Update: CORONAVIRUS (SARS-COV-2)
By Shreya Kanta
As soon as you see the name corona, you panic or you are fed-up mostly due to media frenzy outlets and you rather not read anything about it.
I would suggest you do neither. But instead know the facts with a calm mind and take good care of yourself and your families.
The virus SARS-COV-2 belongs to family of corona virus and is named as such due to it's genetic resemblance to SARS like- bat coronavirus, from which it is likely to originate.
The virus causing disease is officially named as
COVID-19.
It is the seventh known coronavirus to infect humans. Other coronavirus capable of causing illness range from
common cold, middle east respiratory syndrome (MERS), subacute respiratory syndrome (SARS)
The first known infection from virus was discovered in Wuhan, China. The original source of viral transmission to humans remain unclear.
Human-to-Human transmission occurs due to respiratory droplets from coughs, sneezes within a range of about 6 feet (1.8m)Indirect contact is through contaminated surfaces. The time between exposure and symptom onset is typically 5 days and may range from 1-14 days.
Symptoms are usually, cold, cough, fever, shortness of breath and rarely fatigue, headache and muscle or joint pains.
Contact nearest medical facility or nearest doctor if you have any symptoms.
Diagnosis/ tests for the virus is by standard method called RT-PCR (reverse transcriptase polymerase chain reaction) - It is done on respiratory samples collected by sputum or nasopharyngeal swab.
It can also be done by antibody test kit, which use blood samples from the infected people.
As of now, there is no vaccine available to prevent COVID19.
Treatment is symptomatic; by treating symptoms. No specific anti-viral drug has been developed. Antibiotics do not help, as they do not work against viruses.
Infected individuals need to stay isolated till they have recovered.
Best way to prevent it is to follow recommendations given by centre for disease control and prevention (CDC):
Frequent hand-washing with soap and water for 20 seconds especially after going to bathroom, before having food, after blowing your nose, coughing or sneezing.
Avoid touching your face, nose, mouth.Avoid close contact with people who are sickCover your cough, sneeze with tissue, then throw the tissue in trash.Clean and disinfect frequently touched objects or surfaces using cleaning spray or wipeAvoid unnecessary travel and crowded areas.DONOT wear mask if you do-not have any symptoms, as they are not effective, as they are not designed for air-borne particles transmission prevention.
Special N-95 respirators/ face-masks are used by health care workers, who take care of infected people and by people who have symptoms to prevent spreading the disease.
With the virus outbreak, we are on verge of an infodemic of misleading information. Social media like WhatsApp and Facebook add fuel to it.There are number of conspiracy theories, quack treatments, rumours and outright lies.
For instance, applying cow dung on your body, bleach, eating garlic, turmeric, ginger and sesame oil and many more home remedies.
There is no harm in eating garlic or turmeric, but there is a risk which provides false sense of security and undermine efforts of healthcare workers to curb the spread of the disease. And all of these permit quacks to fill the vacuum when scientists need to test potential vaccines and treatments.
Statistics show that case fatality rate of Covid-19 is 3.4%, which is very low as compared to SARS, MERS, Nipah, Ebola, H1N1- which had 60-70 % of fatality rate. And also, infected people dying due to covid-19 are very few compared to deaths due to road accidents, stroke, heart attacks and starvation deaths which occur everyday.
It's easy to go overboard citing an increase of caution, but caution shouldn’t be proxy for lazy thinking. When we act out of caution in the absence of proper risk assessment, the default is overreaction.
When people panic and hoard supplies, scarcity results.
So, Think. Pause. Reflect. Act.
Be kind and serve humanity. -Dr. Shreya Kanta

We are changing a few things in Neurosurgery...
1. We are bringing an end to a hundred-year-old neurosurgical technique for trauma and introducing microsurgery into trauma thereby bettering results and making good training imperative. No more saying that we will stick to an old surgery that any idiot can do with a garden hose and an industrial drill for trauma...
2. We are challenging the system of evidence-based medicine in a wise manner that concludes evidence-based medicine should be based on common sense and not the other way around. Now, if any industry wants us to do something, they do a trial and get evidence and we are ready to put enough metal in the spine to sink a ship or wait and watch in AVMs which could be taken out if one has the expertise... The "evidence-based experts" are still called Neurosurgeons and not Neuroscientists... Over a few decades, this has created so much confusion amongst young neurosurgeons that they end up half neurosurgeons and half neuroscientists... and mostly they are not able to do their jobs properly. They worship centres which do normal surgery... and also worship the data experts who do surgery with P values...
3. We hypothesised how the cooling and cleaning happens in the brain and though this idea was radical a few years back, more and more evidence is coming in to support this and we believe there will be very interesting research happening in the coming years for head injury, degenerative diseases, sleep, etc.
4. We believe that the instruments and viewing in Neurosurgery is outdated and we are trying to develop hybrid Neurosurgery where the Endoscope is fully integrated into the microscope which will effectively become an Exoscope with superimposed navigation.
Many new microsurgical instruments will replace the current old system of clipping, anastomosis, tumour removal and even craniotomy. We are working on some very exciting avenues and this will make Neurosurgery easier. We are doing our best so that the companies which make these don't make billions of dollars and we remain to be their minions promoting them to make money and prevent most of the world from buying even simple microscopes so that they have to depend on WFNS grants to buy a stupid scope.
While the language has been harsh, I believe this is what I wanted to convey and any less would not convey what I truly feel.
Many amongst us in the neurosurgical community are also making fantastic contributions to advance Neurosurgery and I hope they can talk about it here and other forums. Let us learn and collaborate...

Dear Friends,
We are sharing a short video on the NISSL-2 surgical skill workshop conducted on 21st & 22nd December 2019 in S N Medical College, Bagalkot (Karnataka). We were supported by an enthusiastic team and experienced faculty members. Delegates from different parts of the country attended the workshop and shared their experience regarding the same. The highlight of the workshop was hands-on Ventriculoscopy, which is the first of its kind in the country. We are grateful to Dr Sanjeev Kolagi (HOD, Dept of Anatomy, SNMC) and his team for collaborating with us and supporting us.
