A patient with a dorsal hump and overprojected droopy tip. The hump has to be lowered incrementally to avoid over-excision which leads to an unnatural ski-sloped nose. Also, to get a natural result, the level of the new dorsum has to match the new degree of tip projection and rotation.

Tip asymmetry is one of the commonest causes for revision rhinoplasty. This patient had 2 previous rhinoplasties that resulted in an asymmetric tip with bossa formation on the left side and a distorted nasal base and asymmetric nostrils.
