We are Volunteer Doctors from Cavite Medical Society. Due to the enhanced community quarantine, majority of hospital OPD and Doctors' Clinics have been closed.

We know that some patients might still need medical attention but we do not want to expose you to CoViD-19 at the hospital or clinic waiting area.

So to extend help to our frontliners and decongest hospital emergency rooms, We are accepting a FREE online consultation via CMS Face Book Page until the end of the community quarantine period from

We will do our best to answer your medical concern and will refer you to our volunteer specialists if our primary physicians deem it necessary.

Before agreeing to proceed with the online consultation, please read the following:

I agree to consult with CMS Volunteer Doctors by online consultation. I understand and accept that this is not the same as face-to-face consultation.

When the CoViD-19 crisis ended, I have to follow-up at the clinic for a consultation.

tinatanggap na hindi ito katulad ng harap-harapan na pagkonsulta.

My online consultation agreement includes sending a picture of the affected part of my body, as needed. I understand that the pictures I send will be confidential and will only be used in my treatment.

By using Online Consultation, I hereby agree on the following consequences (if applicable):

1.) In some instances the information transmitted may be of insufficient quality to allow for appropriate medical decision and treatment (i.e., poor resolution images, etc.) ;

2.) Delays in evaluation or treatment could occur due to telecommunication delay or failure of the electronic equipment;

3.) In some instances, a lack of access to all of your medical records may result in adverse drug reactions or allergic reactions or other judgement errors;

2.) You understand that the doctor abides the laws that protects the data privacy and security of health information;

3.) You understand your Doctor, in his or her sole discretion and professional judgment, may determine that telemedicine services are not appropriate for some or all of your treatment needs and, accordingly, may elect not to provide telemedicine services to you especially on cases that requires a face to face consultation.
