We aim to ensure that a person living with dementia meaningfully and cognitively engages regardless of where they reside. Therefore, we conduct cognitive online activities for people living with dementia.

Online activities will benefit some people living with dementia who is homebound, have wandering behaviour and, keep their boredom away, prevent psychological issue.

Will assess the person living with dementia for their suitability for online activities from 1st session before moving to 2nd session.

Family members who are living with dementia may contact me for more information. We could do a short presentation on Memory Care!

Hello everybody, I can empathise with the person living with dementia and their family. It is a challenging journey, but services are in place to assist and aim for persons living with dementia to live independently with assistance as long as possible in the community.

The key word is ENGAGEMENT.

I conduct cognitive activities at the home of the person living with dementia and aim to assist them to live with dementia. Activities are going to engage them meaningfully.

Home-Based service where local trained staff do memory care with the person living with dementia.

How we do?
- Design person-centered care activities (individualize activities plan)
- Used Montessori dementia care approach
- Assessment using BPSS model

Aim:
- To increase in self-esteem and sense of accomplishment for person living with dementia
- Stay in the community as long as possible
- Cope with functional ability as much as possible living with dementia
- Minimize caregiver strain
- Management of behavioral symptoms (BPSD)
