Ultrasound Physics and Basic Principles
Ultrasound is a mechanical, longitudinal wave that travels in a straight line and is measured by cycle per second by Hertz(Hz) unit. Audible Sound 20-20,000 Hz. sound requires a medium through which to travel. Ultrasound is a mechanical, longitudinal pressure wave with a frequency exceeding the upper limit of human hearing, which is 20,000 Hz or 20 kHz. Medical Ultrasound is > 2MHz, (2MHz to 16MHz).
The sound waves travel into the body and hit a boundary between tissues (e.g. between fluid and soft tissue, soft tissue, and bone.
Basic Ultrasound Physics Amplitude oscillations/sec = frequency - expressed in Hertz (Hz). Wavelength & frequency.
Ultrasound waves with shorter wavelengths have a higher frequency and produce higher-resolution images, but pe*****te to shallower depths. Conversely, ultrasound waves with longer wavelengths have a lower frequency and produce lower-resolution images, but pe*****te deeper.
Transducer
It’s a device that produces sound waves that bounce off body tissues and make echoes. The transducer also receives the echoes and sends them to a computer that uses them to create a picture called a sonogram. The higher frequency transducer provides better image resolution.
Transducer Types
• Mechanical
Oscillating
Rotating
• Electronic
Linear Arrays
Curved Arrays
Phased Arrays
Ultrasound Artifacts
The ability to recognize and fix correctable ultrasound artifacts is important for getting quality ultrasound images and optimizing the care of your patients.
Here are the main ultrasound artifacts we will cover:
• Mirror Image Artifact
• Acoustic Shadowing Artifact
• Posterior Acoustic Enhancement
• Edge Shadowing Artifact
• Reverberation Artifact
• Comet Tail Artifact
• Ring Down Artifact
• Side Lobe Artifact
Generation of Ultrasound Images
Sound waves are reflected, refracted, scattered, transmitted, and absorbed by tissues due to differences in the physical properties of tissues. Ultrasound images are generated by sound waves reflected and scattered back to the transducer. Transducers receive and record the intensity of returning sound waves. Specifically, mechanical deformation of the transducer’s piezoelectric material generates an electrical impulse proportional to the amplitude of these returning sound waves. Electrical impulses cumulatively generate a map of gray-scale points seen as an ultrasound image. The depth of structures along the axis of the ultrasound beam is determined by the time delay for echoes to return to the transducer. The transducer repeats the process of emitting and receiving sound waves sequentially, resulting in a dynamic picture. The reflection and propagation of sound waves through tissues depend on two important parameters: acoustic impedance and attenuation.
In figure A, Ultrasound waves are reflected, refracted, scattered, transmitted, and absorbed by tissues.
In figure B, Generation of ultrasound images.
• Oscillating voltage is applied to piezoelectric crystals.
• Piezoelectric crystals vibrate rapidly, producing sound waves.
• Ultrasound beam pe*****tes tissues.
• Echoes (reflected sound waves) return them to the transducer.
• Echoes are converted to electrical signals that are processed into gray-scale images.

Reflection:
The ultrasound reflects off tissue and returns to the transducer, the amount of reflection depends on differences in acoustic impedance. The ultrasound image is formed from reflected echoes.
Refraction:
Transmission Some of the ultrasound waves continue deeper into the body. These waves will reflect from deeper tissue structures.
Attenuation:
The deeper the wave travels in the body, the weaker it becomes -3 processes: reflection, absorption, refraction Air (lung)> bone > muscle > soft tissue >blood > water. Speed of sound in Air= 330 m/s, Bone=4030 m/s, Tissues =1040m/s.
Transmission:
Some of the ultrasound waves continue deeper into the body. These waves will reflect from deeper tissue structures.
Muhammad Iqbal, also known as Allama Iqbal, is the National Poet of Pakistan. A poet, philosopher, politician, lawyer, and scholar, Iqbal was born on November 9, 1877, in Punjab, Pakistan, to Kashmiri parents and educated at Scotch Mission College in Sialkot. He received BAs in philosophy, English literature, and Arabic at Government College University, where he was awarded the Khan Bhadurddin F. S. Jalaluddin medal. In 1905, Iqbal worked closely with Sir Thomas Arnold while studying philosophy at Trinity College Cambridge in England.
Having written a treatise on economics, Iqbal also wrote the Urdu ghazal Sare Jahan se Achccha Hindostan Hamara, which became a rallying cry against the British Raj. The song, an ode to Hindustan (present-day Bangladesh, India, and Pakistan), eventually became a patriotic ballad sung largely in India.
Iqbal is widely known as one of the most important figures in Urdu and Persian literature, having written numerous volumes of poetry in both languages. After returning to Lahore in 1908, Iqbal taught philosophy and English literature while practicing law. His first book, Asrár-I Khudí (The Secrets of the Self), was published in 1915, followed by Ramuz-e-Bekhudi (The Mysteries of Selflessness) in 1918—both of which are regarded as the base for his poetic philosophy. He continued to write and publish volumes of poetry up until his death on April 21, 1938.
What Is the Difference Between Sonography and Ultrasound?
Sonography is a technique that uses high-frequency sound waves to produce images of internal body structure. For that purpose, ultrasound waves are used. The images of organs, tissues, and blood flow, are produced. The image generated by the ultrasound is called a sonogram.
Sonography is also known as ultrasonography. The benefits of ultrasound are-
• Non-invasive
• Affordability
• Extremely safe
• No radiation
• Portable
• Widely accessible
• Versatile
• Easy to use
• Real-time imaging
In simple terms, sonography is the technique and ultrasound is the tool.
Sonography is done by sonographers using ultrasound devices. Literally, sonography also means sound writing and ultrasonography means extreme sound writing. This is because the sonographers use high-frequency sound waves to create images during the usg scan.
These high-frequency sound waves move energy through solid, liquid, and air by colliding with the particles around. Sound waves quickly move through solids when compared to air. The waves move at a different speed when the sound comes across your internal structures. The waves bounce off as echoes. When processed by a computer, these echoes create images called sonograms.
Ultrasound devices create both 2D and 3D images. For the past decades, gynecologists use 2D ultrasound to examine the development of the fetus. Even though 2D images are useful, 3D ultrasound can create clearer images of the fetus. Since ultrasound does not use radiation, it is used for the diagnosis in case of an emergency without the need for other imaging techniques.
Ultrasound is also used to guide procedures. An example is a needle biopsy. In a needle biopsy, a needle is used to collect cells from the affected area for diagnosis. It is also used to guide the biopsy of breast cancer. Sometimes, the soft tissues won’t appear clearly in X-rays. In such cases, ultrasound is preferred.
Ultrasound is also used to determine potential pelvic inflammatory disease, pelvic masses, abscesses, and endometriosis. It is also used to examine the reproductive system of patients with infertility issues.
Considered one of the best imaging techniques, sonography helps doctors to make a diagnosis easily not only is sonography safe, but it is also non-invasive, portable, and affordable. Since it is portable, patients who won’t be able to visit a hospital or imaging center can undergo sonography from where they are. But before undergoing the procedure, always enquire about the procedure and follow the doctor’s instructions.
