a National Association acknowledging me in his Presidential Address made me very emotional and it instilled confidence in us that our GOAL OF TEACH 10 and THEY WILL TEACH ANOTHER 10 is correct. Have also met many young surgeons who said our live surgeries motivates them.

THE FRUGALITY OF TIME OR ECONOMY- A SURGEON’S EXPEIRENCE FROM RURAL AND URBAN ONCO SURGERY PRACTICE
Having worked in both low resource and high resource settings for two decades we realized that the patients' needs and perspectives are different in metropolitan and rural areas.

FRUGALITY OF ECONOMY- Having worked in Karimnagar, Nizamabad, Mahbubnagar and in Hyderabad the patients' affordability, job, family support plays a huge role in their choice of surgical approach. 80% of patients' at Chalmeda and Shusruta cancer hospitals in Karimnagar could not afford staplers or laparoscopy, they preferred open surgery and just get rid of the tumor and go home. Affordability was a big issue in those areas. We always choose a lees expensive approach and that which is covered in government schemes. And a frugal choice would be open surgery with hand sewn steps and in fact many patients' wanted to stay for more no of days in the hospital either because of lack of care in villages or not in a hurry to join a job. They prefer to stay longer in the hospital till suture removal.

FRUGALITY OF TIME- In Hyderabad having practiced in Apollo, Yashoda, KIMS and now at American Oncology Institute the goal of patients' are different, they want to waste time in the outpatient or inpatient. They want to be seen quickly by the doctor and prefer day care surgery that is preparation at home, faster return to home and their work and same time cure their health problem. So we need to become innovative in surgical approach and be frugal in time not economy. Examples are

1. An ICG camera though expensive is used to check whether blood supply of intestine is good to join, check sentinel nodes ie., the first node of cancer spread

2. A stapler is used rather than tying as it seals the bowel so that no contamination occurs, join very low bowel like in re**um where we have to struggle to use our hands

3. The use of artificial intelligence as in robotic surgery which enhances the surgeons skills. Like in thoracic cancers like lung, esophagus, thymomas where very vital structures are there and we need to preserve them. Tremors are cancelled with these instruments so the surgeons hands are steady. The view is 3 D, so surgery is like OPEN but MINIMALLY invasive, the 7 directions movement of these instruments helps Surgeon access deeper structures and vital organs with ease and with lesser complications. Trans Oral Surgeries for post radiation recurrences has helped the patients' avoid morbid open procedure and recover faster with these lesser invasive approach.

These procedures give same cure but with faster return to home and faster return to work. Yes its expensive but the patients are frugal in time, saving what's more valuable for them with equal cure rates but with less morbid approaches. Patients' desire in metropolitan cities is to save time be back to their jobs. Many of our cases are admitted on the day of surgery and discharged on the same day like the breast, thyroid cases, hysterectomy in a day or two, colorectal cases in 3 or 4 days. To do so we need highly precise, stable advanced equipment which are expensive but the GOAL here is to be FRUGAL IN TIME not in economy.

Our experience teaches us that our GOAL is to CURE patients but we need to approach patients as per their choices, have a clear discussion with them, understand them, explain them and approach with surgical armamentarium which is FRUGAL in terms of economy or time
