She stopped by our office today to review community activities and to plan for her activities for next month. Yessinia enjoyed how artistic our office was, and asked to take photos. She loves , , glitz and glamour………She wants the world to see that with First Lynx help she can continue to express herself and show her true personality in the community with assistance from Morningside Health as needed. ☀️☀️☀️ Well Good Morning 👋🏽!!!!
Each American state has their own form of governmental assistance to help adults who have been diagnosed ,before a certain age, with a developmental disability with services that will enhance their education, recreation, vocational, medical, and personal care needs for life.

To be exact, I am also from New Jersey. Services for the population I mentioned are funded through a State Division Medicaid Program that supports adults with developmental, intellectual, and/or mental health disabilities. An individuals annual budget will be determined by their level of care within three areas: Support, Behavioral, and Medical.

Once an individual has met the qualifications, applied and was approved to receive assistance they are initially connected , by choice or auto assignment, to what is called a Support Coordination Agency (SCA). The individual will then be provided lifelong case management services by a certified support coordinator.

It is the support coordinator‘s job to connect with the individual to discover their goals. Then create a plan of care that outlines the individuals goals to offer potential providers with a guide to best support the individual accomplish what they hope to achieve. Yes, the care plan outlines an individuals difficulties within the three areas mentioned before but it also describes an informative review of an individuals strengths, interests, and their will for a more comfortable fulfilling life that provides most importantly ,inclusion into the community. A support coordinator offers individuals state funded resources to accomplish their goals but for a fuller scope of opportunities the support coordinator must also inform individuals of community resources.

So without further ado, i’d like to introduce myself. My name is Camille Jackson and I am the support coronation supervisor with First Lynx. First Lynx is a Medicaid approved support coordination agency that has also been released to approve their own plans for the last 10 years. I’ve provided support coordination for the last 12 years of my life and even witnessed New Jersey state services before the development of the self-direction/fee for service model.
