STROKE VS ANEURYSM
A stroke occurs when there’s a ruptured blood vessel in the brain or blood supply to the brain has been blocked.
Symptoms of stroke may include:
Sudden, severe headache
Numbness on one side of body
Weakness in the arms or legs
Difficulty with balance
Vision problems
Confusion
Dizziness
Difficulty with speech
An aneurysm is the result of a weakened artery wall.
Symptoms of aneurysm can include:
Sudden, incapacitating headache
Numbness or weakness in one or both limbs
Memory issues
Blurred or double vision
A drooping eyelid
Seizure
Stiff neck
Nausea
Vomiting
Both have similar risk factors, including:
Age
Uncontrolled high blood pressure
Smoking
Personal or family history of stroke or aneurysm

Hypertension is defined as blood pressure over 130 mmHg systolic and 80 mmHg diastolic.
Based on the cause, hypertension can be primary, or essential hypertension, which occurs without a known cause, and secondary hypertension.
Risk factors for primary hypertension include age, obesity, excess salt or alcohol consumption, physical inactivity, diabetes, and smoking.
It is important to diagnose and treat hypertension early because chronic hypertension can lead to atherosclerosis, heart failure, brain aneurysm, renal failure, hypertensive retinopathy, and many more conditions.
Okay, the first step to diagnose hypertension is measuring blood pressure. So, the BP has to be greater than 130 over 80 on at least 2 separate readings on 2 separate visits.
Now, before primary hypertension is diagnosed, causes of secondary hypertension, like hyperthyroidism, adrenal gland disorders, or the use of estrogen-containing oral contraceptives, must be ruled out.
When it comes to treatment, the first choice is a lifestyle change which includes a low sodium diet, exercise, and stress reduction.
Medications used to treat hypertension include ACE inhibitors, angiotensin receptor blockers, thiazide diuretics, and long-acting calcium channel blockers.

👉The glenoid labrum is a fibrocartilaginous structure that plays a significant role in glenohumeral joint stability One form of injury to this structure is the superior labrum anterior to posterior (SLAP) tear.
👉 The purpose of this study by Dean et al. (2022, https://pubmed.ncbi.nlm.nih.gov/35997579/) was to conduct a systematic review on the performance of physical examination maneuvers in diagnosing SLAP lesions.
👉 One method to further assess physical examination maneuvers for a SLAP tear, is to evaluate them in parallel and series with each other. This process allows multiple tests to be considered in combination to evaluate their overall sensitivity and specificity. Two or more tests are applied in parallel testing, and if any single test is positive, then the overall grouping is considered positive for a given diagnosis. Parallel testing decreases the number of false-negative results and increases false-positive results.8 Ultimately, this results in increased sensitivity and negative predictive value but decreased specificity and positive predictive value (PPV).
👉 In series testing, the tests are applied sequentially and all must be positive for a diagnosis to be made. Series testing on the other hand decreases false-positive results and increases specificity and PPV.

📍 The crank, anterior glide maneuver, or Jobe tests are not included in the figure. The crank test is performed with the patient either sitting upright or lying supine, the arm flexed to 90°, and the examiner standing adjacent to the injured shoulder holding the elbow in a flexed position. An axial load is applied through the humerus with one hand while the other hand performs humeral rotation relative to the glenoid while the shoulder is elevated in the scapular plane.
📍 The anterior glide test is performed with the patient in the seated position; the examiner puts one hand on the patient’s shoulder, proximal to the glenohumeral joint, to stabilize the scapula, acromion, and clavicle. Grasping either the elbow or the proximal humerus, a superior and anterior force is used in line with the humerus while stabilizing the proximal aspect of the shoulder.
📍 The Jobe test is performed, with the patient’s shoulder abducted to 90° and the elbow flexed to 90° , and the examiner holding the patient’s wrist and hand in one hand and the otherhand placed on the patient’s humeral head. The examiner applies a posterior force to the humeral head and externally rotates the humerus.
A headache is a very common disorder, which all of us probably experienced at some point.
Now, headaches can be primary, which includes tension, migraines, and cluster headaches; or secondary, which are caused by another underlying condition, like a head injury, or a brain tumor.
Tension headaches are the most common type. They typically present as a progressive, bilateral, band-like headache, lasting anywhere from 30 minutes to a week.
When it comes to migraine headaches, they can have triggers like some food, weather, bright lights, or lack of sleep. They present with a severe, unilateral, pulsating pain, that worsens with movement. Also, an individual might experience an aura, which consists of visual symptoms like seeing bright lights, or neurologic symptoms such as aphasia, tinnitus, or confusion. Migraines can last between 4 and 72 hours.
Cluster headaches are the rarest of all primary headaches. They usually present as a severe stabbing pain behind one eye. Cluster headaches can last between 15 minutes and 3 hours, occurring every day for about 8 to 10 weeks per year.
Treatment for headaches is different for each type. Acute tension headaches are usually treated with NSAIDs. Amitriptyline can be used if they are chronic. Abortive treatment for migraine headaches is based on NSAIDs, while sumatriptan can be used for more severe cases. Additionally, preventive treatment for migraine headaches includes lifestyle changes, propranolol, or amitriptyline. Finally, cluster headaches are treated with 100% oxygen and sumatriptan. As a preventive treatment, verapamil, valproic acid, or lithium are effective.

TISSUE HEALING
One of the most common issues I encounter when helping people recover from an injury or from surgery is of the healing times of various tissues. Everyone wants to be better now, but this isn’t realistic. Tissue healing takes time. We can’t rush it. And it’s important to understand that tissues heal at different rates depending on their physical makeup and blood supply, among other things. Additional contributing factors, such as diabetes, age, poor nutrition, inadequate rest, etc can further delay the healing process.
This chart shows the average range of healing times for various tissues in the body, including different grades of tissue injury for muscles, tendons, and ligaments. It can serve not only as a guide but also a reminder that we can’t force our bodies to heal faster than physiologically possible. What we can do is optimize the healing process by rehabbing appropriately, eating high quality food, getting adequate rest, and staying patient and positive throughout the journey.
