Supplies and failures in MBBS Years
(Breaking the myth )

If you go to any batch and ask if they had any supply in the medical college then about half of the students would raise their hands . In almost every class one third of the students do have supplies in a year .Most of them are interestingly in the viva exam than written (because our students don’t know how to prepare and give viva ).
I would just discuss here the impact of them mainly than the reasons.
Let us divide into two categories
➡️MBBS Year 1-4 no direct impact on career at any stage especially abroad
➡️MBBS Year 5 (Final year ) Direct impact as you would not be able to start your house job with your rest of the batch (class ) and hence you will be 3-6 months behind your class .You would also not be able to appear in FCPS1 and JCAT etc with them .

🔴Advice : Try your best to avoid a supply in final year

Impact Regarding Job Application :
➡️In Pakistan : In public service commission as as well as some institutes like SKMH you can have a set back due to supply in any year
In rest of the jobs especially residency applications there is NO impact at all.

➡️Abroad UK ,Ireland, US , Gulf etc :
No impact at all . They don’t bother how many attempts you took. You can be matched in US even if you were detained .
Also for UK there is no any discrimination for training or non-training jobs on basis of this .
In the Gulf as well no one ever asks.

🔴Finally try not to fail your exams as they drain you when you have to study same subject again however do remember it is not going to impact your future so don’t worry if you have received one or multiple supplies as long as you clear your final exam in time .

Give one hour to yourself

We at all stages of life don’t give time to ourselves .Your body and mind needs some attention of you. Most importantly your life needs your attention.
Tell me last time you adopted any habit to improve your physical health ?
Tell me what you do in routine for your peace of mind and to train your brain to increase it capacity ?
Think when last time you actually took a step against the wishes of all to move ahead in your career ?
That may sound too ordinary but if you ask from people they don’t consider these things.
Take care of wear and tear of yourself and mend it time and again .This will actually improve your efficacy .Maintain strong career base and be as much independent as you can and as early as possible .
Please give a small portion of everyday to yourself.Focus on your physical health ,mental health and career in that hour .
Don’t live others life .You are already giving 23 hours to them .Give 1 hour to yourself !!!

“Always follow the signs of nature for your career.”

Remember the average IQ of general population is between 90-100 while IQ of doctors and other professionals is around 130. The average people think as per their brain 🧠 capacity.

💭Now let us apply this concept on ourselves .Our brain has a limited capacity and even the brightest and sharpest of us have a certain boundary of wisdom .So we think of options and solutions accordingly‼️
➡️Let’s go another step and think of someone who has IQ thousands times more than us…what solutions HE can find for us ??? Exactly I am talking about our creator ,”Allah”.
So even when you believe what is better for you please still look towards Allah to give you as HE believes best .
For the last few years I try to read and follow the direction of the nature with full trust that it will take me to best pathway and road .I have developed habit of correlating and trying to find the reason of every single thing .
It’s the same concept that is written in the book Al-chemist 📚Follow the signs of nature !!! Most importantly Trust in them .At end you would be where you should be and that definitely will be the best for you .✅

Journey To MRCP UK 🇬🇧
Episode 1:
Do MRCP UK when you are fully determined to complete it otherwise you may clear your written exams but will stuck at PACES.

➡️For fee and exam dates visit official site of MRCP UK .
All parts of the MRCP UK takes place in most of the countries including Pakistan ,India ,UAE so you don’t need to go abroad for any part of it .✅
➡️Eligibility Criteria:
a) PMQ I.e. MBBS Degree
b) You need house job I.e. 12 months experience before appearing in MRCP1 and 6 months more experience in any clinical setting to appear in next parts of MRCP including PACES.

🚫PLAB or IELTS/OET is not required at any part of MRCP.

➡️Preparation Time ⏰:
MRCP 1 4-6 months
MRCP 2 4-6 months
PACES 3 months

➡️Study Resources :

📕PASSMEDICINE FOR MRCP1 :
This 6 volumes series is the Bible for MRCP-1 . It covers almost each and every topic you are going to be assessed on exam day. You can skip the MCQs and move directly to explanation .Read explanations of each mcq as many times as u can from this book. By many times i mean 3 good revisions .Every word of this book should be on your tips.First of all you are going to do it twice before moving to some other book.
💻I strongly recommend to buy the online subscription of PassMedicine which has a separate text book portion as well as small videos to read text and make concepts alongwith MCQs to practice .

📙PASTEST SERIES FOR MRCP1 : This book is added in my guidelines as the second most important book after seeing the increased failure rate in MRCP-1 exams in recent years. This book has extensive knowledge and should be done twice before sitting in exam.
It is difficult book and recommended for those who are not in training especially. They should do it as well alongwith Passmedicine series .

📗ONEXAMINATION SERIES FOR MRCP1 :
The mcqs and explanations in this book are very close to original exam and will be a very good addition to your knowing about different disease and bedside situations. This book shall help you in deciding the right option in exam.
This is the third book in my preference list and you can skip it if you like .
🚫Text Books : There is no need to study any big text book i.e. Davidson etc.

🚫OTHER OPTIONS : People will recommend you many books i.e. Philip Kalra.
Ester series for basic
Oxford clinical book for medicine
No Need to do any of them.

➡️STRATEGY 💡:
Your strategy should be to study targeted material only but with very strong grip on it. No need to read multiple books.Read only few and necessary ones but do them multiple times so that every single information and concepts should be very clear and on your tips.‼️
⚠️Don’t solve the MCQS books . Read the Explanations in them.

R u guys happy wd the decision made by PMDC?? 😌
We have been struggling for ur right since 2021 nd finally grabbed it.🤪
Any suggestions??

05/02/2023

The only positive thing that happened in 2022 in Pakistan was that around 800,000 professionals secured jobs abroad .
This is being portrayed ina negative fashion however I strongly encourage this trend and would totally support this .Same I have been suggesting for the last few years .
Many people say you are leaving your country in the bad time .It’s most important to see if there is really a role you can play in your country which any other can’t play ?Also is your country really need you ?
Most likely it’s your remittances which are more important for your family and your country .It’s an improvement in your skills which can be later on better used by your country .
Professionals including doctors are a commodity .I am happiest we are exporting atleast something .This will also increase the value of the doctors who are behind which will have more space to grow and build better practices as well as job opportunities.
Message to the ones who are in start of their career is that yes do try to go abroad and make a future for yourself and at the same time send remittances as well as yours skills back and create a positive impact !!!
For next couple of years your every effort should be in this direction and definitely you will succee as you get what you effort for !!!

Why do ppl prefer plab rather than fcps?? Briefly, Because during fcps training you have long hectic duties, you are underpaid, you get humiliated everyday, exit exam is s**t, budhe khabti examiners never pass you untill you appear 3-4 times, even after passing part 2, there are no jobs, ppl are doing moships for few thousands after passing part 2,, if you are lucky enough to become sr or ap,, still you are under ms or mpas,, after this long long journey you yourself become psycho and start doing the same with your juniors as you have gone through this and vicious cycle started.

On other hand after passing plab, if you work as junior dr in uk you have same respect as consultant, there will be not much difference between pays of junior doctors and consultants, you have no exit exams, your lifestyle gets better,,, you get justice, you never get bullied, in short heaven on earth is waiting for you after passing plab 2 so leave this country asap, and then pray for this country .. otherwise here a toilet cleaner of filthy politician has more power than you and he can humiliate you without facing any consequence

04/12/2022

Today fcps induction test for medicine was totally absurd
Mcqs were taken from biotechnology nd genetics which medical fraternity have never studied throughout their medical carrier or curriculum.
Not a single mcq from medicine/pgmi book pretest/book for basic sciencs was found in the paper.
Candidates struggled and studied for consecutive 3 months..
The candidates stood up altogether during paper to buycott and question it's merit but the administration started threatening the candidates and brought security/police to stop th which was an attack on the dignity of white coat..
Pgmi have proved it's incompetence.
We strongly condemn the injustice with cream of the society in today's ETEA PGMI test held at PGMI peshawar. It’s complete injustice with junior doctors.
Moreover the test was arranged in a car parking near the sevage system of the building which was not only an insult but also the candidates s could have been infected and the bad smell was making the situation even worst.
MDF will look into the matter and will use every available forum to preserve the dignity of white coat.

Canadian 🇨🇦 Medical Pathway :

It’s bit complicated so I have tried to summarize .Important website links are added to help .

Steps to Get Registered with Canadian 🇨🇦 Licensing Body :

➡️1) Confirm your primary medical degree is acceptable :✅

Visit the International Medical Education Directory (IMED)
http://www.faimer.org/resources/imed.html
part of the Foundation for Advancement of International Medical Education and Research's (FAIMER) website.

➡️The Medical Council of Canada's Physician Credentials Repository (PCRC) :✅

Your acceptable medical degree must be source verified by the Medical Council of Canada's PCRC (Physician Credentials Repository).

🛑You must check with the organisation with whom you are applying which documents need to be sent to the repository. Once verified, the documents are stored for life in the repository, and can be shared with many organisations.

➡️IMGs must provide proof of language proficiency. ✅
This may involve taking the TOEFL (Test of English as a Foreign Language) and the TSE (Test of Spoken English). In Québec, this involves taking the French examination of the Office Québécois de la langue française.

🛑Your medical school, the name of the medical degree and the year of your graduation must be listed on the FAIMER International Medical Education Directory to be accepted in Canada.

➡️2) Make sure you are exam ready :✅

IMGs can test ❓their readiness for the Medical Council of Canada Evaluating Examination (MCCEE) through the Medical Council of Canada Self Administered Evaluating Examination (SAE - EE).

IMGs who take the SAE - EE will receive the number of questions correctly answered as well as a percentile table that compares their performance to the results achieved by other MCCEE candidates.
🛑Fees should be checked at time of applying.

➡️3) Submit your credentials :✅

You can use the MCC's Physician Credentials Repository to establish a confidential professional electronic portfolio of their credentials before arriving in Canada. You can request that the Repository share their portfolio with provincial or territorial medical regulatory authorities, certifying and qualifying bodies.

➡️➡️The MCC requires IMGs who are applying for the first time to the MCCEE to open an application with the Physician Credentials Repository and send a certified copy of their final medical diploma.
🛑The diploma must be successfully source verified through the Repository for the candidate to be eligible to the Medical Council of Canada Qualifying Examination (MCCQE) Part II.

Fees should be checked at time of applying.

➡️4) Take the Medical Council of Canada Evaluation Examination :✅

IMGs must pass the MCCEE to apply for a residency position through the Canadian Resident Matching Service.
http://www.carms.ca/
The MCCEE is offered in 500 sites in more than 70 countries. If IMGs have obtained Canadian or American board specialty certification, they may apply to the Medical Council of Canada for an exemption from the MCCEE. Fees should be checked at time of applying.

Note: The MCC requires all first-time MCCEE candidates to open an application with the MCC's Physician Credentials Repository and send a certified copy of their medical diploma.

Licentiate of the Medical Council of Canada (LMCC)

The LMCC is also one of the requirements to obtain an independent licence to practise medicine in most of the provinces or territories of Canada.

In addition to a pass standing on the MCCEE, you must have passed the Medical Council of Canada Qualifying Examination Part I (MCCQE Part I) and the Medical Council of Canada Qualifying Examination Part II (MCCQE Part II).

You must have completed a minimum of twelve months of acceptable postgraduate training to be eligible for the MCCQE Part II. Source verification of the medical degree or diploma is required for eligibility to take the MCCQE Part II.

For postgraduate doctors who want to join
.

4 years back general medicine was not recommended as it’s era of specialities .However people perspective has changed now .
I strongly recommend 4 years internal medicine equally for males and females .Internal/ General medicine specialists are more needed then ever in this era of infections which is not going to end for decades .You can establish an excellent practice once you become specialist .Just you need to develop your skills and competencies as much as you can while training and get expert in one area alongwith .Like internal medicine specialist with interest in diabetes and so on .
Now problem is most our trainees open books only at time of IMM and FCPS2/MD and don’t practice evidence based medicine like doctors from sub/super specialities do .You need to change this from day 1 of your training and the best way to do this is by appearing in MRCP UK exams .They will polish your skills and make you an excellent physician. This advice is totally Independent of whether you want to go abroad or not .MRCP UK will help you throughout your career not only as an extra qualification but also imparting excellent skills in you .Appear in MRCP UK 1&2 within first two years of training and PACES by 4th year of training .
There are hundreds of jobs in the UK and Gulf for medical specialists. After IMM you can go to UK via CPSP MTI route and get international exposure of two years and so on.
You can appear in SCE exams of different specialities to improve your knowledge in any particular area like acute medicine ,palliative medicine ,geriatric medicine etc
You can either sit in center or periphery and can maintain an excellent practice .
Even if you plan to do a superspeciality do start practice as medical specialist the day you clear your FCPS/MD medicine .
Please give feedback & ask questions in comments (avoid to inbox )

What to do at different points of career ❓
I am writing few algorithms which may help making career decisions ‼️

➡️If you are in medical school

+ Excellent financial support -Go for USMLE
Start preparing for STEP1 at 3rd year of MBBS ✅

+ Normal financial support - Plan for PLAB /Ireland / FCPS/ MD MS and gather information about them especially house job requirements ✅

🔴Most important thing should be to clear your final year MBBS exam in first attempt !!!

➡️Just Cleared MBBS

+Excellent Financial Support: Do USMLE ✅

🔴Note : If you have already cleared USMLE 1 during your MBBS years with good score - Skip House Job and complete your STEPS

+Excellent Financial Support : Start USMLE preparation but do house job alongwith and take career break after house job to complete your USMLE journey
Don’t do any other job apart from house job ✅

+Normal Financial Support : Do OET during third or fourth rotation of the house job and focus on PLAB or Ireland route if planning to go abroad .✅

+Normal Financial Support : Start preparing for FCPS/JCAT 1 in last rotation of house job and clear the exam as soon as you are eligible to appear if planning to stay in country ✅

🛑Note : Do house job that fits every licensing body I.e. 3 months medicine and 3 months medicine allied
3 months surgery and 3 months surgery allied

➡️MBBS + House Job + Doing PLAB or waiting Irish registration and job

Start a job at a private hospital, remain in clinical practice till you go abroad ✅

MBBS + House Job + Doing FCPS1 /JCAT

Look for a job at BHU and till then don’t do private job and appear in first attempt without any other job and clear it ✅
If not clear in first attempt start any available job govt or private

➡️MBBS + House Job + Cleared FCPS1/JCAT and waiting for residency

Start appearing in Royal College Exams of your speciality✅

➡️MBBS + House Job + Not interested or unable to do residency

Start your own practice as soon as possible even if you have to take a lone from bank by opening your own clinic (preferred to do group practice with one or two more doctors ) ✅
🔴Do attend courses to enhance your skills meanwhile

To be continued

Algorithms about ur carrier
doc must read

➡️Done with FCPS/MD1 + Not finding residency
Do apply for residency in private set up wherever you get ✅
🛑In surgical specialities and Gynae training institute matters
In medicine it does not matter !!!

➡️If you are doing FCPS/MD training + Struggling with finances
Start a private clinic of your speciality at any hospital 🏥 3-4 days a week as per your rota
i.e If doing in Paeds do general Paeds OPD,If medicine do general medicine and so on ✅
(Check with your training body /supervisor regarding any restrictions to practice )

➡️If you are doing FCPS/MD training + Not struggling with finances
Start appearing in Royal College licensing exams I.e. MRCP,MRCS ,MRCOG ✅

➡️If doing FCPS residency + Cleared IMM
Appear in OET as soon as possible so that you can apply for MTI seats as and when they become available ✅

➡️If completed FCPS / MD/MS residency and yet to appear in Part 2
Try to give first attempt without being in any job with full preparation ✅

🛑Clearing Part 2 is most important part of your career !!! Be ready to sacrifice money and time for it
Atleast a year

➡️Done with residency + Unable to clear Part 2 after first fully prepared attempt
Start a morning job or clinic and appear in part 2 exams with job side by side ✅

➡️Cleared FCPS2/ MD + Planing to stay in Pakistan
Start clinic straight away and wait with patience for some years to establish the practice ✅

➡️Cleared FCPS2/MD + Not mandatory to stay in Pakistan
Go to gulf and use it as a transit route to go to Ireland or UK ✅
Appear in OET and apply for senior clinical fellowship via CPSP route (if done with FCPS) ✅

Thank You
