Willow Branch Transitions, LLC.
Specializing in palliative care preparation and support for both patient and family.
Did you know…
Approximately 90 million Americans are living with serious illness, and this number is expected to more than double over the next 25 years with the aging of the baby boomers. Approximately 6,000,000 people in the United States could benefit from palliative care.
According to a 2010 study reported in the New England Journal of Medicine, lung cancer patients receiving early palliative care had less depression, improved quality of life and survived 2.7 months longer.
Hello Everyone! I am Shannon and I am a Gerontologist and certified End of Life Specialist. I created Willow Branch Transitions, LLC. to support patients and their families during the most difficult time in their lives.
I am able to assist with a number of services to meet your family’s needs. I can help initiate and navigate family conversations to ensure your loved one is treated with respect and with their specific wishes in mind. I help to create plans for their time of death and can assist with funeral or memorial service planning . I am able to hold spiritual services within the home and can also oversee a funeral if needed.
Most importantly, I am able to be an advocate for the entire family during this difficult time. If something needs to be done, we will find a way while allowing you the time you deserve with your loved ones.
Please feel free to reach out to set up a free Zoom, or Phone video call to see if I am able to help you and your family.
