HOW SOON CAN ONE GET PREGNANT AFTER A MYOMECTOMY ?

I often get asked this question, “how soon after my myomectomy can we start trying to get pregnant”?

Along the way in my training and ever since, I have heard all kinds of answers from my professors and colleagues:“women need to wait a year” ; “six months”; “three months”; “two months”. None of these answers were based on scientific studies, they were just opinions.

A few years ago there was a study that helped us answer this question. The study was conducted at three hospitals in Japan, where the authors performed a pelvic MRI before and then 6, 12, 24 and 52 weeks after abdominal myomectomies.

In addition to the standard MRI, they also injected contrast solution into the women’s veins (which shows up in blood vessels) to see when blood flow in the healing uterine muscle wall was back to normal.

What they found was that all women had healing of the uterine lining area and return to a normal uterine size at around 12 weeks. Also, 12 of the 14 women (86%) had normal blood flow to the uterine muscle by 12 weeks.

Based on this study, it appears that women can start trying to get pregnant three months after myomectomy. Even for the rare patient who heals a bit more slowly, the healing should be complete by the time the uterus starts to grow with the pregnancy. WHAT IS CONSIDERED A LARGE FIBROID ?

If you have uterine fibroids, you may wonder what causes them to grow. Although the exact cause of fibroid growth is unknown, we know that fibroids are influenced by estrogen. It is often observed that fibroids go through growth spurts. Fibroids may begin to develop quickly but then slow down and speed up at different intervals. This is often seen when fibroid symptoms decrease for a few weeks and then become worse than before. Over time, some fibroids can grow large and cause additional problems. This is why it’s important not to disregard your symptoms and wait for them to improve.

THE SIZE AND LOCATION OF FIBROIDS

Fibroids can vary in size from as small as a seed to as large as a melon, and they can develop in numbers. Where a single fibroid may be less than an inch, groups and clusters of fibroids can be much larger. Depending on their size and location, fibroids can affect various parts of your body, such as expanding the uterus so that it impacts other organs or blocking the fallopian tubes, which may cause difficulty conceiving.

A large fibroid is one that is 10 cm or more in diameter. The largest fibroids can range from the size of a grapefruit to the size of a watermelon.

THE SIZES OF UTERINE FIBROIDS RANGE FROM SMALL TO LARGE:

Small Fibroids can be less than 1 cm to 5 cm, the size of a seed to a cherry.

Medium Fibroids range from 5 cm to 10 cm, the size of a plum to an orange.

Large Fibroids can be 10 cm or more, ranging from the size of a grapefruit to a watermelon.

The largest fibroid ever removed was reported to weigh over 100 lbs. You can imagine how the weight of large fibroids can put stress on the uterus and body.

Fibroids and weight gain usually go hand in hand. Fibroids are tissue. As they grow, they increase your overall body weight, just as adding any extra tissue to your body would. Larger fibroids may cause a highly visible protrusion. It’s the kind of body change no amount of exercise or dieting will reduce.

An increase in symptoms usually causes a decrease in the desire to exercise. Severe pain or pressure can make movement difficult. Excessive bleeding is just awkward and anxiety-producing.

Uncertainty about what your body will do, means you do less. Less exercise means lowered chances to burn off calories, which leads to body fat.

Fibroids also often cause fatigue from anemia caused by heavy bleeding. Less energy means activity levels drop even further. Now you are dealing with weight gain, not just from fibroids, but from restrictions to movement, fear of exercise, lack of energy.

CAN A MAN WITH PROSTATE CANCER GET A WOMAN PREGNANT ?

It is nearly impossible for a man to retain his ability to father children through sexual in*******se after initial treatment.

During prostatectomy, both the prostate and the nearby seminal vesicles are removed. Seminal vesicles and the prostate provide semen that carries the s***m down the urethra and out the p***s during ej*******on. The loss of semen following surgery makes ej*******on impossible, so the s***m cannot physically leave the body.

Radiation therapy nearly always impairs fertility. Radiated prostate cells and seminal vesicles tend to produce semen that cannot transport the s***m well. S***m can also be damaged, though this occurs far less frequently with more accurate dose planning.

POST-TREATMENT FERTILITY OPTIONS

For men who wish to father children after treatment for prostate cancer, the best chance for fertility is s***m banking. Semen containing s***m is frozen in liquid nitrogen. After thawing, up to 50% of s***m will regenerate and can be used for artificial insemination.

Extracting s***m directly from the testicles might be another option. After harvesting s***m from testicular tissue, a single microscopic s***m is injected into a single microscopic egg. If an embryo forms, it is implanted into the woman’s uterine wall and allowed to grow.

The success rates for the two procedures combined—s***m extraction followed by injection of the s***m into the egg—is less than 50%.

What is prostate surgery for?

The prostate is a gland located underneath the bladder, in front of the re**um. It plays an important role in the part of the male reproductive system that produces fluids that carry s***m.

Surgery for partial or complete removal of the prostate is called a prostatectomy. The most common causes for prostate surgery are prostate cancer and an enlarged prostate, or benign prostatic hyperplasia (BPH).

SURGERY FOR PROSTATE CANCER

Surgery is a common choice to try to cure prostate cancer if it is not thought to have spread outside the prostate gland.

The main type of surgery for prostate cancer is a radical prostatectomy. In this operation, the surgeon removes the entire prostate gland plus some of the tissue around it, including the seminal vesicles.

OPEN OR LAPAROSCOPIC RADICAL PROSTATECTOMY

In the more traditional approach to prostatectomy, called an open prostatectomy, the surgeon operates through a single long skin incision (cut) to remove the prostate and nearby tissues. This type of surgery is done less often than in the past.

In a laparoscopic prostatectomy, the surgeon makes several smaller incisions and uses special long surgical tools to remove the prostate. The surgeon either holds the tools directly, or uses a control panel to precisely move robotic arms that hold the tools. This approach to prostatectomy has become more common in recent years. If done by experienced surgeons, the laparoscopic radical prostatectomy can give results similar to the open approach.

OPEN PROSTATECTOMY

RADICAL RETROPUBIC PROSTATECTOMY

For this open operation, the surgeon makes an incision (cut) in your lower abdomen, from the belly button down to the p***c bone, as shown in the picture below. You will either be under general anesthesia (asleep) or be given spinal or epidural anesthesia (numbing the lower half of the body) along with sedation during the surgery.

If there is a reasonable chance the cancer might have spread to nearby lymph nodes (based on your PSA level, prostate biopsy results, and other factors), the surgeon may also remove some of these lymph nodes at this time (known as a pelvic lymph node dissection). The nodes are sent to the lab to see if they have cancer cells in them. If cancer cells are found in any of the nodes, the surgeon might not continue with the surgery.

THE BENEFITS AND LIMITS OF LAPAROSCOPIC SURGERY FOR UTERINE FIBROIDS

Uterine leiomyomas (i.e., fibroids, myomas) are the most common gynecologic tumors in women of reproductive age

Clearly, the majority of such lesions are asymptomatic

Symptoms directly attributable to these benign tumors represent the most common reason for laparotomy in non-pregnant women in the United States and also in Taiwan

Whereas in decades past, hysterectomy was seen almost as a panacea for uterine leiomyomas, more recently attention has been paid to the development of pharmaceutical agents and less-invasive procedures.

Frequently, such procedures are designed to retain the uterus Of these, myomectomy may be a choice among the uterine-sparing treatments for symptomatic uterine myoma

The surgical mode of access usually employed in myomectomy is traditional exploratory laparotomy or its modification-mini-laparotomy (MLT) or ultra-mini laparotomy (UMLT) , though recently, laparoscopy or a combination of laparoscopy and MLT , vaginal surgery and hysteroscopic myomectomy have represented valid alternatives. However, myomectomy alone provides varying degrees of symptom control and a high percentage of recurrence, not only for the tumors themselves, but also for the symptoms. For example, one study reported that symptom resolution varied from 84.0% to 100% depending on different items and 21 (19.4%) of 108 patients experienced a recurrence after an average interval of 16 months (range, 1.8-47.4 months) . Therefore, an alternative or additional therapy might be required to provide longer durable symptom control and minimize tumor recurrence. One of the strategies is laparoscopic uterine vessel occlusion (LUVO), also known as laparoscopic uterine artery occlusion (LUAO) .

The rationale for using LUVO in the management of symptomatic myomas is found in the successful experience with uterine-artery embolization (UAE), which was introduced in 1995 as an alternative technique for treating fibroids . Since then it has become increasingly accepted as a minimally invasive, uterine-sparing procedure, and studies have reported the relief of excessive menstrual bleeding or pressure in 80-90% of patients .

LUVO provided similar relief of symptoms 89.4% with symptomatic improvement and 21.2% with complete resolution of symptoms) in 2001 in a 7- to 12-month follow-up of 87 patients after LUVO .
