Santos-Matibag, the University President, was awarded a Plaque of Merit for her steadfast efforts in her advocacy to provide accessible healthcare and education that significantly contributed to changing the landscape of the implementation of the Safekeeping and Development Goals of the Bureau of Jail Management and Penology Regional Office III (BJMPRO-III).
The award was given on August 8 during the 33rd Anniversary Celebration of BJMPRO-III by JCSUPT PAULINO H MORENO JR, the Regional Director of the Jail Bureau.

TO ALL 3rd YEAR STUDENTS OF BACHELOR OF SCIENCE IN FISHERIES, please be informed that your Physical/Medical Examination in our campus clinic is rescheduled on July 1 to 5, 2024.

PRE-ENROLLMENT PHYSICAL/MEDICAL EXAMINATION SCHEDULE AND GUIDELINES:
STEP 1. GO TO BPSU AFFILIATED LABORATORY/DIAGNOSTIC CLINIC. All BPSU students must undergo a series of laboratory test in one of the branches/clinic of Batol- Canlas Diagnostic Center (BCDC) and MedNet Laboratory and Diagnostic Center. The tests required are chest x-ray, Complete blood count with platelet count, urinalysis, and Hepatitis B surface antigen (HBsAg).
**Students must cover the cost of the laboratory fees and pay directly at Batol- Canlas Diagnostic Center and MedNet Laboratory and Diagnostic Center.**
SCHEDULE:
๐ฃ LABORATORY TESTS at Batol-Canlas Diagnostic Center and MedNet Laboratory and Diagnostic Center start on JUNE 24, 2024.
๐ฃPHYSICAL/MEDICAL EXAMINATION AND ISSUANCE OF COMPLIANCE SLIP (at campus clinic) is on July 1 to 5, 2024.

and by scanning the QR Code below. You can also get a copy from your campus nurses. Paste your 2x2 picture to the right upper corner of the FILLED- OUT Physical/Medical Examination Record Form, attached with your laboratory results.
STEP 3. PROCEED TO PHYSICAL/MEDICAL EXAMINATION ON OUR CAMPUS CLINIC.
*If a medical condition requiring treatment such as pulmonary tuberculosis (PTB) is identified during the Physical/Medical Examination, prompt intervention is necessary, and a specialist's certification of fitness to enroll is required.
*If certain medical conditions, such as heart disease, asthma, thyroid mass, severe scoliosis, etc. are detected during the examination, they must be referred to a specialist. Certification of eligibility to enroll from the specialist is required in these cases.
STEP 4. SECURE MEDICAL COMPLIANCE SLIP
Once all the necessary steps have been completed and medical examination cleared by University physician, a COMPLIANCE SLIP will be issued by the campus nurses on duty.
STEP 5. PROCEED TO REGISTRAR'S OFFICE.

1. Our campus clinic is open from Monday to Friday, except Holidays and work suspensions, 8:00 AM to 5:00 PM.
2. To ensure the smooth flow of Medical Examination, ADHERE TO YOUR PROGRAM AND YEAR LEVEL SCHEDULE. YOU WILL NOT BE PRIORITIZED BEYOND THE SPECIFIED SCHEDULE.

๐ฃPHYSICAL/MEDICAL EXAMINATION AND ISSUANCE OF COMPLIANCE SLIP (at campus clinic) START ON JUNE 18, 2024.
STEP 2. FILL- OUT THE PHYSICAL/MEDICAL EXAMINATION RECORD FORM, that can be downloaded at https://drive.google.com/file/d/194hTaZVNAnsQVyhHe5-iKt6w_UDa_LfK/view?usp=sharing
and by scanning the QR Code below. You can also get a copy from your campus nurses. Paste your 2x2 picture to the right upper corner of the FILLED- OUT Physical/Medical Examination Record Form, attached with your laboratory results. Bring CLEAN, long brown envelope and do not write anything on it.
STEP 3. PROCEED TO PHYSICAL/MEDICAL EXAMINATION ON OUR CAMPUS CLINIC.
*If a medical condition requiring treatment such as pulmonary tuberculosis (PTB) is identified during the Physical/Medical Examination, prompt intervention is necessary, and a specialist's certification of fitness to enroll is required.
*If certain medical conditions, such as heart disease, asthma, thyroid mass, severe scoliosis, etc. are detected during the examination, they must be referred to a specialist. Certification of eligibility to enroll from the specialist is required in these cases.
STEP 4. SECURE MEDICAL COMPLIANCE SLIP
Once all the necessary steps have been completed and medical examination cleared by University physician, a COMPLIANCE SLIP will be issued by the campus nurses on duty.
STEP 5. PROCEED TO REGISTRAR'S OFFICE.
๐IMPORTANT REMINDERS:
1. Our campus clinic is open from Monday to Friday, except Holidays and work suspensions, 8:00 AM to 5:00 PM.
2. Physical/Medical examination will be on a first come, first served basis.
3. No laboratory results; No Physical/Medical examination.
4. We implement a NO FACE MASK, NO ENTRY policy in our campus clinic.
5. Incomplete requirements will not be accommodated.
6. Bring your own ballpoint pen.
