There are several ways to lengthen and or augment the nose, rotate the tip or say project the nasal tip. One way is the use of nasal implant which nowadays is the most commonly utilized. Another way is to use the patient’s/ person’s own cartilage such as septal cartilage, ear cartilage and or rib cartilage. There are advantages and disadvantages associated with each technique. Patient came in desirous of nasal augmentation due to relatively low bridge, short nose and wide ala. She underwent augmentation rhinoplasty using ePTFE implant, tip plasty using ear cartilage, alarplasty. ✨

Patient concerns include bulbous tip and relatively low dorsum. She underwent augmentation/structural rhinoplasty using ePTFE(gore-tex), alarplasty, tip plasty , collumelar strut graft using ear cartilage. At 1 month, patient is very happy with the outcome of the surgery. *Patient has thin skin.

“Easier said than done.”Gynecomastia surgery involves surgical excision and/or liposuction depending on the grade or severity upon presentation. Common complaints of my patients include prominent breast while wearing a shirt , ashamed of being topless while in the beach or swimming pool and relatively low self-esteem. Gynecomastia is enlargement of the male breast. It is caused by increase in ductal tissue, stroma and/or fat. The most common cause is unknown(idiopathic).

Other common causes are obesity, physiologic, endocrine, neoplasms/cancers, systemic diseases and drug induced.

Diagnosis is made thru careful history and physical examination(PE). Laboratory evaluation depends on the findings of history and PE.

In patients with gynecomastia for more than 1 year and with normal history and PE, surgery may be indicated.

Surgical procedures include liposuction and surgical excision depending on the stage or grade.

Happy patient 🙂 She was concerned of dark circles around her eyes, alar flaring, wide alar base and relatively low bridge.She requested to look younger and enhanced but of course she wanted it to be natural looking. She underwent augmentation rhinoplasty, tip plasty, alar base reduction, upper and lower blepharoplasty and chin augmentation.

Diplomates of PAPRAS

Patient major concerns include wide and uneven ala, bulbous tip, short collumela and relatively low bridge. The following procedures were performed namely Collumer strut graft, tip plasty using ear cartilage, alar trim, alar base reduction, ePTFE(goretex) augmentation rhinoplasty via closed technique. Goals of the surgery achieved. Patient is happy and satisfied with the surgical outcome. Currently at 3 months post op.

Gynecomastia is a condition associated with enlarged breast tissue in males. If it persists beyond 12 months, surgical options may be considered such as breast tissue excision with or without liposuction or skin excision.

Augmentation rhinoplasty using ePTFE(goretex), tip plasty and collumelar strut graft using ear cartilage. Tip and bridge definition appears more defined as time goes by. Always remember that a patient with relatively thick skin may have delayed results in terms of tip definition. It may take 6 months up to 1 year.

Natural looking female rhinoplasty. This technique involves strong collumelar and tip support. Pictures A. before surgery B. 1 week after surgery C. 2months after surgery. Patient is happy and satisfied with the outcome of the surgery. Soft tissue swelling including external and internal may take 6-12 months to resolve and external nasal definition will continue to improve during this period.

Augmentation rhinoplasty via closed technique, alar base reduction. Pictures are A. Before and B. 7days after surgery. Nasal tip swelling will improve in the next 3-6months. Patients with relatively thick skin tend to have longer healing time and late appreciation of tip definition. Lateral side profile assumes its final shape earlier than the frontal view(Facefront); nose narrows at frontal view(front) for at least 12-18 months. #
