Choice of OHA in MRCGP

မြန်မာ GP တွေ DM ကုတတ်ပါတယ်။ ဒါပေမယ့် စာမေးပွဲဖြေတဲ့အခါ မေးကွက်လေးတွေရယ် ၊ Guideline အတိုင်း ဘောင်ဝင်အောင်ကုနိုင်ဖို့ရယ်ကို MRCGP Exam မှာ Assess လုပ်ပါတယ်။

OHGP 5th Edition Page 320, 321 ဟာ အပိုင်ရနေရပါမယ်။
- DM Control ကို HbA1c ဘယ်လောက်နဲ့ ဖြတ်သလဲ။
- Mono therapy, Dual therapy, Triple therapy တွေမှာ ဘယ်ဆေးကို ဦးစားပေးပြီး အရင်ပေါင်းမှာလဲ

- Side Effects of OHAs
ဆိုပြီး မေးခွန်းတည်ဆောက်တတ်ပါတယ်။

Hypoglycemia SE ရစေတဲ့ ဆေးတမျိုးမျိုးပါနေရင် HbA1c ကို အမြင့်ထားပြီး control ရပါတယ်။ < 53 ပါ။ ဆေးတိုးတာက > 58

💥 Pioglitazone also increase risk of CA bladder, bone fracture, fluid retention and heart failure.

Sodium glucose cotransporter 2 inhibitor (SGLT2i) Gliflozins e.g Canagliflozin, Dapagliflozin, Empagliflozin.

ဒါ့ကြောင့် BMI များတဲ့လူနာ HbA1c တက်နေတယ် control မကောင်းဘူး ဆိုရင် SGLT2i တွေကို ဦးစားပေးရွေးရပါမယ်။

ဒီနေရာမှာ GLP-1 mimetics e.g exenatide, liraglutide, lixisenatide တွေကလည်း Weight ကျစေတယ် BMI > 35 ဆိုရင်ပေးမယ် ဆိုပေမယ့် Page 321 ကို သေချာကြည့်ရင် Triple Therapy Fail ပြီး Insulin ဟာ လူနာလုပ်ငန်းခွင်နဲ့ မသင့်တော်မှ သုံးရမယ် ဆိုတာ သိရပါမယ်။ တန်းသုံးလို့ မရပါ။ ဒါ့ကြောင့် ဦးစားပေးက SGLT2i ပါ။

မြန်မာ GP တွေ DM ကုတတ်ပါတယ်။ International Level ကုတဲ့အခါမှာ ယခင်လို ကုနေကျအတိုင်း Metformin + Gliclazide တခါတရံ Sitagliptip ပေးပြီး ကုနေရုံနဲ့ မလုံလောက်တော့ပါဘူး။ International Level နဲ့ လျော်ညီစွာ upgrade လုပ်ဖို့ လိုလာပါတယ်။

DM first line Management က Lifestyle Modification, healthy eating and exercise ဖြစ်ပါတယ်။
DM first line Drug က Metformin ဖြစ်ပါတယ်။
Control ရနေရင် ၆ လ ၁ ခါ HbA1c ဖောက်စစ်ရပါမယ်။

Latest Guidelines ဆိုရင် OHGP 5th Edition က နောက်ကျတတ်ပါတယ်။ Latest NICE Guidelines ကိုသာ အတည်ယူရပါမယ်။
