A FECUNDITY STUDY

We are B.Sc. students (Excel, Phoebe, and T**i) of Computer Science Department, Federal University Lokoja lead by a Ph.D. student (Muhammad Ahmad Shehu) of Computer Science Department, Federal University of Technology Minna and an academic staff of Computer Science Department, Federal University Lokoja, that are on a research of Women fecundity determination using intelligent models, and we are currently on the data collection phase of the research. The questionnaire link given below represents the details needed to collect data (your answers to the questions) for the research.

Putting in mind the private nature of the data to be collected, the questionnaire does not include personal information that could attach the answers of the questionnaire to the individuals answering the questionnaire. Also, since the data to be collected is solely for the team researches, the team will make sure that the data collected is not submitted for public use.

The questionnaire is in two phases, where phase 1 is a one-time questionnaire that should be filled once throughout the period of the study, while phase 2 requires answering the phase questions from 1 to 12 periods (i.e 1 to 12 menstrual cycles) through a year or from 1 cycle to the cycle where pregnancy was observed. For each cycle, the questions are to be answered from 1 to 29 times (i.e. from 1 to 29 days), where at least the questions has to be answered each time/day an episode of in*******se occurs. A sample of a filled questionnaire is attached at the last page of this questionnaire. However, for any assistance needed, the below contacts can be used. Each period is to start from day 1 of the menstrual cycle (where day 1 is the first day the menstrual bleeding is observed) and end in the last day of the menstrual cycle (where the last day is the day before the next menstrual bleeding is observed).
