Syndactyly Part 1: Hands intertwined by nature's design 🤝💕
This little child and her mother were in my opd when I went to the rural phc.
The fused fingers (syndactyly) in her left hand caused her a little discomfort.
She was therefore instructed to come see us at our main hospital, where she underwent a standard medical examination and pre-anesthesia evaluation.
She was listed under plastic surgery for syndactyly release operation.
You can see preoperative and postoperative pictures.

OVARIAN CYST

√Ovarian cysts are fluid-filled sacs that may be simple or complex.

√Ovarian cysts can be complicated by rupture, hemorrhage, and torsion, which are considered emergencies.

√Ovarian cysts can occur at any age but are more common in reproductive years and have an increased occurrence in menarchal females due to endogenous hormone production. 

√Simple cysts are the most likely to occur in all age groups, and mixed cystic + solid and completely solid ovarian lesions have a higher rate of malignancy than simple cysts.

√Risk factors for ovarian cyst formation include
1.nfertility treatment- Patients treated with gonadotropins
2.Tamoxifen
3.Pregnancy- form in the second trimester when hCG levels peak.
4.Hypothyroidism
5.Maternal gonadotropins
6.Cigarette smoking
7.Tubal ligation- Functional cysts have been associated with tubal ligation sterilizations.

EDH - Epidural hematoma
∆ Is a condition in which there is collection of blood within the space between the outer layer of the dura mater and the inner table of the skull.
∆ It usually occurs as a result of trauma to the brain either due to rta or fall from height.
∆ It is confined by the lateral sutures (especially the coronal sutures) where the dura inserts.
∆ It is a life-threatening condition, which requires immediate image diagnosis and sometimes emergency surgery (evacuation of hematoma), for a better outcome.

SPINAL CORD TUMOUR

36year old patient came to us with complaint of severe backache and difficulty in walking

MRI spine was done and he was diagnosed with intradural spinal cord tumour (? Schwannoma)

Patient was posted for excision. Post Operatively patient has bilateral lower limb power 3/5, with limb physiotherapy he may regain 5/5 power.

MARKER X-RAY FOR SPINAL CORD TUMOUR

We placed a coin approximately at the site spinal cord tumour (seen in the mri spine) and shoot a xray so that the vertebrae which is involved can be identified easily and we have an easy to take incision.

In this picture coin is nearly placed opposite to D8 vertebrae, as the spinal cord tumour is extending D7-D8.

STSG - "Split Thickness Skin Grafting"

A 16year old boy came to the casualty with a/h/o RTA while riding a mini four wheelers which occured 2days before visiting to us. Patient has dead, necrotic tissue with pus filled dirty foul smelling wound.
We first posted him for wound debridement with VAC application and after 1 week, we again posted him for VAC removal with skin grafting.

CHOLELITHIASIS -"Gall Bladder Stones "
A 55year old female came with the complain of pain in right upper abdomen since 2 months, radiating towards back. There was a h/o burning micturition, h/o nausea, h/o loose stools.
Patient was vitally stable.

On palpation : per abdomen was soft, Non-tender. There was a Vague lump of around 8 x 8 cm present in Right hypochondrium which was mobile, cystic in consistency, non tender, surface was smooth. Bowel sounds present all over abdomen.

(1)Xray Abdomen erect was WNL
(2)USG Abdomen Pelvis: Grossly distended gall bladder with multiple gall stones and tumefactive sludge was seen.
(3)Upper GI Endoscopy was WNL
(4)CECT Abdomen Plevis
Cholelithiasis with dilated cystic duct and CBD.

(5) On MRCP: Cholelithiasis with choledocholithiasis with mucocele formation was seen.
Flow void in cystic duct was likely suggestive of calculus

OPERATIVE PROCEDURE done in this patient was "OPEN CHOLECYSTECTOMY with CBD EXPLORATION DONE UNDER GA + EPIDURALEPIDURAL with T-tube insertion".

CHOLELITHIASIS
The term cholelithiasis refer to the presence of (gallstones) hardened deposit within the fluid in the gallbladder.
(A small organ under the liver.)

In this video I am making an incision over gall bladder, so that i can take a look at the stones.
Normal size of gallbladder is 7x4 cm.
But in this patient due to presence of stones, gallbladder got distended (swollen/ increased in size) and was of size 15x10cm.

According to World Federation for Mental Health (WFMH) the theme for World Mental Health Day 2020 is 'Mental Health for All: Greater Investment – Greater Access'.

The theme stresses on the fact that mental health is a human right and that it is time mental health is made available to all.
Dr Tedros Adhanom Ghebreyesus, Director-General of the World Health Organization stated:

“The world is accepting the concept of universal health coverage. Mental health must be an integral part of UHC. Nobody should be denied access to mental health care because she or he is poor or lives in a remote place.”

Peau d' orange appearance.
It means the appearance of the skin over breast is similar to that of 'skin of orange'.
It is a dimpled or pitted and thickened appearance of skin over the breast due to blockage of subcuticular lymphatics with edema of skin.
There will be redness over breast.
It is a classical sign in case of carcinoma of breast.

To gather information regarding your heath and medical condition is very important but at the same time, our resources should be reliable, and not misleading. Otherwise it will lead to medical anxiety.

Taking care of Yourself
* Use gentle products to clean your face.
* Use electrical shaver instead of razors.
* Avoid shaving cream that burns or stings.
* Use good moisturizer after shaving.
* Use sunscreen with SPF 15.
* Avoid products that contains ALCOHOL, WITCH HAZEL, FRAGNANCE, ASTRINGENTS or EXFOLIATING PRODUCTS.
* If you don't know your trigger factors, make a diary and keep records of your daily activities, diet and flare-ups.

Hypothyroidism (underactive thyroid) is a condition in which your thyroid gland doesn't produce enough of certain crucial hormones. It is more common in females than males.
Over time, untreated hypothyroidism can cause a number of health problems, such as obesity, joint pain, infertility and heart disease.
So see your doctor if you're feeling tired for no reason or have any of the other signs or symptoms of hypothyroidism, such as dry skin, a pale, puffy face, constipation or a hoarse voice.
If you are getting treated for hypothyroidism, always schedule the follow-up with your doctor to ensure clinical response of therapy and dose adjustment.
RISK FACTORS:-
*Are a woman
*Are older than 60
*Have a family history of thyroid disease
*Have an autoimmune disease, such as type 1 diabetes or celiac disease
*Have been treated with radioactive iodine or anti-thyroid medications
*Received radiation to your neck or upper chest
*Have had thyroid surgery (partial thyroidectomy)
*Have been pregnant or delivered a baby within the past six months.
