PASSPORT REQUIREMENTS:

GENERAL REQUIREMENTS
• Filled up application form
• Personal appearance
• Confirmed appointment
• Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. (Report of Birth duly authenticated by NSO if born abroad)
• No need to submit a passport size photo
• Valid picture IDs and supporting documents to prove identity 2 pcs

For Married Women:
• Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or original and photocopy of the Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable.
• Report of Marriage duly authenticated by NSO if married abroad
For Minor Applicants (below 18 years old)
General Requirements:
• Confirmed appointment (except for 1 year old and below)
• Personal appearance of the minor applicant
• Personal appearance of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child)
• Original Birth Certificate of minor in Security Paper issued by NSO or Certified True Copy of Birth Certificate issued by the Local Civil Registrar and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable. Report of Birth duly authenticated by NSO is required if minor was born abroad.
• Document of identity with photo, if minor is 8-17 years old (for first time and renewal applicant) such as School ID or Form 137 with readable dry seal
• For minor applicants who never attended school, a Notarized Affidavit of Explanation executed by either parent (if minor is a legitimate child) / by mother (if minor is an illegitimate child) detailing the reasons why the child is not in school, is required
• Marriage Certificate of minor’s parents duly authenticated by NSO
• Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child)
• Original and photocopy of valid passport of the person traveling with the minor
• Photocopy of valid passport of either parent (if minor is a legitimate child) / of mother (if minor is an illegitimate child) or identification documents (Please refer to List of Acceptable IDs)
Additional Requirements:
If minor is not traveling with either parent or alone
• Personal appearance of the mother is required if minor applicant is an illegitimate child.
• Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion and relationship to the minor. If minor will be traveling alone, ASC must be executed by either parent, stating that his/her child will be traveling alone. If minor is illegitimate, mother should execute the ASC.
• Original and photocopy of DSWD Clearance
• There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.

Flight details as well as bookings are through the website, www.cebupacificair.com. For added flexibility, new bookings include CEB Flexi for free. This allows passengers to rebook their flights up to two times, giving peace of mind and ease to move travel dates. Meanwhile, for passengers with existing bookings, they may rebook to any of the above-mentioned flights through the “Manage Booking” page on the website, http://bit.ly/CEBmanageflight.

This is a developing situation and we will have an agile approach to rebuilding our network schedule. It may be necessary for us to add or cancel flights at last minute given the fluidity of restrictions and directives from the Inter-Agency Task Force (IATF), the Local Government Units (LGU) and other concerned government agencies.

We will provide updates through our website and official social media accounts and endeavor to inform our passengers in a timely manner.

We strongly urge passengers to check guidelines of the IATF, or with the LGU of origin and destination. Passengers may be asked to present proof of purpose of travel and other requirements upon entering the airport or at arrival at the destination.

CEB has implemented new flexibility policies to give passengers peace of mind in the event their flights are cancelled. They have the following options:

1. Free rebooking
Rebook to any other travel date within three (3) months. Change (rebooking) fees and fare difference are waived.

2. Full Travel Fund
Place the full cost of the ticket in a Travel Fund valid for one (1) year. Use the Travel Fund within one (1) year—either book a flight up to one (1) year ahead, or pay for add-ons (e.g. baggage allowance, seat selection, etc.)

If the Travel Fund is not used within one (1) year, passengers can apply for a full refund.

3. Full refund
Processing of refunds will start after the Community Quarantine is lifted and regular work schedules resume. However, due to the unprecedented volume of requests for refunds, the process will take as long as three (3) to four (4) billing cycles.

iba iba po ng experience ang narineg ko sa mga Biyahero natin..

2. naka kuha kanga ng ticket kaso pag dating sa LGU/PNP
di kanaman pinayagang maka kuha ng TRAVEL Authority
dahil sa ibang kadahilanan,,, ex. BABY, BATA, SENIOR, BUNTIS, eto ung di pa malinaw na pwedeng maka biyahe.

kaya maaring masayang ticket mo....

Sila po ang mag decide kung makaka biyahe po kayo...

B. sa aming mga TICKETING OUTLET..
mas okay po samin na my travel authority and medical cert napo kayo... para sure na makaka biyahe...

ang CONS lang dito... maaring mahal na ung ticket na makukuha niyo, lalot kung malapit na ang biyahe niyo...

mas okay po kung OPEN DATE ang permit/pass

C. ang MEdical Certificate naman mas okay kunin agad ito...

D. ang Aming advice
1. magtanong sa LGU/PNP, kung ilang araw bago makuha ang Travel Authority at medical certificate,

2. pag nalaman mo na kung ilang araw.. saka tayo mag pabook,,, na lagpas sa bilang ng araw bago mo makuha ang PERMIT/PASS

3. saka tayo mag pabook ng ticket.. kasabay ang pag kuha ng MEDICAL CERTIFICATE, TRAVEL AUTHORITY na naka ayon sa ticket na inyong nabili

4. inadvice din po namin na hangat maari wag po mag madali ng pag BIYAHE, dadagdsa po kasi ang mga tao sa
pagkuha ng permit,,, at madugo po iyon..
mas okay na kumuha ng ticket na malayo pang araw ng biyahe... mga JUNE 15 pataas....

4. kung pwede din i open date ang TRAVEL authority mas okay, kasi maaring maKANSELA padin ang mga biyahe...
atleast my buffer date po tayo...

- Areas in GCQ/MGCQ ay may selected flights na. Pero gaano ba ka difficult/easy to reach the airport baka po kasi hindi maka rating sa airport dahil naharang sa checkpoint.

2. CHECK THE PASSING BORDER PROTOCOL hangga't maaari.
- Baka sa day of travel ka na kukuha ng Travel Pass. Marami po ang requirements. Refer to your Local Units for the Borders Pass requirements.

3. NASA TAMANG EDAD AT MAAYOS NA KONDISYON KA BA?
- May mga restricted sa GCQ/MGCQ strict rules.

4. PURPOSE OF TRAVEL
- this time very important travels lang po ang allowed, kung ang purpose ng travel niyo is gumala lang hindi po pwede(until further notice).

5. ALWAYS BRING EXTRA CASH and MASK
- baka may delay or i quarantine pa pagdating sa destination o di kaya hindi papasukin dahil walang mask.
