Additional information about ESD
The main advantages of the ESD procedure
include:
• Less invasive curative treatment for early GI tumors
• Faster return to regular activities anddiet
• Less postoperative pain
• No visible scars
ESD is an advanced therapeutic endoscopic
procedure. Since it mimics surgical resection, the ideal operator is someone well versed in surgical management as well as advanced endoscopic procedures.
At present, Cebu Doctors’ University Hospital is the only center in the Visayas and Mindanao capable of performing this procedure safely. The physician who performs ESD in Cebu was trained
by Prof. Haruhiro Inoue in Tokyo, Japan, and was certified after completing the one year training program.
What is ESD?
Endoscopic Submucosal Dissection (ESD) is an advanced therapeutic endoscopic procedure used to diagnose and treat early stage gastrointestinal cancers. This procedure allows complete excision of larger early stage tumors with no visible scars.
Typically, diagnosis of gastrointestinal
cancer is made by evaluating symptoms, and comparing them to an imaging study, most commonly CT scan. Patients are then subjected to an endoscopic examination, with biopsy for a tissue diagnosis. If the biopsy is confirmed as malignancy (cancer), the patient is usually advised for surgical resection.

How is laparoscopic appendectomy done?
The appendix is most commonly removed by
single incision over the right lower quadrant or
by laparoscopic (keyhole) surgery. Laparoscopic
appendectomy is done using a high definition camera, and 3-4 small openings in the abdomen.
The benefits of laparoscopic surgery include:
• Minimal post operative pain
• Faster recovery and return to work
• Better cosmetic results
• Most patients go home 1-2 days after the
operation and can return to normal activities much quicker, depending on the case.
What is the appendix?
Your appendix is a finger-like tubular organ
that protrudes from the large intestine (cecum)close to where the small and large intestine join together. The inside of the appendix is continuous with the inside of the cecum. If the opening becomes blocked, usually by hardened food waste (fecalith), this results in swelling,inflammation, and subsequent infection.
What is appendicitis?
Infection of the appendix is called appendicitis.This infection typically happens suddenly. As the appendix continues to swell, the blood supply to the appendix becomes blocked. This results in tissue necrosis, gangrene, and eventual rupture.

What is a hernia?
A hernia is a weakness or tear in the abdominal muscles that allows organs, such as intestines or fatty tissue, to protrude through the weakened area, resulting in a bulge under the skin. They are most commonly found in the groin (inguinal) area, and in males. This bulge is usually accompanied by symptoms:
• Enlarging bulge especially when straining,
lifting heavy objects or standing long
periods.
• Inguinal pain
• Discomfort or aching sensation
• Vague feeling of fullness
• Nausea and constipation...
How is the ERCP performed?
In our institution, the procedure is typically carried out under sedation in the radiology suite.
A flexible tube with a camera at the tip (endoscope) will be inserted through your mouth and down
your throat until it reaches the small intestine. A thin catheter is then inserted through the scope
channel and into the bile duct or pancreatic duct,and a small amount of contrast solution is given
as an x-ray is taken to visualize the duct anatomy, and to look for any causes of obstruction.
What interventions can be done with ERCP?
If a blockage is found, your doctor may be able to treat the problem during the procedure.
Sphincterotomy involves making a small cut to enlarge the opening of the bile duct to allow
extraction of stones. This also improves drainage of bile. Removed stones are dropped into the
intestine and are excreted with your waste.Stenting is the insertion of a tube in a blocked or
narrowed duct to improve drainage. This stentmay be permanent (metal) or temporary (plastic).
Endoscopic Retrograde Cholangio-
Pancreatography or ERCP is a special
endoscopic and x-ray procedure used to study the bile ducts, pancreatic duct, and gallbladder.Bile is a yellow green fluid produced by the liver, and stored and concentrated in the gallbladder. It’s function is to aid the digestion of fats or lipids in the small intestine. The bile ducts are channels that drain bile from the liver and gallbladder. The pancreatic ducts drain the pancreas. Both ducts empty into the duodenum, the first part of the small intestine.
