Abdominal Pain and Symptoms

Abdominal pain can strike at any time, and sometimes it can be difficult to determine whether or not it is an emergency. This is because abdominal pain can occur as a result of numerous causes – including indigestion, intestinal gas, food poisoning, menstrual cramps, heartburn, gallstones, pancreatitis, inhibited blood supply to the colon (also known as ischemia), stomach cancer, kidney stones, inflammatory bowel disease, umbilical hernia, appendicitis or a urinary tract infection.

If abdominal pain is mild and does not inhibit a person’s ability to function, or does not otherwise compromise quality of life, people generally choose to carry on, opt for an over-the-counter or home remedy, or schedule a non-urgent appointment with their medical practitioner. Sometimes, however, it is not easy to decipher whether or not your abdominal pain requires immediate medical attention. Abdominal pain can sometimes indicate serious illness and thus should not be ignored.

If your abdominal pain is severe or persists more than 24 hours, it is typically recommended by professionals that you see a doctor or go to the ER right away. You could very well be dealing with something as simple as cramps or minor food poisoning, but it would be beneficial to have a second opinion to rule out life-threatening illness. Pay close attention also to accompanying symptoms (for example fever, painful urination or constipation) and bring them to the attention of the physician. You should also call a doctor or get to a hospital immediately if your abdominal pain is accompanied by bloody diarrhea, bloody vomit, black stools, or fever above 101ºF / 38.3ºC.

A severe pain in the abdomen could mean appendicitis, which can be a life threatening problem. However, appendicitis is often difficult to diagnose in the early stages – and by the time it has become severe, it can be very dangerous. For example, the younger brother of the author of this article was diagnosed with the stomach flu by his doctor. The doctor sent him home, where he stayed drinking chicken soup and tea, thinking all he needed was rest and home remedies to recover. However, his condition worsened. When the pain became so severe that he was rushed to the Emergency Room, he received a far more extreme diagnosis: His appendix had ruptured and he needed an urgent laparoscopic appendectomy to save his life.

Although there is often “overlap” between the areas of the abdomen that may be in pain due to a particular cause, this general chart may be of value in a preliminary assessment. However, we do stress that (as with the rest of this website) this is not medical advice, and that appropriate professional medical care should be sought, especially in cases of greater severity.

THE PELVIC INFLAMMATORY DISEASE.

Causes, signs, prevention and treatment

PID is the inflammation of female reproductive organs such as the uterus, ovaries, cervix, endometrium, and the fallopian tubes, due to the infection of s*xually transmitted bacteria. Sometimes it also caused by the normal bacteria present in the va**na. This bacterium enters via the va**na and reaches to the uterus, fallopian tube, and ovaries. If PID is not treated for long-term, then it can lead to the formation of scars with fibrous band growth between the surrounding organs and tissues.

Causes of PID

>The root causes of PID are mostly by s*xually transmitted infections such as gonorrhea and chlamydia.
>It can also be transmitted from the non-s*xually transmitted infections such as bacterial vaginosis.
>PID can also occur after the rupture of an appendix.
>In some cases ruptured bowel infections can also cause PID.
>Unprotected s*x can cause PID very easily.
>It can also result from a termination of pregnancy or childbirth.
>Having past history of PID and STD

Symptoms of PID

Many women’s doesn’t have any signs and symptoms. When the condition becomes mild to serious, symptoms may arise include:

>Pain in the abdomen is the most common symptoms
>Pain during s*x
>Irregular and long-lasting periods
>Unusual discharge with bad odor from Virgina
>Fell very tired
>Pain when you p*e or hard time to go
>Fever (temperature 38° C [100.4° F] or higher) or chills
>Fainting and vomiting
>Often urination

Risk factors of PID

>Childbirth, abortion or miscarriage can make the va**nal mouth unclosed. This makes the high possibility of bacteria to enter into the reproductive organs.
>An intrauterine device (IUD): It is a type of birth control placed into the uterus, which increases the risk of developing PID.
>An endometrial biopsy also increases the risk of PID due to some bacterial infections during the biopsy.
>Appendicitis: It slightly increases the risk because the infection from appendices is spread over the pelvic regions.
>Douching: Douching is washing or cleaning out the va**na with water or other mixtures of fluids. Frequent douching may be at risk for developing PID.
>Women who are having several s*xual partners.
>People who do not use barrier contraceptives
>Women who are under age of 25years and s*xually active.

Metronidazole (Flagyl) :
(Ronidazole may be a better choice)

Metronidazole is highly effective in the treatment of many anaerobic bacterial and protozoal infections. Oral metronidazole is the drug of choice for trichomoniasis. Trichomonal resistance to this agent has also been reported; current resistance rates are reported at 5-10%.

The mode of action of metronidazole is not well understood. The drug appears to be absorbed into cells; intermediate metabolites bind DNA and inhibit protein synthesis, causing cell death.

--------------------

Metronidazole:

Metronidazole has been used for many years in the ther**eutic management of trichomoniasis, giardiasis, and amebiasis. It is active against obligate anaerobic bacteria. It is not active against facultative anaerobes, obligate aerobes, or microaerophilic bacteria other than Campylobacter fetus. At concentrations readily attained in serum after PO or parenteral administration, metronidazole is active against Bacteroides fragilis, B melaninogenicus, Fusobacterium spp, and Clostridium perfringens and other Clostridium spp. It is generally less active against nonsporeforming, gram-positive bacilli such as Actinomyces, Propionibacterium, Bifidobacterium, and Eubacterium spp. Metronidazole is also somewhat less active against gram-positive cocci such as Peptostreptococcus and Peptococcus spp, but the less sensitive strains are usually not obligate anaerobes.

Metronidazole is bactericidal at concentrations equal to or slightly higher than the minimal inhibitory concentration. The precise mode of action is unclear, but reduction in an anaerobic environment yields a compound that then binds to DNA, causing loss of the helical structure, strand breakage, and impairment of DNA function. Only susceptible organisms (bacteria and protozoa) appear to be capable of metabolizing the drug.

The pharmacokinetic pattern of metronidazole generally follows that expected of a highly lipid-soluble basic drug. It is readily but variably absorbed from the GI tract (bioavailability 60%–100%), with serum concentrations peaking within 1–2 hr, and becomes widely distributed in all tissues. Metronidazole penetrates the blood-brain barrier and also attains ther**eutic concentrations in abscesses and in empyema fluid. It is only slightly bound to plasma proteins. Biotransformation is quite extensive, and parent drug and metabolites are excreted by both the renal and biliary routes. The elimination half-life in dogs is ~4.5 hr, and in horses, 1.5–3.3 hr.

The principal clinical indications for metronidazole include the treatment of specific protozoal infections (amebiasis, trichomoniasis, giardiasis, and balantidiasis) and anaerobic bacterial infections such as those that may be seen in abdominal abscesses, peritonitis, empyema, ge***al tract infections, periodontitis, otitis media, osteitis, arthritis, and meningitis, and in necrotic tissue. Metronidazole has been successfully used to prevent infection after colonic surgery. Nitroimidazoles also act as radiosensitizers, and metronidazole has been used as an adjunct to the radiotherapy of solid tumors.

Adverse effects are not commonly associated with metronidazole. High doses may induce signs of neurotoxicity in dogs, such as tremors, muscle spasms, ataxia, and even convulsions. Reversible bone marrow depression has been reported. The drug should not be used in pregnant animals, particularly during the first trimester, although the evidence for carcinogenicity and mutagenicity is still tenuous. Metronidazole may produce a reddish brown discoloration of the urine due to unidentified pigments.

Recommended dose rates for metronidazole in dogs are 44 mg/kg, PO, followed by 22 mg/kg, qid, for anaerobic infections; 25 mg/kg, PO, bid, for giardiasis; and 66 mg/kg/day, PO, for trichomoniasis. Courses of therapy are generally 5–7 days. Both PO and IV preparations are available.

Cold Compresses As soon as your baby develops a fever, the first thing to do is put a cool, wet washcloth on your babys forehead. As the water from the wet washcloth evaporates from the skin, it will draw the fever out and the temperature will come down quickly.
