The akinosi technique also known as closed mouth technique is typically used when administering local anesthesia for dental procedures involving mandibular teeth. It involves injecting local anesthesia solution into pterygomandibular space near the mandibular nerve without the need to fully open the mouth .This technique is particularly useful when patient has limited mouth opening or the procedure require the patient to mouth close.
GOW-GATE BLOCK-GENERAL METHODOLOGY
Steps:
* The mouth is opened as wide as possible
* Insert the needle high into the mucosa at the level of the 2nd maxillary molar just distal to the mesiolingual cusp
* Use the intertragic notch as an extraoral landmark to help reach the neck of the mandibular condyle
* Advance the needle in a plane from the corner of the mouth to the intertragic notch from the contralateral premolars (this position varies in accordance with individual flare of the mandible) until it contacts the condylar neck.
* Withdraw the needle slightly and perform aspiration to observe whether the needle is in a blood vessel
* After a negative result on aspiration, slowly inject the anesthetic
* Have the patient keep the mouth open for a few minutes after injection, to allow the anesthetic to diffuse around the nerves.
Compensating curves are anatomical curves that occur naturally in the teeth to facilitate balanced occlusion during jaw movements ¹. There are two main types of compensating curves ¹:
- Anteroposterior compensating curve: The most common example is the Curve of Spee, which ensures contact between posterior teeth during forward jaw movement.
- Mediolateral compensating curve: This includes the Curve of Monson, Curve of Wilson, and Curve of Pleasure/Anti-Monson, which help achieve lateral balance during jaw movements.
The superior alveolar nerve block is a dental anesthesia technique used to numb the upper teeth and surrounding tissues. Indications include:
- Dental procedures on the upper teeth, such as fillings, extractions, or crowns
- Pain relief for conditions like toothache or abscess
- Surgical procedures in the upper jaw
Technique:
1. Locate the superior alveolar nerve location (approximately 1 cm above the gum margin, opposite the first molar)
2. Use a topical anesthetic to numb the area (optional)
3. Insert the needle at a 45-degree angle, aiming towards the nerve
4. Advance the needle 1-2 cm until bone contact is made
5. Aspirate to ensure no blood vessel pe*******on
6. Inject 1-2 ml of anesthetic solution (e.g., lidocaine or articaine)
7. Wait 5-10 minutes for the anesthesia to take effect
Note: This is a basic overview, and the technique may vary depending on individual patient needs and the dentist's professional judgment.
Many dentists find the Dr. Grant Chyz () technique easier for making a Ribbond composite restoration.
The Dr. Chyz technique does not use a flowable composite on the tooth. Dr. Chyz presses the wetted Ribbond into an uncured pad of a viscous filled composite outside of the mouth and then lifts that Ribbond-composite piece off of the mixing pad so that there is approximately 0.5mm of the filled viscous composite underneath the Ribbond. The Ribbond-composite piece is then placed onto the cavity surfaces with the composite side of this piece being placed against the tooth.
Press the Ribbond through the composite so that the Ribbond is closely against the tooth. If necessary, add additional pieces of Ribbond into the cavity to cover as much of the internal surfaces of the tooth as possible. Light-cure after all of the Ribbond pieces are placed.
