ZIKA VIRUS INFECTION:
For the 2015-16 outbreak,
Zika virus outbreak (2015 - present).
Zika fever is an illness caused by the Zika virus, a member of the genus Flavivirus. Symptoms are similar to other Flaviviruses such as Dengue fever or the Alphavirus Chikungunya[1] but are milder in form and usually last four to seven days. Most cases (60-80%) are asymptomatic. The main clinical symptoms in patients are low-grade fever, conjunctivitis, transient arthritis/arthralgia (mainly in the smaller joints of the hands and feet) and maculopapular rash that often starts on the face and then spreads throughout the body. In general the disease symptoms are mild and short-lasting (2–7 days). Health officials studying the 2015 Brazilian outbreak suspect that the disease may cause microcephaly, a birth defect, in the newborn babies of infected women. However, there are very few case reports in the literature.[4]Hemorrhagic manifestations have been documented in only one instance, hematospermia(blood in semen). During the outbreak in French Polynesia there was also noted to be a concomitant increase in cases of Guillain-Barré Syndrome which could have been due to Zika but this association remains unproven.
In November 2015, reports from the Brazilian Health Ministry alerted people to a previously unknown connection between the Zika virus and cases of newborn microcephaly in Northeastern Brazil based on two cases of severely affected fetuses in whom amniocentesisconfirmed the presence of the Zika virus in the amniotic fluid. The ultrasound findings in these two fetuses, as reported on January 5, 2016, showed that both have a small head circumference (microcephaly) due to the destruction of different parts of the brain. One of the fetuses was also found to have intraocular calcifications and microphtalmia. The Brazilian Ministry later confirmed the previously suspected connection between zika infection of pregnant women and newborn microcephaly, with at least 2,400 reported cases of microcephaly in the country in 2015 as of 12 December, and 29 fatalities. According to a CNN report, Brazilian health officials were also recommending that parents consider putting off pregnancy due to the sharp increase in cases of microcephaly.
Treatment:-
There is currently no specific treatment for Zika virus infection. Care is supportive with treatment of pain, fever, and itching. Some authorities have recommended against using aspirin and other NSAIDs as these have been associated with hemorrhagic syndrome when used for other flaviviruses. Additionally, aspirin use is generally avoided in children when possible due to the risk of Reye syndrome.
Diagnosis:-
It is difficult to diagnose Zika virus infection based on clinical signs and symptoms alone due to overlaps with other arboviruses that are endemic to similar areas. Zika virus can be identified by RT-PCR in acutely ill patients. However, the period of viremia can be short and the World Health Organization recommends RT-PCR testing be done on serum collected within 1 to 3 days of symptom onset or on saliva or urine samples collected during the first 3 to 5 days. Later on, serology through detection of specific IgM and IgGantibodies can be used. IgM can be detectable within 3 days of the onset of illness. Serological cross-reactions with closely related flaviviruses such as dengue and west nile fever as well as vaccines to flaviviruses are possible. Commercial assays for Zika diagnosis are not yet available. The US Centers for Disease Control and Preventionadvises that the differential diagnosis of suspected Zika virus infection is broad and includes, in addition to dengue, "leptospirosis, malaria, rickettsia, group A streptococcus, rubella, measles, and parvovirus, enterovirus, adenovirus, and alphavirus infections (e.g., Chikungunya, Mayaro, Ross River, Barmah Forest, O’nyong-nyong, and Sindbis viruses).
Transmission:-
Zika virus is a mosquito-borne flavivirus closely related to dengue virus. While mosquitoes are the vector, the reservoir species remains unknown, though serological evidence has been found in West African monkeys and rodents.
Transmission is via the bite of mosquitoes from the Aedes genus, primarily Aedes aegypti in tropical regions. It has also been isolated from A. africanus, A. coargenteus, A. luteocephals, A. vitattus, and A. furcifer.
There have also been confirmed reports of two cases of sexual transmission and two cases of vertical perinatal transmission. Like other flaviviruses it could potentially be transmitted by blood transfusion and several affected countries have developed strategies to try and screen blood donors.
January 2016 CDC travel alert:-
Because of the "growing evidence of a link between Zika and microcephaly" the Centers for Disease Control and Prevention (CDC) issued a travel alert on January 15, 2016 advising pregnant women to consider postponing travel to the following countries and territories: Brazil, Colombia, El Salvador, French Guiana, Guatemala, Haiti, Honduras, Martinique, Mexico, Panama, Paraguay, Suriname, Venezuela, and the Commonwealth of Puerto Rico. The agency also suggested that women thinking about becoming pregnant should consult with their physicians before traveling.
January 2016 WHO recommendation:-
On 17 January 2016 the Pan American Health Organization (PAHO), the regional office of the United Nations' World Health Organization, considering the increased number of cases of congenital anomalies, Guillain-Barré syndrome and other neurological or autoimmune syndromes in zika-affected areas, recommended that its member states "establish and maintain the capacity to detect and confirm Zika virus cases, prepare healthcare facilities to respond to a possible increase demand of specialized care for neurological syndromes, as well to strengthen antenatal care".
