Let’s talk about Adjacent Segment Disease (ASD). This is a condition characterized by degeneration of vertebral segments above or below a previously fused section of the spine. This is thought to occur due to the increased stress on the neighboring spinal segments, sometimes leading to pain and other symptoms from further degenerative changes.

This is a case of a very active female in her 60s. She had previously undergone L4-5 posterior lumbar decompression with instrumented fusion performed by another surgeon about a five years ago. She initially did very well, however, she gradually developed low back pain radiating to both hips and legs. This gradually worsened, and she was eventually unable to walk more than 50 yards at a time due to severe hip and leg pain. Imaging studies revealed adjacent segment disease at the L3-4 segment, just above her prior fusion with associated severe spinal stenosis. I performed a revision lumbar spinal surgery, with decompression of the L3-4 segment and extension of her fusion construct to this segment. She did extremely well, with immediate resolution of her preoperative hip and leg symptoms, and she has since returned to all her previous activities.
