Hello everyone. Finally, some good news. COVID vaccine got approved in Canada. Let’s hope that works and things start getting back to normal. Now coming to the question which I posted last week. We got 2 answers (I was hoping for more answers but it's ok. Kudos to those who attempted). CORRECT ANSWER IS E
Coming to the question, the first rule of attempting these questions is to identify the concept which they are assessing you on. It seems in this question that they want to assess you on periodontal treatment. Right? WRONG. The concept they are assessing here is periodontal treatment planning. Let’s see how. First identify the key points- CROWN WITH SUBGINGIVAL MARGINS, 1MM OF ATTACHED GINGIVA, NO SIGN OF INFLAMMATION OR LOSS OF ATTACHMENT?
A. Root planing- It is the process by which residual embedded calculus and portions of cementum are removed from the roots to produce a smooth, hard, clean surface. The primary objective of scaling and root planing is to restore gingival health by completely removing elements that provoke gingival inflammation (i.e., biofilm, calculus, and endotoxin) from the tooth surface. THERE WAS NO SIGN OF INFLAMMATION.
B. Coronally positioned flap- The purpose of the coronally displaced flap procedure is to create a split-thickness flap in the area apical to the denuded root and position it coronally to cover the root. There was NO ATTACHMENT LOSS OR RECESSION SO NO COVERAGE WAS NEEDED.
C. Localized gingivectomy- The word gingivectomy means “excision of the gingiva.” The gingivectomy technique may be performed for the following indications: 1. Elimination of suprabony pockets, regardless of their depth, if the pocket wall is fibrous and firm. 2. Elimination of gingival enlargements. 3. Elimination of suprabony periodontal abscesses. NONE OF THESE ARE MENTIONED IN THE QUESTION
D. Autogenous connective tissue graft- Indicated for root coverage or increasing the width of attached gingiva. There is NO LOSS OF ATTACHMENT and DO WE REALLY NEED TO INCREASE THE WIDTH OF THE ATTACHED GINGIVA? HOW MUCH IS THE MINIMUM WIDTH OF ATTACHED GINGIVA NEEDED FOR HEALTHY PERIODONTIUM????
E. THERE IS NO INDICATION THAT THIS TOOTH REQUIRES PERIODONTAL TREATMENT- Now as discussed above choices A,B,C were definitely not indicated. The only point to think about in this question is whether 1mm of attached gingiva enough? Does the width of the attached gingiva needed to be increased?
Well, the explanation is-
Widening the attached gingiva accomplishes the following four objectives:
1. Enhances plaque removal around the gingival margin.
2. Improves esthetics.
3. Reduces inflammation around restored teeth.
4. Gingival margin binds better around teeth and implants with attached gingiva.
The width of the attached gingiva varies in different individuals and on different teeth of the same individual. The width of the attached gingiva is determined by subtracting the depth of the sulcus or pocket from the distance between the crest of the gingival margin and the mucogingival junction. NO MINIMUM WIDTH OF ATTACHED GINGIVA HAS BEEN ESTABLISHED AS A STANDARD NECESSARY FOR GINGIVAL HEALTH. People who practice good, atraumatic oral hygiene may maintain excellent gingival health with almost no attached gingiva. However, teeth with subgingival restorations and narrow zones of keratinized gingiva have shown HIGHER GINGIVAL INFLAMMATION SCORES than teeth with similar restorations and wide zones of attached gingiva and in these situations increasing the width of attached gingiva is an option. BUT IN THIS QUESTION, THERE IS NO GINGIVAL RECESSION OR INFLAMMATION, hence no treatment is indicated. (Newman, M. G., Takei, H. H., Klokkevold, P. R., & Carranza, F. A. (2018). Which of the following periodontal procedures is indicated on a maxillary canine that will receive a full crown with subgingival margins when the abutment has 1mm of attached gingiva, no sign of inflammation or loss of attachment?
A. Root planing.
B. Coronally positioned flap.
C. Localized gingivectomy.
D. Autogenous connective tissue graft.
E. There is no indication that this tooth requires periodontal treatment.
