After the great achievement of publication of last international consensus statement (Head & neck, 10.1002/hed.26960. 23 Dec. 2021, doi:10.1002/hed.26960) on thyroid thermal ablative therapies, signed with important society around the world, new projects are in the pipeline.

An official TNT webinar will take place in spring, when world opinion leaders will share their ideas about the present and future of thyroid thermal ablation.

First of all

What about parathyroid glands?
Ultrasound guided interventional diagnostic and therapeutic procedures, mainly Fine Needle Aspiration and Ethanol Ablation, can be applied also in some parathyroid conditions.

Although, it is important to not think of Ethanol Injection as a main therapeutic tool in parathyroid condition.

The main application is the treatment of inoperable patients, with high surgical risk, who cannot undergo surgery.

First of all

What about parathyroid glands?
Ultrasound guided interventional diagnostic and therapeutic procedures, mainly Fine Needle Aspiration and Ethanol Ablation, can be applied also in some parathyroid conditions.

Although it is important to not think of FNA as a main diagnostic tool in parathyroid condition.

The main application is in identifying the tissue. Not always you can be certain that what you're looking at is parathyroid, and FNA, in some cases shown in this pics, can be helpful.

Parathyroid glands are one of the last organs identified: first description in 1852 in a dissection of an Indian rhinoceros and some years ago, due to lack of operatore experience and the quality of image, US evaluation was considered not effective for parathyroid glands.
Over the years, US became the mainstain for the morphological evaluation

Few tips on thyroid scintiscanning.

Isotopes that are selectively accumulated by thyroid tissue can be detected by a gamma camera.

Data are transformed into a visual display, and radioactivity in specific areas is quantified.

Main use is in defining areas of increased or decreased function (hot or cold areas) relative to the function of the remainder of the gland.

Almost all malignant nodules are hypofunctioning, but about 80% of benign nodules are also nonfunctioning.

Functioning nodules (hot nodules) are rarely malignant.

Scintiscanning with radioactive iodine can also be used to demonstrate that intrathoracic masses represent thyroid tissue and to detect ectopic thyroid tissue in the neck.

Thyroid nodules are generally benign hyperplastic (or colloid) nodules of benign follicular adenoma. However, multiple retrospective studies confirm that about 5-15% of clinically relevant nodules prova cancerous...

ACR TI-RADS is a reporting system for ultrasound evaluation of thyroid nodules proposed by the American College of Radiology (ACR). It consists in a standardized scoring system based on the features of the nodule on ultrasound examination, linked to recommendations for when to use fine needle aspiration (FNA) or ultrasound follow-up of suspicious nodules, and when to safely leave alone nodules that are benign/not suspicious.

Total score is determined from five categories of ultrasound findings:
- Composition
- Echogenicity
- Margin
- Shape
- Echogenic foci

If multiple nodules are present, choose the four highest-scoring nodules (not necessarily the largest) to be scored, reported, and followed up.

Iodine is essential for the production of normal quantities of thyroid hormone.

Few tips on the recommendend dietary daily intake from and main consequences of deficit.

During the last few years, several papers demonstrated the effectiveness of ultrasound assisted percutaneous Radiofrequency Ablation ( ) of cold solid benign thyroid nodules.

The procedure is carried out on outpatients. With no surgical scar, as a needle is used, RFA may reduce by 50%- 85% the volume of a benign thyroid mass in a single session.
The efficacy of volume shrinkage is related to the amount of nodular tissue ablated (destroyed) by the procedure.
Other factors (nodule shape, echostructure, intrinsic tendency to grow) may also influence the clinical outcome.
The shrinkage obtained by RFA is generally sufficient to eliminate or, at least, to better compressive symptoms and cosmetic discomfort, hence improving the health-related quality of life.

Thyroid nodules are close to vital structures, common carotid artery, internal jugular vein, vagus nerve, trachea, laryngeal recurrent nerve, brachial plexus, esophagus, muscles and strap muscles.
In order to save these structures, RFA must leave behind some viable tissue around them. Therefore, over the years, it could happen re-growth of the nodule and treatment repeated.

The publication in 2014 of the Thyroid Cancer Genome Atlas has been an important advance in thyroid nodules’ management.

New molecular techniques, including nextgeneration DNA and RNA sequencing, give the opportunity to analyze any given thyroid nodule, obtained via a thyroid fine needle aspirate and to predict with a high degree of certainty whether any given nodule is benign or malignant.
