What goes in clinical documentation.
At minimum: the medical record should have -
1. Person referring the patient when appropriate
2. Chief Complaint
3. Presenting problem
4. Mental status evaluation/assessment
5. Personal, mental, and family history
6. Comprehensive biopsychosocial history at the initial visit
7. Social history including but not limited to over the counter and herbal preparations
8. Diagnosis
