The simplest appearing thing can often be the hardest...

Yesterday was certainly a day!

Patient presented for a #6 (Upper right canine) Extraction. Simple right?
Wrong! ๐ฅต
This ended up becoming a surgical extraction. Hereโs why:
1. The canine had an extremely long root
2. The canine eminence (bone) was extremely thick
3. The patients bone was very dense as she has a history of chewing on chicken bones etc.
4. There were no adjacent teeth to provide good leverage for luxation
5. There was a large mesial cavity that made the crown prone to fracture
What a recipe for difficulty! ๐คฏ
I reflected an envelope flap to access the tooth, removing minimal buccal bone and a little palatal (trough technique), and finally delivered the tooth with upper anterior forceps!
All done with 2 carpules of 2% lidocaine with 1:100,000 epinephrine
5 sutures were placed with 3.0 gut!
The patient was such a trooper!! ๐
Today I got a call from her saying how much she appreciated everything I did for her yesterday because she was so nervous but it went so well ๐ฅฐ๐ฅฐ

Letโs talk about how to brush the correct way!
โUp and down, and scrubbing side to side are a BIG no-no.โ
When you brush like this you end up missing the gum line where the plaque likes to hide. It can also cause gum recession leading to sensitivity!
So let me tell you the correct way!โ
Using a ๐๐ผ๐ณ๐ ๐๐ผ๐ผ๐๐ต๐ฏ๐ฟ๐๐๐ตโฆ
First:
๐ญ. Letโs divide the mouth into ๐ฐ ๐พ๐๐ฎ๐ฑ๐ฟ๐ฎ๐ป๐๐ (see pic 2)
2. Each quadrant should be brushed for ๐ฏ๐ฌ ๐๐ฒ๐ฐ๐ผ๐ป๐ฑ๐

โข We want to make ๐ฐ๐ถ๐ฟ๐ฐ๐๐น๐ฎ๐ฟ motions, and ๐ณ๐น๐ถ๐ฐ๐ธ๐ถ๐ป๐ด motions to ensure we remove all the plaque and bacteria.
This brushing routine should take ๐ฎ ๐บ๐ถ๐ป๐๐๐ฒ๐
After that we want to:
โข ๐๐ฟ๐๐๐ต ๐๐ต๐ฒ ๐๐ผ๐ป๐ด๐๐ฒ for about 30 seconds
โข ๐๐น๐ผ๐๐ (however long it takes you to get around the whole mouth).
Your whole oral hygiene routine should take between ๐ฏ-๐ฑ ๐บ๐ถ๐ป๐๐๐ฒ๐!
Using this method ๐๐๐ถ๐ฐ๐ฒ ๐ฎ ๐ฑ๐ฎ๐ ensures you donโt miss any teeth when brushing, and keeps your mouth and teeth clean and healthy!

Many are celebrating #420 today. But you may be wondering, what has ma*****na got to do with Dentistry?
Many Dentists, including myself, always ask patients if they smoke ma*****na. Not only is it important because it has an effect on gum health, but because it is known (through experience) by clinicians to directly affect the effectiveness of local anaesthetic (LA) aka the โnumbing injectionโ.
We give LA for many reasons, including fillings and extractions to ensure that the patient doesnโt feel pain as we perform the treatment.
Hereโs where ma*****na comes inโฆ experience has taught us that smokers almost always require MORE anaesthetic to become profoundly numb, or in many cases, CANNOT GET NUMB AT ALL.
It is unclear how long ma*****na is detectable in the body and how long the effects of smoking it last. In my experience it can take up as many as 3 weeks of stopping smoking before treatment in patients who smoke frequently (3 blunts a day) for it not to affect local anaesthetic and be โoutโ of your system.
In my experience, it is harder to get numb if:
- You smoke frequently
- You smoke closer to the day of treatment
- You smoke a high dose/strong strain.
That being said, Iโve had patients where even smoking ONCE a day totally prevents LA from working.
Unfortunately, research in this are is limited and hard to come by. Nevertheless, experience suggests that smoking ma*****na before dental treatment is a big NO NO. More often than not, patients will have difficulty getting numb.
I encourage people to be open and honest with their Dentist (and other health care professionals) about their ma*****na use.
Pro Tip: Better safe than sorryโฆ DONโT SMOKE MA*****NA BEFORE COMING TO THE DENTIST!

Things that make you say WOAH! ๐ฏ
What do you do when you have a wisdom tooth with roots overlapping the inferior alveolar nerve (IAN)?
I extracted this #17 this week.
Roots that have proximity to the IAN; as can be seen by the overlapping nerves on the xray, increase the risk of exposing or damaging it during the extraction. This makes what would have been a simple extraction, a more complex case. This damage can be partial temporary damage or, in cases of complete severance of the nerve, permanent damage.
Risk of IAN damage can be reduced with careful surgical technique.
Tooth removal should be performed with an elevator, focusing on luxating the tooth completely loose, avoiding applying apical pressure with forceps as to avoid the risk of nerve compression or stretching.
Itโs also really important to achieve proper profound anaesthesia.
In this case I used 2 carpules of 2% Lidocaine with 1:100,000 epinephrine and 1 carpule of 4% Articane with 1:100,000 epinephrine. This was for inferior alveolar nerve block, local infiltration, PDL block, long buccal never block, and lingual nerve block.
Though the roots were long, their fusion made this extraction less challenging.
It took 20 minutes in total.

Have you ever been to the dentist, finished your appointment, and your dentist rattled off all these things that need to be done next time, but used all these terms youโve never heard of?
When we talk about ๐ซ-๐ฅ๐๐ฌ๐ฆ, there are several different types we can do. Each type of x-ray helps us to see different things and diagnosis different lesions.
๐๐ถ๐๐ฒ๐๐ถ๐ป๐ด๐
- These help us to detect caries (tooth decay) inbetween the teeth and interproximal bone height useful for diagnosing periodontal disease.
๐ฃ๐ฒ๐ฟ๐ถ๐ฎ๐ฝ๐ถ๐ฐ๐ฎ๐น
- This helps us to see the entire tooth including the roots, in relation to the jaw bone. We can detect if there is an infection at the apex of the root, of if there is any damage to the root, including fracture or resorption.
๐ฃ๐ฎ๐น๐ฎ๐๐ฎ๐น/๐ข๐ฐ๐ฐ๐น๐๐๐ฎ๐น
- This is used to help us detect whatโs occurring at the roof of the mouth. This type of x-ray can be used to detect fracture of the tooth, cleft palates, cysts and abscesses, and fractures of the jaw bones.

- This helps us see all the teeth in the mouth, in relation to each other and other anatomical structures in the face. Useful for detecting impacted teeth and complications of the of the jaws and sinus, including rumours and cysts. We can use this type of x-ray when doing major dental work such as orthodontic braces.

Operculectomyย is the surgical procedure of removing the operculum, or the flap of gum that partially covers a tooth. This procedure is done to treat pericoronitis, a condition characterised by pain and inflammation of the operculum.

Letโs talk about DENTAL ABCESS! ๐ฆท๐ฆ
A ๐ณ๐ด๐ฝ๐๐ฐ๐ป ๐ฐ๐ฑ๐๐ฒ๐ด๐๐ is a collection/pocket of pus caused by bacterial infection, that can form in the gums, bone and teeth.
I had a patient come in last week complaining about a gum swelling and tenderness. to see whatโs going on.
*HISTORY* ๐
44 yr old male patients history of smoking in his 20s. Gum swelling and 3/10 tenderness for the last 3 days. Orofacial examination revealed palpable tender left submandibular lymph node.
Exam revealed generalised moderate supra- and subgingival calculus and several carious lesions, including #15 MO.
Tooth abscess can occur for a few reasons:
- ๐๐๐๐๐ ๐ณ๐๐๐๐ข: poor oral hygiene (brushing and flossing) can cause caries (dental decay), which erode the enamel and dentin, allowing bacteria into the pulp (innermost later where nerves and blood vessels run).
- ๐ฟ๐๐๐๐๐๐๐๐๐๐ (๐๐๐) ๐ณ๐๐๐๐๐๐: Gum disease causes the gums to pull away from the teeth. This leaves deep pockets where bacteria can grow. If tartar builds up too much, or if food gets stuck in the pockets, pus forms. If the pus can't drain, it forms an abscess.
- ๐ฒ๐๐๐๐๐๐ ๐๐๐๐๐: with the inside of the tooth exposed, bacteria has direct access to the pulp, leading to infection
Depending on the severity of the abscess, we can treat it several different ways:
- Antibiotics
- Extraction
- Root Canal
- Incision and Drainage.

โผ๏ธEDUCATIONAL POST - CROWNS & VENEERSโผ๏ธ
Not all that glitters is gold...
Iโve seen an increase in young people getting crowns and veneers. As the article states, โshaving downโ your teeth is permanent and irreversible. The only solution is to place a prosthesis (crown or veneer etc).
Many describe getting veneers, but the level of reduction seen in the pictures from the article are actually for CROWNS!
to see the difference.
VENEERS usually cover one surface of the tooth, and can be as thin as 0.5mm. Very little of the tooth needs to be โshaved downโ (aka tooth reduction). We can also do โno-prepโ veneers, which donโt require any tooth reduction.
CROWNS cover all surfaces of the teeth, and usually require 2mm of tooth reduction, thus being a more destructive procedure.
Although long lasting, they may need replacement after 10-15 years which can be costly. For crowns, teeth may eventually require root canal or extraction as each time the crown is replaced, more tooth structure is removed. Eventually, the tooth may be deemed โnon-restorableโ as there may be insufficient tooth structure left to restore it. Patients may also experience sensitivity due to compromised tooth integrity and increase in temperature of the tooth during the procedure.
Though the crown/veneer cannot develop decay, the tooth underneath it is still susceptible, particularly where there are loose or open margins where the tooth and crown meet. Dental plaque can collect around this margin, introducing bacteria to the tooth which can lead to decay. So it is particularly important to have good oral hygiene (brushing and flossing) following a crown or veneer.
So, whatโs good about crowns and veneers? Crowns and veneers can be excellent treatment options for those with already compromised teeth, including teeth that are already heavily restored, chipped, broken, crooked, decayed, root canal treated, discoloured, small, or have other abnormalities.
For uncompromised โhealthyโ teeth, there are many other less invasive procedure that can improve your smile:
- Whitening
- Orthodontic treatment
- Composite bonding

to see the HUGE 2 rooted premolars that came from these sockets.
I extracted #4 and #5 (right maxillary 1st and 2nd premolars) using 1 carpule of 2% Lidocaine with 1:100,000 epinephrine for right MSA nerve block. ๐
As you can see, I placed 1 interrupted suture using 3.0 absorbable catgut to ensure proper reattachment of the papilla.

My 63 year old female patient came in for a routine cleaning (oral prophylaxis)
Guess what, sheโs a frequent tea drinker! The results of which can be seen on the lingual surfaces of her mandibular anterior teeth.
I performed the prophylaxis using an ultrasonic scaler to remove those stubborn stains.
