WELCOME TO CHINA!NO QUARANTINE!

China has finally decided to remove some of its most stringent travel restriction measures, a move that has been long-awaited by business groups. Starting from January 8, 2023, among other changes, China will no longer conduct nucleic acid tests and centralized quarantine for all inbound travelers, and measures to control the number of international passenger flights will be lifted.

What are the changes?
On December 26, 2022, China’s National Health Commission (NHC), a cabinet-level executive department of the State Council that is responsible for formulating national health policies, announced that the official name for COVID-19, the “novel coronavirus pneumonia”, will be changed to the “novel coronavirus infection” (COVID-19 infections), and preventive and control measures for a Class B infectious disease will be applied to COVID-19 infections. Previously, novel coronavirus pneumonia was classified as a Class B infectious disease, but was subject to the prevention and control measures of a Class A infectious disease, due to its high transmissibility and risks to health and life.

Moreover, COVID-19 infections will no longer be included in the administration of quarantinable infectious diseases as stipulated in the Border Health and Quarantine Law of the People’s Republic of China.

Another circular released by the NHC on the same day, the Overall Plan for Implementing Class B Infectious Disease Management for COVID-19 Infections (the Overall Plan), further clarifies that starting January 8, 2023, China will:

No longer impose quarantine measures on COVID-19 infections;
No longer identify close contacts of COVID-19 infections;
No longer delineate high and low-risk areas;
Provide classified treatment for COVID-19 patients and adjust medical insurance policies;
Adjust PCR tests to voluntary testing;
Adjust the frequency and content of epidemic information release; and
Take NO quarantinable infectious disease control measures against entry persons and goods.
To be more specific, the Overall Plan mentions that China will “improve management of personnel exchanges between China and foreign countries” and implement the following measures:

Inbound travelers to China will no longer need to apply for a health code from Chinese embassies or consulates, though a negative nucleic acid test from the last 48 hours will still be required.
Nucleic acid tests and centralized quarantine for all inbound travelers will be canceled. If the health declaration is normal and the customs port routine quarantine check is normal, they can be released into the community without further requirements.
Measures to control the number of international passenger flights, including the “five-one” policy (in which every foreign airline was required to maintain only one air route to China and operate no more than one flight a week) and the passenger load factor limit, will be lifted. Nevertheless, airlines will still be required to prevent disease on board, and passengers shall be required to wear masks when flying.
China will further optimize arrangements for foreigners returning to China for work resumption, business, study, family visits, and reunions, and provide visa facilities accordingly.
The entry and exit of passenger transport by water and land ports will be gradually resumed.
China will resume outbound tourism in an orderly manner in light of the international epidemic situation and the service support capacity of all sectors.

What is Autism?

Autism is not a single disease entity. It is part of a range of developmental disorders known as autistic spectrum disorders (ASD). They begin in childhood and last through adulthood.

There is currently no cure for ASD. Treatment these days includes a wide range of specialist education and behavioural programmes that can help improve symptoms.

Symptoms of autism
ASD has a wide variety of manifestations. These can be categorized into three broad groups.

Problems with social interactions. The affected children may have difficulty understanding the emotions and feelings of others
Problems with language and communication skills – This may manifest as delayed language development and difficulty in starting conversations
Unusual behaviors and patterns of thoughts – this could include repetitive movements and activities and the child getting upset if routines are altered.
Incidence of autism
In England it is estimated that 1 in every 100 children has an ASD. According to the Centers for Disease Control and Prevention (CDC). ASDs occur in all racial, ethnic, and socioeconomic groups, but are almost five times more common among boys than among girls. CDC estimates that about 1 in 88 children is identified with an autism spectrum disorder (ASD).

Over the last two decades the number of cases of autism are seemingly on the rise. This may not mean that the condition is becoming more widespread. Some experts argue that the rise in diagnosed cases may be due to health professionals getting better at diagnosing cases correctly. Earlier children with autism would be missed commonly and they were merely labelled “painfully shy” or “slow”.

What is Abdominal Pain?

Abdominal pain or pain in the belly is the reason for around 5% of all emergency department visits. It is a symptom but may signify a life-threatening condition underneath.

Types of abdominal pain
Sometimes the pain may be acute among patients with an existing condition including those with peptic ulcer disease, acute pancreatitis among alcoholics etc.
Sometimes there may be acute pain in an otherwise normal person. This includes examples like appendicitis, gastritis, biliary colic, gastroenteritis etc.
Acute pain can occur following injury to the abdomen which includes rupture of the liver, spleen, etc.

Examination and diagnosis
Examination, level of emergency and urgency of diagnosis and management depends on the cause of the pain. All patients with abdominal pain do not require diagnostic tests. Sometimes, clinical evaluation alone is sufficient for diagnosis and management.

Some of the major factors that are assessed by emergency physicians include:

Degree of pain - severity is best evaluated with a familiar 1-10 scale.
Changes in vital signs like heart rate, blood pressure, respiratory rate. If the vitals are abnormal with heart rate over 100 beats per minute, blood pressure less than 100 (systolic), respiratory rate over 20 per minute etc. a deeper cause should be suspected.
History of any disease like diabetes, alcoholism, or injury that may provide clues for diagnosis.
Location of the pain - there are six anatomic locations. These include – right hypochondriac region, epigastrium, left hypochondriac region, right lumbar, left lumbar, umbilical, right iliac, left iliac and hypogastric region.
Nature of onset of the pain. Pain may originate abruptly or may begin slowly over time.
Nature of pain.
Radiation of the pain or if the pain moves to the back or to the groin.
Any presence of aggravating and reliving factors. For example, some pains may be relieved after eating and some after sitting or lying in a particular position.
Presence of other symptoms such as vomiting, diarrhea, dizziness, pain on urination etc.

Scientists create new tool to identify genomic safe harbor sites in specific tissues

Scientists at St. Jude Children's Research Hospital have created a tool that can find safe places to introduce genes into human DNA. The tool is an early step in the process to improve the safety and efficacy of gene and cell therapies. The work appears today in Genome Biology.

Gene therapy, where a patient is given a functional copy of a dysfunctional gene, has shown success in curing certain genetic disorders. However, the field has encountered safety issues, including the inadvertent activation of an oncogene that led to cancer in some patients. In response, the field has searched for "safe harbor sites" – places in the genome where a gene can be inserted without causing cancer or other problems. The scientists created a pipeline that uses genomic and epigenetic information from specific tissue, such as blood cells, to find safe harbor sites.

A novel way to find safe harbor sites
The tool compares the DNA sequences that are highly variable between healthy people, using data from the 1000 Genomes Project. If a region of DNA is often deleted or inserted in healthy people, the researchers reasoned that it could likely also be altered safely by a gene therapy.

"Our method is a new way to identify genomic safe harbor sites in a tissue-specific manner," Cheng said. "Nobody has tried it from this angle. Our first step was to find the genomic loci that show a high frequency of insertion or deletion among healthy individuals."

If DNA in a single cell was a string, it would be two meters long. But in addition to the linear sequence, DNA can loop into complex 3D structures using chromatin, the proteins associated with DNA, to fit within a cell. Just like a string, DNA can have loops that affect its function. The St. Jude tool considers the presence of these loops and other structures when searching for accessible safe harbor sites.

The "Big Three" of Biocompatibility Testing

Biocompatibility testing is performed on medical devices to ensure that they will not cause any adverse effects once in contact with or implanted into a patient, and as a standard, all such devices must be assessed with regards to cytotoxicity, sensitization, and irritation, often termed the "big three."

The evaluation of all medical devices for biological contact is set out by international standard ISO 10993, which includes personal protective equipment such as gloves and masks, gowns worn by the patient, surgical tools, and any other implanted or externally applied device.

What are the big three?
Cytotixicity
Cytotoxicity is the measure of direct toxicity towards cells and is routinely tested for biocompatibility by incubating the medical device in mammalian cell culture to allow native proteins and other biomolecules to interact with and adhere to the surface as they would in situ. The device and/or any leeched materials (extract) are then removed from culture and placed in contact with relevant cells in vitro, either directly or with a porous separating layer, to establish the toxicity of the device to adjacent cells by measuring cell viability after that at various time points.

Any number of cell viability assays may be appropriately applied to determine material cytotoxicity towards a particular cell line, or alternatively, high-throughput cell analysis techniques such as flow cytometry may be employed. Importantly, cytotoxicity testing can be used to reveal the mechanism by which the material is inducing cell death, allowing appropriate design changes to be made.

ISO 10993-5 calls for the use of L929 mouse fibroblast cells as a standard and advises qualitative evaluation of cells by microscopy and cytochemical staining, where changes in cell morphology and membrane integrity are indicative of material toxicity.

What is Incontinence-Associated Dermatitis?

Incontinence-associated dermatitis (IAD) is a form of dermatitis caused by irritants that develop from chronic exposure to urine or liquid f***s. Incontinence-associated dermatitis (MASD) consists of one of four types of moisture-associated skin damage. Moisture-associated skin damage has been defined as gradual inflammation and the erosion of the skin as a result of prolonged exposure to several sources of moisture which may include perspiration, wound exudate, mucus, saliva, urine, or f***s.

The remaining etiologies of moisture-associated skin dermatitis include intertriginous dermatitis (caused by perspiration), peri-wound MASD (caused by wound exudate), and peristomal MSD (caused by ostomy output).

It is important to identify the correct cause and differentiate incontinence-associated dermatitis from other skin conditions to guide prevention and treatment. Evidence-based medicine, which uses explicit and judicious evidence in making decisions about the individual care of patients, is necessary. These comprise a professional's clinical experience, the patient's preference, and the best guidance sourced from the best external advice; these components must be considered when developing a care plan.

The extent of incontinence-associated dermatitis
Irritant-associated dermatitis is an under-reported health care condition. In 2003, an initial sample of 20,000 and 28 American adults with 30 acute and chronic conditions did not report any findings associated with incontinence or its impacts on the skin, such as incontinence-associated dermatitis.

In a recent review, which examined the extent of the phenomenon, the study of a ward examined 272,841 encounters in which incontinence-associated dermatitis was diagnosed. The infant prevalence was estimated to be 7%, with the maximum prevalence occurring between nine and 12 months.

The prevalence of incontinence-associated dermatitis in the critically ill further ranges between 7 and 50%, and incontinence-associated dermatitis occurs in half of those patients who suffer from diarrhea in the intensive care unit. Similarly, incontinence of urine, f***s, or both, are associated with high levels of incontinence-associated dermatitis; of those with incontinence, 45.7% suffered from incontinence-associated dermatitis.

In long-term and acute care, a study has reported an incidence of 7.6% in a long-term acute care facility in the United States. The reason for this rate of prevalence was attributed to the use of soap, water, and washcloths routinely in the care of persons with incontinence-associated dermatitis.

Dissociative amnesia is a type of dissociative disorder characterized by the mental separation an individual may experience from a specific aspect of their self. This amnesia typically occurs following a traumatic or stressful life event. It has been argued that the dissociation enables the individual to cope with the event. Those with the disorder may experience some of the following symptoms.

Inability to recall vital information about one’s self which may be stressful or traumatic in nature
Memory issues experienced are not associated with other psychological disorders such as posttraumatic stress disorder, dementia, or Alzheimer’s
The symptoms significantly impact the individual’s ability to manage their daily life
Those with dissociative amnesia may forget a particular event, who they are, or their personal history. Furthermore, they may or may not be aware that they have issues with memory recall; they may, however, show symptoms of being confused.

How is Dissociative Amnesia Treated?
Treatments for dissociative amnesia aim to relieve symptoms, recover any lost memories, and deal with any issues surrounding the event that led to the development of the disorder. It is important to note that there is no single recommended treatment option, however, psychotherapy may be of benefit.

In a recent study published in the Journal of the American Heart Association, researchers assessed midterm and management outcomes of acute myocardial infarction (AMI) among patients with rheumatic immune-mediated inflammatory diseases (IMIDs).

AMI has been associated with a cascade of local and remote immune response activation. In addition, studies have reported a positive association between rheumatic IMIDs and the risk of cardiovascular disorders such as ACS (acute coronary syndrome). However, the long-term prognosis of ACS among patients with rheumatic IMIDs has not been well-characterized.

About the study
The present study evaluated AMI outcomes among patients with rheumatic IMIDs.

The study comprised 1,654,862 Medicare beneficiaries with a 3.6% prevalence of rheumatic IMIDs, the most common of which was rheumatoid arthritis, followed by systemic lupus erythematosus, and hospitalized between January 2014 and December 2019. The outcomes of patients with AMI and concomitant rheumatic IMIDs such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), dermatomyositis, psoriasis, or systemic sclerosis were compared to those among 1:3 (IMID group: controls) propensity-score-matched (PSM) control patients with no rheumatic IMIDs.

Malaria: New vaccine candidate shows promise in clinical trials

Results from a clinical trialTrusted Source published in 2021 showed that the R21/Matrix-M malaria vaccine candidate had high efficacy against symptomatic malaria in children aged 5–17 months at 12 months following a primary three-dose regimen.

New findings from the same clinical trial recently published in The Lancet indicate that the administration of the booster shot of R21/Matrix-M 12 months after the primary three-dose regimen helped maintain a high efficacy against symptomatic malaria over the 12-month follow-up period after the booster dose.

Dr. Azra Ghani, a professor at Imperial College London, who was not involved in this research has commented that “[d]espite ongoing efforts to reduce the malaria burden — including through the provision of insecticide-treated bed nets, improvement in access to treatment, and chemoprevention — malaria continues to pose an unacceptably high burden, resulting in over 640,000 deaths globally each year, mostly in young children in Africa.”

“These new results demonstrating high sustained efficacy of the R21 malaria vaccine over a 2-year period are therefore very welcome,” says Dr. Ghani.

How malaria infections occur
Malaria caused an estimated 627,000Trusted Source deaths worldwide in 2020, with nearly 93% of all malarial deaths occurring in Sub-Saharan African nations. Moreover, 80% of deaths due to malaria in Africa occurred in children aged under 5 years.
