We are open Mondays, Tuesday, Wednesday, Friday and Saturday from 10:00 AM to 1:00 pm
• Our online medical service provides professional medical advice which may include drug prescription with instructions for proper dosage, laboratory requests, appropriate first aid and general healthcare information.
• The medical advice given by the doctor during the online consultation are for non-emergency cases and will be based on the information and/or images shared by the patient. It is not a substitute for cases that need actual physical assessment or tests.
• Note that every online consultation is only for one patient at a time.
• The guardian for minors shall assume full responsibility for the information they provide to the doctor during the online consultation.
• If you believe that your child need immediate medical attention, please go to the nearest medical emergency facility. If prescription is needed, payment is required before prescription is released Payment is on a case to case basis

INFORMED CONSENT FOR TELECONSULTATION SERVICES

Introduction
Teleconsultation involves the use of electronic communications to enable health care providers at different locations to share individual patient medical information for the purpose of improving patient care. Providers may include primary care practitioners, specialists, and/or subspecialists. The information may be used for diagnosis, therapy, follow-up, education, certification and/or accreditation and may include any of the following:
- Patient medical records
- Medical images
- Live two-way audio and video
- Output data from medical devices and sound and video
files
- Other similar medical information captured in whatever
media
Electronic systems used will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption.

Expected Benefits:
1. Improved access to medical care by enabling a patient to remain at any location while the physician obtains test results and consults from healthcare practitioners at distant/other sites.
2. More efficient medical evaluation and management.
3. Obtaining expertise of a distant specialist.

Possible Risks:
As with any medical procedure, there are potential risks associated with the use of teleconsultation. These risks include, but may not be limited to:
1. In rare cases, information transmitted may not be sufficient (e.g. poor resolution of images) to allow for appropriate medical decision making by the physician and consultant(s);
2. Delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment;
3. In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information;
4. In rare cases, a lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors;

By agreeing to this form, I understand the following:
1. I understand that the laws that protect data privacy and the confidentiality of medical information also apply to tele-consultation, and that no information obtained in the use of teleconsultation which identifies me will be disclosed to researchers or other entities without my consent.
2. I understand that I have the right to withhold or withdraw my consent to the use of teleconsultation in the course of my care at any time, without affecting my right to future care or treatment.
3. I understand that I have the right to inspect all information obtained and recorded in the course of a tele-consultation interaction, and may receive copies of this information for a reasonable fee.
4. I understand that a variety of alternative methods of medical care may be available to me, and that I may choose one or more of these at any time. My physician has explained the alternatives to my satisfaction.
5. I understand that teleconsultation may involve electronic communication of my personal medical information to other medical practitioners who may be located in other areas for purposes of my treatment.
6. I understand that it is my duty to inform my physician of electronic interactions regarding my care that I may have with other healthcare providers.
7. I understand that I may expect the anticipated benefits from the use of teleconsultation in my care, but that no results can be guaranteed or assured.
8. I understand that I am not allowed to record any conversation with my Physician as per Sections 1 and 4 of the Anti-Wiretapping Act of the RA 4200 that states that it shall be unlawful for any person, not being authorized by all the parties to any private communication or spoken word, to tap any wire or cable, or by using any other device or arrangement, to secretly overhear, intercept, or record such communication or spoken word by using any recording devices.

Patient Consent To The Use of Teleconsultation
I have read and understand the information provided above regarding teleconsultation, have discussed it with my physician or such assistants as may be designated, and all of my questions have been answered to my satisfaction. I hereby give my informed consent for the use of teleconsultation in my medical care.
I hereby authorize Sandra S. Litao, MD to use teleconsultation in the course of my diagnosis and treatment.
I fully understand that the Physician concerned or his/her staff is absolved from all obligations arising from misrepresentation, untruthful or incorrect information
provided by me.
I am fully aware that neither DLSMHSI nor DLSUMC together with its officers can be held liable for any misconduct, misgivings or negligence in the course of the tele consultation.
I am fully aware that resorting to this set up is a private engagement between me and the Physician concerned.

Republic Act No. 11332
Mandatory Reporting of Notifiable Diseases and Health Events of Public Health Concern Act requires the patient to provide TRUTHFUL INFORMATION about one’s health condition and possible exposure. Violation of this act shall be penalized with a fine of not less than Twenty thousand pesos (PhP20,000.00) but not more than Fifty thousand pesos (PhP50,000.00) or imprisonment of not less than one (1) month but not more than six (6) months, or both such as fine or imprisonment, at the discretion of the proper court.
(Ang batas na ito ay nagtatalaga na ang mga pasyente ay kailangang mag deklara ng totoo at tamang impormasyon tungkol sa kanilang kalusugan at posibleng pagkakalantad sa sakit. Ang paglabag dito ay may sapat ng kaparusahan na pagbabayad ng hindi bababa sa Dalawampung Libong Piso at hindi hihigit sa Limampung Libong Piso o pagkakakulong na hindi bababa sa isang buwan at hindi hihigit sa anim na buwan.
