Ang colon cancer ay isang uri ng cancer na nagbabago sa tissue ng colon at/o re**um. Madalas itong nagsisimula bilang isang maliit na growth o (mga) bukol na tinatawag na polyp. Ang mga polyp ay maaaring lumaki hanggang maging kanser. Kapag nakita ang colon polyps during a colonoscopy, maari silang tanggalin, bago sila maging cancer.

Dapat na magsimula ng screening thru colonoscopy or at least stool test para sa colon cancer simula sa edad na 45-50 yrs old. Along mahalaga ito sa mga mayroong kapamilya na nagkaroon ng colon cancer.

Para sa mga masHigh risk- maaring mas maaga ipagawa ng inyong duktor ang colonoscopy . Kadalasan ang colonoscopy ay ginagawa as outpatient under IV sedation ( habang tulog). Magagamit ang PhilHealth Benefits for Colonoscopy kahit outpatient.

I was fortunate this week to be asked to give a talk to senior HS students on what a future career in Health in the Philippines might mean for them. I truly thought I would sit in a classroom of under 50 students and then answer questions on the path it takes to become a doctor. It sounded like a cinch so I said yes easily.

Lo and behold the classroom of 50 was actually I think 600plus students live and some others online in other campuses :0 Anyway, God really is wise because I ended up being the learner in the process of making this talk.

Some important stuff that stuck to me:
1. The Philippines needs more than 44.5 skilled healthcare workers per 10,000 population for UHC. We have the illusion of meeting this but see succeeding numbers.

2. We need 14.3 doctors per 10,000 (in 2019 we were only at 8.1:10,000 with some regions having only 1 doctor per 10,000)
- we needed to have produced 65,000 physicians by 2025 to meet this need.

3. The target nurse to population ratio is 27.4:10,000 and we have a surplus at 52:10,000, in NCR the ratio is 78:10,000 -- BUT see number 4 and 5.

4. In 1990-2017 , 350,361 health care workers- 95% of them nurses left this country to be OFWs in other countries because we provide them horrible pay and working conditions . 84 % of them are temporary migrants - meaning they have likely left families here back home to work hard for.

5. Many nurses choose to work nonclinical BPO positions because of unjust working conditions and pay

6. We need more dentists/ OTs/ PTs/ Rad techs/Nutritionists (considered low density HCPs).

The list of what I learned can go on and on and on. So I am grateful to Mrs. Ruth Salamat . I am grateful to have met the other panelists including Atty Kiko Pangilinan . It was sobering and out of my comfort zone. But some things made me feel better/hopeful.

1. The youth in that conference gave me hope. Maybe they will be better than us. Maybe they will vote better than us.They asked surprising and even thought provoking questions. I pray they find their paths to help our battered country.

2. The introspection made me remember to thank God, my husband and my parents for helping me get to where I am today. It took 15 years- someone needed to foot the bill (!). But because of them I am in a job/profession that the world needs, that allows me to help others (though it may be in the worst times of their lives) and that I can continuously work hard to be better at. I can't imagine being anything else.

For Liver Cancer Prevention and Awareness Month, here are some tips for the workup and diagnosis of primary liver masses/tumors:

1. Masmabuti na gawin ang inyong imaging tests (CT or MRI) and blood tests sa institution/center na may kasanayan sa dedicated liver imaging. This could save precious time (and money). It may save lives too .

2. For Liver masses/ para sa mga bukol sa atay - may special dynamic scans po na kailangan (Triphasic/4 phase CT or MRI/ liver specific contrast MRI). Kulang po ang conventional imaging upang magamit sa diagnosis, staging and treatment planning.

3. Always get the digital images from radiology for your doctors to review and use for evaluation and treatment planning (DVD/USB files po ito and not just films or paper reports).

4. May mga pagkakataon na ang Liver Cancer ay kaya na ma-diagnose kahit walang Liver Biopsy.

*hanggat kaya po- early diagnosis and urgent multidisciplinary evaluation and treatment with specialists (when possible and available) is best for Liver cancer.

Talk to your doctor and ask questions. Prevention and Early detection is best!

Simple things/tests to check your liver health this January

1. Include AST/ ALT in blood tests (Together with CBC platelet count, your doctor can compute FIB 4 or APRI score- which screens for Liver Cirrhosis)

2. Liver Ultrasound- ask for further evaluation if fatty liver is found

3. Hepatitis screening- HBsAg, AntiHCV test

4. Want to know if you need a Hepatitis Vaccine?
Test you antiHBs titer and AntiHAV IgG

Ask your doctor to check your liver :)
