This case of a class III, with a dilacerated & malformed central incisor, missing upper lateral incisor, missing lower second premolar, midline shifts in both arches, reduced overjet & overbite, & mesially tipped lower 1st molar, was handled with mindfulness & care (especially around the UR1!) to arrive at this final result.

The patient is enjoying her pretty smile & healthy functional bite, through those crucial adolescent years & beyond, and we couldn’t be happier!

Sharing this very special case & dear patient to my heart. Presented first to my office at the age of 9 complaining of missing top teeth (UR1 & UR2), & mobility in UL1.
المرحلة الأولى من علاج حالة انطمار سن أمامي علوي :)

Patient was in her mixed dentition with a reported history of trauma to the dentition at the age of 2.5 yrs.
She had a skeletal class III base, clinically unerupted UR1 + UR2, premature loss of upper Cs & lower primary molars, & Grade I mobility on UL1 due to traumatic occlusion (only anterior tooth in occlusion)

Her OPG & subsequent CBCT revealed:
-UR1: Crown dilaceration + enamel invagination (dens in dente type I)+ bulbous & short root morphology + pulp stone + buccal cortical bone dehiscence
-UR2: missing + presence of a complex odontome -UL1: pulp stone
-Ul2: Dens Invaginatus type I
After endodontic & restorative consultation, and the discussion of treatment options with the parents; the decision was made to go ahead with tooth exposure & traction, root canal treatment as soon as accessibility allows, & crown buildup.

The patient was scheduled for GA for the exposure procedure & extraction of the odontome. She was also fitted with an URA with a posterior bite plane to disocclude UL1 until the surgery appointment.

⭐️Tooth was successfully tracked with a partial fixed appliance & a gold chain, and went under several sequential decoronation procedures until it was accessible for RCT & full crown restoration. It was also nudged from palatal occlusion (Class III) into a positive overjet & overbite, & the patient was fitted with a fixed 1-1 retainer & a removable one after traction was completed and while waiting for the rest of permanent dentition to erupt.

☺️At the end of stage 1, the patient & mother were ecstatic that she could finally smile with two front teeth instead of one! :)

تحريك الأسنان بأي جهاز تقويمي (ثابت أو متحرّك شفاف) هو عبارة عن عملية بيولوجية تقوم بها خلايا الجسم.

This sweet girl came to us with her father complaining about her canine tooth that erupted higher than the rest of her teeth and didn’t come down. She also wanted her front teeth to be aligned as they were rotated and she didn’t like their appearance.

🌟She was treated with braces in both jaws. No tooth extraction was undertaken but rather an upper jaw expansion to accommodate the teeth and correct their inclination (long axis).

This young man came to me seeking a solution to his protruding top teeth, and a more harmonious and symmetrical smile. He had discrepancy in the size of his upper lateral incisors, namely a small one on the right and a peg-shaped one on the left.

🌟Orthodontic treatment was provided to fix the smile and occlusion. Since the overjet was large and space was needed in the arch to correct it, the choice was made to extract the defected lateral incisors. His canine teeth were suitable candidates for gradual cusp reduction and camouflage, and the extraction space allowed successful retraction of his protruding teeth into a more aesthetic alignment. Gingival margin correction was not planned as the patient had a low lip line. His final result showed a much more symmetric smile and a beautiful incisal show and inclination that made him happy.

مراجعنا زارنا وهو يرغب بعلاج البروز في أسنانه العلوية والحصول على ابتسامة متناسقة وتماثل بين الشق اليمين واليسار. عند الفحص تبين وجود خلل في حجم القاطع الثاني حيث كان صغير الحجم على اليمين، وذل شكل مستدق الرأس على اليسار.

🌟تم العلاج عن طريق التقويم لتعديل الابتسامة والإطباق. وبسبب حجم البروز الكبير الذي يحتاج وسع في الفك لتصحيحه، تم اختيار خلع القواطع الثانية على الجهتين لتوفير الوسع وإرجاع الأسنان للخلف، وتمويه الأنياب بطريقة بسيطة لتبدو كالقواطع. بعد العلاج حصل المويض على نتيجة جميلة متماثلة الشقين وبميلان أسنان ملائم لوجهه.
