KETO DIET AND KIDNEYS:
Higher-protein ketogenic (keto) diets may hasten kidney failure and cause other medical problems in patients with kidney disease, according to the most comprehensive review yet of these diets.

Keto diets are low in carbohydrates and high in fat with 1.2-2.0 g/kg/d of protein. Long-term risks of the keto diet include heart disease, cancer, diabetes, kidney stones, Alzheimer's disease, and other diseases.High animal fat consumption is associated with increased risks for albuminuria and chronic kidney disease (CKD). Evidence shows that ketogenic diets can lead to further harm and should not be recommended.
A diet consisting of vegetables, fruits, whole grains, and legumes, appropriately planned will lead to healthier long-term weight control, in addition to helping preserve kidney function to the extent possible.

A radical new treatment for severe high blood pressure could slash sufferers’ risk of heart attacks and strokes. The one-off therapy, which takes about an hour, involves blasting nerves in the kidneys with sound waves to stop them sending signals to the brain that drive up blood pressure.

More than a third of patients are able to stop taking blood pressure tablets altogether after the ultrasound treatment, while others are able to cut right back – reducing the risk of side effects
Doctors say the treatment lowers blood pressure to levels at which the risk of a heart attack or stroke is cut by a fifth – and trials so far show a single session works for at least three years.

Immune Thrombotic Thrombocytopenia Linked to the AstraZeneca COVID-19 Vaccine

Uncommon cases of thrombosis and thrombocytopenia followed a first dose of the AstraZeneca adenovirus-vectored COVID-19 vaccine.

Thrombotic events following the first dose of the AstraZeneca COVID-19 vaccine providing evidence of a new syndrome, vaccine-induced immune thrombotic thrombocytopenia (VITT).

11 patients in Europe (median age, 36) developed severe thrombotic events (primarily portal, splenic, and upper mesenteric vein thrombosis, cerebral vein thrombosis, and pulmonary emboli) with concurrent thrombocytopenia within 16 days of vaccine.6 died.All had PF4 ab.
In addition, 24 individuals with suspected venous thrombotic disease after vaccination tested positive for high IgG antibody levels against PF4.

New drug for preventing Diabetic kidney disease.
Diabetes damages small blood vessels throughout the body, affecting the kidneys as well as other organs and tissues including skin, nerves, muscles, intestines, and the heart. More than 1 out of 4 adults with diabetes will eventually develop kidney disease.

People with type 2 diabetes and kidney disease are three times more likely to die of heart-related causes than those with type 2 diabetes alone. Diabetes is also the leading cause of kidney failure, accounting for 44% of new cases.

Finerenone is a non-steroidal, selective mineralocorticoid receptor (MRA) antagonist and it works by directly targeting and blocking the receptors that cause inflammation and scarring of major organs, such as the heart and kidneys.

The trial found that finerenone slowed diabetic kidney disease progression and reduced cardiovascular event rates. It also reduced albuminuria (protein in the urine) by more than 30% at 4 months, had minimal effects on blood pressure, and had no effect on glycemia (blood sugar).

Since September 2020, the task force has been examining every aspect of the inclusion of race in calculations that estimate adult kidney function and are used in both diagnosis and treatment.

To inform its final report, the task force would like to hear from the kidney community and other stakeholders on the breadth of topics reviewed, the implications in the estimation of kidney function, and alternative approaches to the current calculations used to estimate kidney function. The task force invites everyone interested to participate in the process as we seek a solution to address our charge that is safe; equitable; will not disadvantage any patient by race, s*x, or age; and will promote accurate and consistent GFR estimation for all patients throughout the United States. eGFR equations take race (white vs Black)into account even though Race is a social, not a biological, construct .This has likely led to inequalities in kidney healthcare in minorities.
Vital MD is working hard with Kidney care organizations to do away with discriminatory policies which have no scientific basis. Remove race from kidney function calculations. Color of kidney is same.

Black or African American and Hispanic or Latino people are significantly less likely than their White counterparts to receive any kidney care before kidney failure, missing key opportunities for intervention.

Omega 3 no good ;study suggests:
Among statin-treated patients at high cardiovascular risk, the addition of omega-3 CA, compared with corn oil, to usual background therapies resulted in no significant difference in a composite outcome of major adverse cardiovascular events. These findings do not support use of omega-3 fatty acid formulation to reduce major adverse cardiovascular events in high-risk patients.

Beware of Licorice supplement :
In a recent case report a 50 year old man experienced sudden cardiac arrest after eating a few bags of licorice candy daily for few weeks. His kidneys failed and he died shortly after.

Licorice contains glycyrrhizin large amounts of which can lead to very high blood pressure and dangerously low potassium– potentially leading to cardiac arrest and death. Many products contain licorice, including herbal teas ,OTC supplements and jelly beans. Avoid consuming those regularly and in large quantities, especially if there is pre-existing cardiovascular or kidney disease.

Good news for kidney stone formers:
Increased urinary Oxalate is an important cause of recurrent kidney stones. Kidney stones lead to multiple complications like blood in urine, infections, severe pain, even kidney failure with obstruction.
Hyperoxaluria is associated with excessive intake of oxalate in the diet, as well as decreased dietary calcium, hyperabsorption of oxalate, or increased endogenous production of oxalate. This clinical trial examined the effectiveness of an acid stable oxalate decarboxylase (OxDC) to reduce urinary oxalate in healthy subjects on a high oxalate diet and concluded that an orally administered OxDC is capable of significantly reducing urinary oxalate levels in healthy volunteers on a high oxalate diet. This is important since it will alleviate the need for life long dietary restrictions and the constant fear of stone recurrence.

LUNG CANCER screening for blacks maybe inadequate.
Current USPSTF and CMS guidelines, established by results from the National Lung Cancer Screening Trial, recommend low-dose computed tomography (CT) screening for individuals that meet the following criteria: age 55–80 or 55–77 years, respectively; a 30 or more pack-year cigarette smoking history; and, in former smokers, having quit smoking within the past 15 years.However, Black individuals are more likely to start smoking at a later age, smoke fewer ci******es per day, and smoke for a longer duration; they are less likely to quit smoking and tend to be diagnosed with lung cancer at an earlier age. Previous research including 48,364 ever-smokers in the Southern Community Cohort Study showed that a smaller proportion of Black individuals would have met the current USPSTF criteria compared to White individuals—17% vs 31%, respectively.

Black ever-smokers are at increased risk for lung cancer compared to White ever-smokers, after adjusting for age and smoking history.

Assessing risk of heart disease may be as simple as giving stool sample!
'Gut microbiota has a profound effect on cardiovascular function, and this could be a potential new strategy for evaluation of cardiovascular health.”

Researchers used data from the American Gut Project (an open platform for microbiome research based in the United States) to analyze microbial composition of stool samples using artificial intelligence. Nearly 1,000 samples were analyzed, and approximately half of the samples were from people with CVD. The model was able to identify gut bacteria that could potentially help identify individuals with CVD.Eg
Bacteroides, Subdoligranulum, Clostridium, Megasphaera, Eubacterium, Veillonella, Acidaminococcus and Listeria.

The Exclusion of Patients with CKD in COVID-19 Clinical Trials: This study aimed to assess the CKD-related inclusion and exclusion criteria for COVID-19 trials, using data from data available on the World Health Organization International Clinical Trials Registry Platform4 (WHO-ICTRP; data extracted April 22nd, 2020). The data suggest that patients with CKD are being excluded from almost half of all registered clinical trials for COVID-19. The authors concluded that the scientific community has a responsibility to build an evidence base for the treatment of all patients with COVID-19, including those with CKD who may be the most clinically vulnerable.
