1. Do not use ear buds: it may push any wax further in our damage the soft skin, leading to an infection.
2. Use a towel to just dry the ear from outside.

Whenever a patient presents with a breathy voice with Hoarseness and on clinical examination we see a unilateral vocal fold paralysis, these three things are the commonest causes and we need to evaluate all three with aCT scan!

This monsoon, like every year, insects look for warm and dry places to rest. This is more common at night.
When people go on treks, camping and stay outdoors in tents or in the open, the EARS are sometimes a perfect place for insects to rest and take shelter.
Unfortunately, this relationship between man and insect is doomed because it results in severe pain and inflammation in the ear.
It is imperative to remove such creepy crawlies as it may result in permanent damage like ear drum rupture.
Visit your nearest ENT and please refrain from self medication or trying to remove it yourself. Many a times people slap the ear trying to get it out, but that results in more trauma.

When we start out in MBBS and our clinical learning starts—the first thing we are taught is to ASK QUESTIONS.
Just taking what the person says and treating them for that is not the way. Sometimes to get to the root cause we need to go in depth and ask questions.
Certain cases have occurred recently, where despite asking relevant questions, the patient has failed to reveal relevant information which may have been life threatening.
Always tell the doctor about the medications you take, what other stuff may be going on which you feel may impact your treatment, no matter how minor.
ENT post 49: Swimming and The Ears
Summer time brings with it a lot of patients with itching or pain in the ears especially after swimming.
Majority of people who swim do not have any such trouble, but the small percentage of patients who do suffer from ear issues have the same symptoms year after year.
So to prevent such things from happening, here are a few ways you can protect your ears this summer.
ENT Post 48: DUST MITES
The skin prick test result shows strong positive reaction to testing with Dust mites.
There are 3 dust mites: D. Pteronyssinus, D. Farinae and Blomia Tropicalis.
These are microscopic insects which live in our bedding, mattresses, sofa crevices, carpets etc.
People who are allergic to “dust” are actually allergic to these mites. It is impossible to get rid of them completely but there are several measures to reduce their population.
One such measure is ensuring adequate sunlight and ventilation in the house.
Another one is using mattress and pillow casings. These will prevent the mites from coming in contact with the patient and thus prevent allergic reaction.
We conducted a CME on and its intricacies for a group of doctors this last Friday.
It is an area we specialise in and look forward to sharing our knowledge in future interactions as well.

Here are some ways to protect yourself from loud noises and resultant permanent damage to your hearing.
ENT post 47: Skin prick test
The gold standard in allergy testing is the Skin prick test. A small amount of the test material is introduced into the subcutaneous layer of a patients skin, which is rich in mast cells. (The cells responsible for release of histamine-which is the substance responsible for the symptoms of an ALLERGIC REACTION.)
If a patients skin does show a response like the one in the picture, it means the test is positive (when compared to a test and control substance).
I have tested the 3 Dust mites on my own skin, to check if I am allergic to it or not. Clearly there is a response, which means I am allergic to the dust mites.
One can test for any allergen, including mites, pollen, fungi, food, cockroach and insects and dog and cat amongst other pets.

ENT post 46: Loud noise exposure can lead to many consequences.
The WHO has guidelines for industries and factories where there is significant noise exposure, in order to prevent their employees from suffering from damage.
These include limiting the time of exposure and providing protective ear muffs of certain standards.
But in today’s world, each one has possibility of exposure to loud noise due to use of personal devices.

ENT Post 45: Capillary Hemangioma a.k.a Vascular Malformation
This is a abnormal blood vessel formation which may bleed. Commonest location is the head and neck.
The symptoms include bleeding, pain and swelling. It may also gradually increase in size and lead to a cosmetic defect.
Here we show a patient who developed this swelling and was bleeding. An MRI was done which showed no significant feeder vessel.
Hence it was treated conservatively by injection of a sclerosant. The swelling reduces to to inflammation and then gradually falls off, as visible in the after picture.

STORY OF A PATIENT
"So one night I spent quite a while searching for my headphones. This happens often as my toddler likes playing with the box they come in. Finally gave up and went to bed.
Next morning luckily before I went to the loo I had a peep in--- and found one of them there. Got them out and dried them up. Luckily the Waterproof claim by the company was proven."
Ear phones are placed anywhere and then they go into your ears. They are snug fitting and may abrade the skin if worn for long periods. This may give rise to infections.
So to prevent this:
1. Clean the tips of ear phones regularly.
2. Store them in clean place.
3. KEEP OUT OF REACH OF TODDLERs

ENT post 44: NASAL POLYPS!!!
This is a CT scan of a case where the patient had Nasal obstruction; both sides and which was worse at night when they sleep.
No complaints of runny nose or recurrent cold.
A quick examination showed us a greyish white mass hanging behind the soft palate.
The CT scan showed polypoidal mass at the posterior end of the septum (middle bone of the nose which separates it into two cavities)
Patient was taken up for surgery AFTER giving adequate medication and patient was sure it was not getting soured by medication.
The polyps were attached to the lateral wall of the nose; and were neatly excised with minimal blood loss.
Takeaway: Some cases may need a small surgery to completely relieve symptoms while some may need a lot of medication following surgery
Example: Fungal sinusitis.
ENT post 43: Sinusitis!!!!
This is a CT scan showing
1. Maxillary sinus which is completely opaque—THERE IS NO AIR IN THE SINUS—NOT GOOD)
2. Inferior turbinate hypertrophy (See our previous post to know more about that)
3. Co**ha bullosa (explained in previous post)
Besides that there is a DEVIATED SEPTUM to the right. Which means the nasal bone is deviated to the right.
What does this medical jargon mean—-Patient has the following symptoms:
NASAL BLOCK, NASAL OCNGESTION and RECURRENT COLDS.
Treatment is Medical and Surgical for such a case!

ENT Post 42: A CT with 3 findings. What needs correction?
This patient had typical complaints of and recurrent and
They were getting minimal benefit with
Their CT Shan showed
1. RIGHT CO**HA BULLOSA: When a normal bone is filled with air and it BALLOONS up.
2. HYPERTROPHY OF BOTH INFERIOR TURBINATES: The lower bone in the side of the nose which—if enlarged—extends to the back of the nose and discharges mucous into the throat, causing the constant IRRITATION and CLEARING OF THROAT.
3. HYPOPLASTIC LEFT MAXILLARY SINUS WITH OPACITY: The sinus is smaller than the opposite side and it means the sinus does not have air, but is filled with something else—could be mucous or polyps.
Such a patient will benefit with surgical correction of the above 3 problems.
To our surprise when we operated this patient we found something unexpected in the sinus.

ENT Post 41: What is the problem with persistent ear discharge or UNTREATED EAR DISEASE??👂❌ Lateral Semicircular Canal--the balance organ damaged by disease.🎡🎡
is a condition which destroys the bone of the ear slowly. It may present with many complications, such as:
1. Severe hearing loss
2. Dizziness when blowing the nose or while in the toilet
3. Infection spreading to the brain
Here we have a patient with persisting ear discharge for several years, and dizziness of recent onset. the CT Temporal bone shows extensive cholesteatoma and erosion of the Lateral semicircular canal.
That structure is responsible for maintaining balance while we rotate our heads.
If there is erosion of that bone, it may lead to dizziness when we blow our nose or strain in the toilet. It occurs due to transmission of pressure to the inner ear.
Treatment will involve for sure. The disease will be removed and the bony defect repaired.

ENT Post 40: An unusual cause of
A 21 year old male presented with change of voice for the last one month. He is not a smoker.
An endoscopy revealed whitish lesion covering the vocal cords.
Patient was given treatment for (inflammation of larynx) with a suspicion of (pre-malignant condition).
With no improvement after 10 days; patient was planned for surgery. But we consulted our senior and on his recommendation started the patient on medication for a fungal infection.
Lo and behold; in 10 days patient was absolutely normal.

Always be open to ASKING for help when faced with a doubt.

ENT post 39: Facial Palsy—one side of the face gets paralysed.

There are numerous causes of facial palsy, from trauma to tumours to infections.
Q. What is RAMSAY HUNT SYNDROME?
A. It is facial palsy caused by the HERPES ZOSTER VIRUS. It is also known as HERPES ZOSTER OTICUS.
There is weakness of one side of the facial muscles along with vesicles in the ear which are extremely painful.
The treatment needs to be aggressive and it consists of steroids and antivirals.
The recovery rate as compared to IDIOPATHIC/BELLS FACIAL PALSY (one without any cause) is
Bell’s palsy 98% at 9months.
Ramsay Hunt 75% at 9months.
Even though it is only a viral infection, it can lead to permanent consequences.
Be aware and seek treatment asap. Otherwise “Is it too late to say I’m sorry..?”
