What appears to work in controlling shortsightedness progression are
I) Increase outdoor activities. 1 hour per day for those with moderate near work. 2 hours per day for those with intense near work. Sunlight from 8am to 11am and 4pm to 7pm. Avoid sunlight around 12pm to 3pm.
II) Reduce near tasks. Minimise near tasks or rest your eyes regularly, every 20 minutes of near tasks, rest your eyes by looking at object further than 6 feet and blink at least 20x.
III) Wear specialty spectacle glasses like Miyosmart lens from Hoya Malaysia.
IV) Wear Ortho K contact lens.
V) Apply Atropine eye drop.
Outdoor activities are very important in preventing onset of shortsightedness and shortsightedness progression.

Why myopia management is essential?
Short term goal.
Reduce the frequency of prescription changes, providing better vision for kids between eye exams.
Long term goal.
Reduce the risk of eye diseases and vision impairment in adulthood. The risk in the charts below are compared to someone with no myopia.
Each -1.00 D (100 degree) increases lifetime risk of myopic maculopathy by 67%
Each prevented -1.00D reduces lifetime risk of myopic maculopathy by 40%.

Visual Environment Advice
1. Elbow rule. Keep forearm distance between eyes and book.
2. 20/20/20 rule. Every 20 minutes of near work, take a break for 20 seconds by looking at 20 feet away.
3. The 2 hour rule. Less than 2 hours screen time after school and don't sit for too long.
4. At least 2 hours a day outdoors. Be sun smart and active fir 1 hour a day.
Myopia Risk Factors.
1. Age of myopia onset. Younger age likely to progress faster.
2. Myopic parents. Myopia is hereditary.
3. Time spend outdoor. Aim for at least 2 hours a day.
4. Time spend on close work. Aim for less than 2 hours per day of leisure screen time.
5. Binocular vision disorder.
WHAT IS MYOPIA?
Myopia or short-sightedness is an eye condition that causes distant objects to appear blurry while near objects can be seen clearly.
This occurs if the eye is longer than normal or has a cornea that is too steep. Due to this, light rays focus in front of the retina instead of on the retina, causing blurred vision.
The earlier a child becomes myopia, the faster the progression and increased risk of severity.
WHAT ARE THE SYMPTOMS?
-Blurry vision when looking at distant objects
-Sitting too close to the television or squinting to see distances.
-Headaches caused by eye strains
-Sensitive to light and glare.
-Difficulty seeing while driving especially at night
WHAT CAUSES MYOPIA?
-Risk factors for the development of myopia include:
-Intense near work
-Lack of sunlight exposure
-Genetics
HOW CAN IT BE DIAGNOSED?
It is diagnosed by a basic eye exam which involves refraction.
IS MYOPIA A DISEASE?
Myopia is not a disease. It is a refractive error which means it is an optical condition that prevents the eye from bending the light properly.
ARE THERE GRADES OF MYOPIA?
Yes.
Low Myopia: From 0 to -3.00D
Moderate Myopia: From -3.00D to -6.00D
High Myopia: more than -6.00 diopters of refractive error.
WHAT IS PROGRESSIVE MYOPIA?
Progressive myopia is shortsightedness that continues to worsen year after year. In most people myopia stabilizes by late teens. If the prescription continues to increase after teen years or increases rapidly in the early teens, serious eye complications may occur.
CAN MYOPIA BE CURED?
There is no specific cure for it but corrective glasses or contact lenses work quite well in the majority of the patients. Once myopia is stabilized, usually around the age of 18-20 years, LASIK procedure may be done. For very high myopia, Implantable Collamer Lens (ICL) implant surgery is an option.
IS MYOPIA REVERSIBLE?
Established myopia is not reversible. With age, the length of the eyeball, and hence the minus eye number is likely to increase.
SHOULD ONE WEAR GLASSES ALL THE TIME FOR MYOPIA?
Depends on the grade of myopia. One may require glasses for certain activities like watching TV or driving. If someone is significantly nearsighted he/she may require glasses all the time. Get proper consultation from your optometrist.
DO MOBILE AND COMPUTER SCREENS CAUSE MYOPIA?
Intense near work of any type is associated with myopia (reading for long hours, long screen time, etc). More importantly, prolonged screen use can cause eye strain. Therefore, it is recommended to take breaks every 20 minutes to look at far objects 20 feet away and blink 20 times (20:20:20 rule). Books and devices should be held at a distance of about one elbow length. Additionally, sitting upright while reading is important as is holding reading material perpendicular to the line of sight. Holding it upright rather than lying on a table.
HOW CAN ONE PREVENT MYOPIA?
Sunlight exposure is essential to prevent abnormal elongation of the eye. Therefore, parents must encourage at least 90 minutes of outdoor activities at home and at school for growing children.
WHAT ARE THE COMPLICATIONS OF HIGH MYOPIA?
–Retinal Tears and Retinal Detachment
-Myopic macular degeneration
-Early cataracts
-Glaucoma
CAN MYOPIA CAUSE BLINDNESS?
In rare situations, if the retinal detachment is not treated in a timely fashion, blindness may result.
