In today's #MondayMinute, Karly reminds us about the quickly approaching June 15th cutoff for Aged and Disabled and Traumatic Brain Injury waiver members and their families to submit a transition plan with your care manager. FSSA announced in January that waiver members who receive attendant care provided by a legally responsible individual would need to transition to an alternative provider or to Structured Family Caregiving by July 1. In order to have new service plans approved by July 1, those plans must be developed with your care manager, who will then submit it to the state, by June 15. #2024WaiverChanges #UpcomingDeadline #Reminder #MondayMinute
