PROCEDURE FOR ADDING AND CHANGING OF SUBJECTS
MID-TERM 2023
*NOTE: Students who are requesting overload or concurrent enrollment, please consult your RIC if you are qualified to carry overload or enroll subjects concurrently BEFORE proceeding with the processing of the request for adding/changing subjects.
1. The student fills out a Google Form (see link below) to request a copy of the Adding and Changing Form.
2. The Records in-charge (RIC) will provide the Adding and Changing Form to the student via email. The student must print the Adding and Changing Form.
3. The student fills out the form with his/her complete information. Information on the schedule of the subjects may be obtained from the OAd website (https://drive.google.com/file/d/132Nd3ZqO4TND1HJ4VtnFyjcSVSodJTxD/view?usp=sharing). *Note: you may coordinate with the respective department of subject offered for the name of faculty.
4. The student will ask the faculty for the approval of enrollment in his/her class. Then, the student may proceed to the registration adviser and college dean for approval of adding and changing form.
5. After the form was signed, the student may proceed to the Accounting Office for the assessment of fees and must pay the assessed fees at the Cashier’s Office.
6. The student will return to the OAd and submits his/her Adding and Changing Form with the official receipt to the assigned OAd personnel.
7. The RIC will evaluate the form and give it to the OAd IT staff for encoding.
8. After the adding/changing of subjects has been encoded, the updated registration form/Form-6 will be available on the student portal.

PROCEDURE FOR ENROLLMENT OF IRREGULAR STUDENTS
MID-TERM 2023
1. The student fills out the Google Form (see links below) to request a copy of the Preliminary Registration and Trial Form (PRTF).
2. The Records in-charge (RIC), who shall also act as the student’s registration adviser, will provide the PRTF to the irregular student via email. The student must print his/her own PRTF.
3. The student fills out the PRTF with his/her complete information. Then, go to the department/college for the signature of each subject's faculty.
4. After completing the PRTF, the student must submit the scanned copy of the signed PRTF on this Google Form (see link below).
5. The RIC will evaluate the student's PRTF and give it to the OAD IT staff for encoding.
6. After the student's PRTF has been encoded, the registration form/Form-6 will be available on the student portal.

Procedure for Adding and Changing of Subjects
2nd Semester SY 2022-2023
Attention students requesting overload or concurrent enrollment!
Please consult your RIC to know if you are qualified to carry overload or enroll subjects concurrently BEFORE proceeding with the processing of request for adding/changing of subjects.
Download the form for Adding and Changing of Subjects (ACA.OAD.YYY.F.010) from the website of the Office of Admissions: (https://oad.clsu2.edu.ph/procedure-for-changing-and-adding-of-subjects/)
Fill out the form. Look for the schedule of the subject/s you wish to add from the OAd website (https://oad.clsu2.edu.ph/procedure-for-changing-and-adding-of-subjects/). Make sure that there are no conflicts in the schedules of your subjects.
Have the concerned instructor approve your enrollment in his/her class.
Have the form signed by the other officials, as required.
Go to the Accounting Office for the assessment of fees.
Pay the fees at the Cashier’s Office.
Submit the form to the Office of Admissions for encoding.
After 48h, download your updated Form 6.
