May Thurner syndrome is compression of the left common iliac vein between the spine and the right common iliac artery.

This causes females to have left legs larger than right legs. Females can present with significant venous insufficiency, including swelling that is worse at the end of the day and pigmentation changes of the skin. Females with May Thurner Syndrome may also have labial and vaginal varicosities.

Worst presentation is females, presenting with extensive deep venous thrombosis of the left leg due the compression. This will require a patient to undergo lysis of the clot in the hospital.

At Texas Vascular Institute we proactively evaluate females with these signs and symptoms to rule out May Thurner syndrome in the correct clinical setting. We conduct an ultrasound of the legs and a possible CT scan of the pelvis to evaluate for compression.

The treatment is placing a stent in the left common iliac vein to allow blood to return to the heart. This significantly decreases the risk of the patient having extensive deep venous thrombosis from the compression.
