Patients often receive relief from vertigo through care and Epley maneuvers. Careful assessment is necessary to differentiate central and peripheral causes.
The following is the GRACE-3 recommendation for the assessment of patients with acute dizziness. This protocol is supported by a high certainty of evidence when performed by experienced ER practitioners.

I’m an evidence based chiropractic orthopedist who loves the philosophy and art of chiropractic.
I did my orthopedic training to rule out metastatic cancer of the sacrum AND better understand the physiology of the vertebral subluxation.
I am a hands-on chiropractor. I think it’s what distinguishes us above all others. I adjust patients. I am trained to perform Palmer Knee-Chest, Gonstead, and Cox adjustments. The way BJ, Clarence and Dr. Cox taught us.
I don’t offer therapies, lasers, fancy lights, butter knives, or suction cups. I refer patients for those therapies to my colleagues. I know the indications and contraindications to meds and surgery and always advocate for patient safety.
I teach my colleagues how to read MRI studies to determine whether the PAO is torn and is a contraindication to our adjustments.
I teach my colleagues the nuanced intra-articular, extra-articular, and neurogenic causes of synovial joint dysfunction.
I teach my colleagues how to read fMRI BOLD images that Harvard and Chiropractic researchers have affirmed CNS changes with chiropractic adjustments.
I don’t think evidence based chiropractic and respect for the art and science of chiropractic are mutually exclusive.
In fact, I think these tenets provide deeper meaning and better outcomes.

Chiropractic Care and Cervical Artery Dissection: NO EVIDENCE OF CAUSATION - PostGradDC Highly powered medical research concludes the chiropractic care is safe and effective.
Through care, we often afford relief to patients who present with IVD herniation and spinal stenosis that manifest motor deficits, neurogenic bladder, dyspareunia, and ED.
Disc herniations can resorb. Radiculitis, Modic 1 and peri-articular edema can be quelled. Pain and neurological deficits can improve. Many patients respond well to chiropractic care without surgical intervention.
In his new NCMIC Examiner article, we offer the case of a patient written by Dr. James Demetrious. After carefully discussing risks and benefits of chiropractic, medical and self-care, the patient chose surgical intervention. She had a favorable outcome: https://www.ncmic.com/resources/case-studies/busy-mom-weighs-surgical-options-for-back-pain/

High quality research clearly indicates that chiropractic care is extraordinarily safe and effective. The most common side effect of chiropractic care is soreness and stiffness.
The first job of the chiropractic physician is to determine if there are any conditions that may require medical referral. Sometimes patients have symptoms due to pre-existing conditions like cancer, fractures and infections that need to be carefully identified.
Rarely, patients seek chiropractic care with head, neck and neurologic symptoms because of a pre-existing and worsening tears of the arteries of the head and neck that can lead to a stroke.
The biomechanical research is very clear. Chiropractors do not cause these tears. Pre-existing connective tissue disorders may cause weakness in the vessels causing them to be susceptible.
In 2018, Dr. Demetrious published a paper that reflects that fluoroquinolone antibiotics like Cipro and Levaquin may lead to weakness of the vessels that causes the artery tears and stroke.

Chiropractic uniquely reduces the need for opioids. Patients who initially seek care before other health care providers experience less opioid use, surgical interventions, morbidity, and cost.
Initial Chiropractic Care Decreases Opioid Use - PostGradDC Initial chiropractic care should become the standard of care for neuromusculoskeletal complaints.
